Three patients with mucoepidermoid carcinoma of the bile duct are described. In two patients the tumor arose from the common hepatic duct. Mucoepidermoid carcinoma of the extrahepatic bile duct has not been previously reported. The clinical features and pathologic behavior of this rare type of tumor are similar to the usual biliary adenocarcinoma. The coexistence of Clonorchis sinensis infestation and primary pyogenic cholangitis raises the possibility of an etiologic association.

The records of patients with carcinoma of the extrahepatic bile ducts, including the ampulla of Vater, operated on at the University of Rochester Medical Center between 1960 and 1980 were reviewed. All 47 patients had similar manifestations and laboratory findings; percutaneous transhepatic cholangiography and endoscopic retrograde cholangiopancreatography were the two studies that consistently localized the tumor. In a case of congenital absence of the common bile duct, the common hepatic duct emptied directly into the gallbladder; the latter drained by a long cystic duct into the second part of the duodenum. To my knowledge, only eight prior cases with this or very similar anomalies have been reported in the literature. This rare arrangement is common in certain lower animals and is explainable by embryologic studies. Case histories of 47 patients with histologically confirmed carcinoma of extrahepatic bile ducts presenting to M. D. Anderson Hospital and Tumor Institute over a 30-year period were studied. The eighth report case of choledochocele is described. This lesion is a cyst communicating with the terminal portion of the common bile duct. It is characteristically lined with duodenal mucosa and is probably a form of duplication of the duodenum. The surgical treatment is marsupialization of the cyst to the interior of the duodenum.

Our experience with five cases of cystic dilatation of the extrahepatic bile ducts is reported and compared with the literature. The following conclusions have been reached: (1) The etiology of this anomaly is primarily congenital but may involve an acquired component. Microbial populations of the small bowel and bile salt metabolism were studied in 15 patients with lesions of the stomach and small intestine. These types of microorganism could be correlated with the site and extent of stasis in the small bowel and the presence of a normally functioning stomach.
