Antiarrhythmic Drug Therapy for Acute Management of Atrial Fibrillation and Atrial Flutter.

This review outlines the pharmacologic strategies for acute management of atrial fibrillation and atrial flutter, with a focus on rate and rhythm control. It details the efficacy, indications, and safety profiles of commonly used agents, including beta-blockers, calcium channel blockers, and antiarrhythmic drugs such as amiodarone, ibutilide, dofetilide, and flecainide. The article highlights patient-specific considerations, emerging therapies, and the need for safer, faster acting drugs as atrial fibrillation prevalence rises. Both traditional and novel approaches are discussed to guide evide

[Actual and theoretical points of view in the application of coronary therapeutic agents].

The medicamentous therapy of the angina pectoris vera and of the chronic ischaemic heart disease is at present based on three groups of medicaments: nitrate compounds, beta-blocking agents and calcium antagonists. The underlying therapeutic principle which is common for them consists in the reduction of the oxygen requirement of the myocardium so that an improvement of the complaints and a larger load capacity may be achieved. The improvement may be objectified also at the behaviour of the haemodynamics and the ECG under load. The so-called coronary dilating remedies and the beta-stimulators d

Enhancing Treatment Outcomes in Atrial Fibrillation: A Contemporary Literature Review of the Impact of Optimizing Patient Well-Being in Treatment Management.

Atrial fibrillation (AF) is the most common heart rhythm disorder, defined by an irregular and rapid heartbeat. It is the most prevalent cardiac arrhythmia in the United States, characterized by irregular heartbeats due to asynchrony between atrial and ventricular contractions. AF can be categorized as paroxysmal or persistent and, as such, poses significant health risks, including heart failure and stroke. Factors like age, sex, lifestyle, and existing health conditions elevate AF risk. There have been a lot of debates around AF risk management and its impact on prognosis. This literature rev

EEG Alpha-Rhythm-Related Changes in BOLD fMRI Signal in Neurofeedback Training.

Interaction of EEG and BOLD brain activity was studied in subjects during EEG-biofeedback training course (20 sessions). Healthy male subjects aged 20-35 underwent a training course of sound-reinforced upregulation of alpha- (20 participants) or beta-activity (9 participants). Pretraining, intermediate (after 10 sessions), and post-training fMRI-EEG recordings were conducted in resting state and during the participants' attempts to upregulate the power of target EEG activity. Regression analysis was carried out on three sessions in total; the main changes in BOLD signal connected with alpha rh

[Melatonin in dermatology. Experimental and clinical aspects].

Melatonin (N-acetyl-5-methoxytryptamine) is a hormone with multiple functions in humans, produced by the pineal gland and stimulated by beta-adrenergic receptors. Serum melatonin levels exhibit a circadian rhythm with low levels during the day, rise in the evening and maximum levels at night between 2 and 4 a.m. Melatonin participates in the regulation of several physiological processes such as seasonal biological rhythm, daily sleep induction, aging and modulation of immunobiological defence reactions. Furthermore, melatonin has a highly lipophilic molecular structure facilitating penetration

Beta oscillatory neurons in the motor thalamus of movement disorder and pain patients.

Excessive beta oscillations (15-25Hz) in the basal ganglia have been linked to the akineto-rigid symptoms of Parkinson's disease (PD) although it remains unclear whether the underlying mechanism is causative or associative. While a number of studies have reported beta activity in the subthalamic nucleus and globus pallidus internus, relatively little is known about the beta rhythm of the motor thalamus and its relation to movement disorders. To test whether thalamic beta oscillations are related to parkinsonian symptoms, we examined the spectral properties of neuronal activity in the ventral t

Beta Oscillations in the Sensory Thalamus During Severe Facial Neuropathic Pain Using Novel Sensing Deep Brain Stimulation.

OBJECTIVE: Advancements in deep brain stimulation (DBS) devices provide a unique opportunity to record local field potentials longitudinally to improve the efficacy of treatment for intractable facial pain. We aimed to identify potential electrophysiological biomarkers of pain in the ventral posteromedial nucleus (VPM) of the thalamus and periaqueductal gray (PAG) using a long-term sensing DBS system. MATERIALS AND METHODS: We analyzed power spectra of ambulatory pain-related events from one patient implanted with a long-term sensing generator, representing different pain intensities (pain >7

The role of circadian rhythm in choroid plexus functions.

For several years, a great effort has been devoted to understand how circadian oscillations in physiological processes are determined by the circadian clock system. This system is composed by the master clock at the suprachiasmatic nucleus which sets the pace and tunes peripheral clocks in several organs. It was recently demonstrated that the choroid plexus epithelial cells that compose the blood-cerebrospinal fluid barrier hold a circadian clock which might control their multiple functions with implications for the maintenance of brain homeostasis. However, the choroid plexus activities regul

[Effects of low frequency repetitive transcranial magnetic stimulation on Electroencephalograph rhythm of children with autism].

Autism spectrum disorders (ASD) is a complex developmental disorder characterized by impairments in social communication and stereotyped behaviors. Electroencephalograph (EEG), which can measure neurological changes associated with cortical synaptic activity, has been proven to be a powerful tool for detecting neurological disorders. The main goal of this study is to explore the effects of repetitive transcranial magnetic stimulation (rTMS) on behavioral response and EEG. We enrolled 32 autistic children, rTMS group ( n = 16) and control group ( n = 16) and calculated the relative power of the

Circadian genes, the stress axis, and alcoholism.

The body's internal system to control the daily rhythm of the body's functions (i.e., the circadian system), the body's stress response, and the body's neurobiology are highly interconnected. Thus, the rhythm of the circadian system impacts alcohol use patterns; at the same time, alcohol drinking also can alter circadian functions. The sensitivity of the circadian system to alcohol may result from alcohol's effects on the expression of several of the clock genes that regulate circadian function. The stress response system involves the hypothalamus and pituitary gland in the brain and the adren

Propafenone: an effective agent for the management of supraventricular arrhythmias.

Propafenone is a sodium channel blocking antiarrhythmic drug. It also has beta-adrenergic, potassium channel, and weak calcium channel blocking activity. The drug is metabolized in the liver with rates dependent on the debrisoquin phenotype. The saturable metabolism results in nonlinear pharmacokinetics. The metabolites retain sodium channel blocking activity but little beta-adrenergic blocking activity. Both controlled and noncontrolled studies have documented its efficacy in a variety of supraventricular arrhythmias. Intravenous propafenone is effective in converting atrial fibrillation to n

Acoustic Neurofeedback Increases Beta ERD During Mental Rotation Task.

The purpose of the present study was to identify the effect of acoustic neurofeedback on brain activity during consecutive stages of mental rotation of 3D objects. Given the fact that the process of mental rotation of objects is associated with desynchronisation of beta rhythm (beta ERD), it was expected that suppression in this band would be greater in the experimental group than in the controls. Thirty-three participants were randomly allocated to two groups performing the classic Shepard-Metzler mental rotation task (1971). The experimental group received auditory stimuli when the level of

Treatment of supraventricular tachyarrhythmias associated with hyperthyroidism by radioiodine, amiodarone and propylthiouracil.

Beta-blockers and calcium antagonists have been advocated for thyrotoxicosis induced tachyarrhythmias. Amiodarone is generally considered as contraindicated because of its high iodine content. Since amiodarone combined with propylthiouracil induced a greater fall in serum thyroid hormone concentrations than propylthiouracil alone, we treated 2 hyperthyroid patients with supraventricular arrhythmias by radioiodine (day 0) followed after 24 h by amiodarone and propylthiouracil. Serum T3 was normalized on day 2 (patient 1) and 3 (patient 2). Effective t1/2 of intrathyroidal 131I were 6.6 and 4.3

Proportional feedback stimulation for seizure control in rats.

PURPOSE: A responsive electrical brain stimulation system using control feedback was investigated for the treatment of seizures. METHODS: A proportional feedback stimulation system was designed. Penicillin-induced episodic seizures were created in rat primary motor cortex. Both intracranial (proximal to seizure focus) and extracranial EEGs were monitored. Current stimulation was applied at the seizure focus by using the intracranial EEG as the current-stimulus template. Different gains (H) for determining feedback stimulus amplitudes were tested. RESULTS: The effect of feedback stimulation o

Modulation of Non-Rhythmic Temporal Prediction by Subthalamic Nucleus Deep Brain Stimulation (STN-DBS) in Parkinson's Disease.

BACKGROUND: Accurate temporal prediction, essential for adaptive motor behavior, relies on corticobasal ganglia circuits. In Parkinson's disease (PD), both motor and non-motor functions are impaired. Deep brain stimulation (DBS) of the subthalamic nucleus (STN) effectively alleviates motor symptoms, but its effects on non-motor domains, like temporal prediction, remain less understood. OBJECTIVE: This study aimed to investigate how STN-DBS influences temporal prediction performance and its underlying oscillatory dynamics in PD patients, with a particular focus on beta band power and delta ban

Stimulation selectively blocks subthalamic high β-cortical high γ coupling in Parkinson's disease.

Subthalamic stimulation in Parkinson's disease may operate by suppressing high-beta connectivity along the hyperdirect pathway and by promoting gamma motor cortical processing. We examined how the phase of beta activity shapes the amplitude of the gamma rhythm along hyperdirect connections and how stimulation influences this coupling. Thirty-eight patients with akinetic-rigid Parkinson's disease treated with bilateral subthalamic stimulation were recruited. A high-density electroencephalogram was recorded at rest and while patients drew self-paced and traced spirals on a digital tablet at four

Circadian Rhythm Disruption and Sleep Disorders in Alzheimer's Disease: Mechanistic Insights and Therapeutic Potentials.

Alzheimer's Disease (AD) is the leading cause of dementia worldwide, with significant cognitive and behavioural impairments that devastate individuals and their families. Cohort-level findings, demonstrate the broader population-level implications of Sleep and Circadian Rhythm Disruption (SCRD) in AD and underscore the need for early interventions, emphasizing the importance of timely action. However, the mechanism remains unclear. SCRD impairs the glymphatic system, which is responsible for the clearance of neurotoxic proteins such as amyloid-β and tau during slow-wave sleep, accelerating neu

Swallowing-related neural oscillation: an intracranial EEG study.

OBJECTIVE: Swallowing is a unique movement due to the indispensable orchestration of voluntary and involuntary movements. The transition from voluntary to involuntary swallowing is executed within milliseconds. We hypothesized that the underlying neural mechanism of swallowing would be revealed by high-frequency cortical activities. METHODS: Eight epileptic participants fitted with intracranial electrodes over the orofacial cortex were asked to swallow a water bolus and cortical oscillatory changes, including the high γ band (75-150 Hz) and β band (13-30 Hz), were investigated at the time of

Resting-state connectivity predicts visuo-motor skill learning.

Spontaneous brain activity at rest is highly organized even when the brain is not explicitly engaged in a task. Functional connectivity (FC) in the alpha frequency band (α, 8-12 Hz) during rest is associated with improved performance on various cognitive and motor tasks. In this study we explored how FC is associated with visuo-motor skill learning and offline consolidation. We tested two hypotheses by which resting-state FC might achieve its impact on behavior: preparing the brain for an upcoming task or consolidating training gains. Twenty-four healthy participants were assigned to one of tw
