Mechanical Failures in Magnetic Intramedullary Lengthening Nails.

BACKGROUND: Magnetic intramedullary lengthening nails (MILNs) have gained popularity in recent years for the treatment of limb-lengthening discrepancies. We sought to catalog mechanical failures and their prevention and management in a large, single-institution series. We specifically assessed the rate of mechanical failures, the types of failures observed, and management strategies. METHODS: We retrospectively reviewed 377 patients (420 limbs) who underwent limb lengthening with early (P1) or later (P2) MILN iterations with ≥12-month follow-up. Mechanical failure was defined as mechanical br

Anterior lengthening in scoliosis occurs only in the disc and is similar in different types of scoliosis.

Relative anterior spinal overgrowth (RASO) was proposed as a generalized growth disturbance and a potential initiator of adolescent idiopathic scoliosis (AIS). However, anterior lengthening was also observed in neuromuscular (NM) scoliosis, was shown to be restricted to the apical areas and to be located in the intervertebral discs, not in the bone. In this study the goal was to determine if other scoliotic curves of known origin exhibit the similar mechanism of anterior lengthening without changes in the vertebral body. Therefore CT-scans of 18 patients in whom a short segment congenital malf

Locked intramedullary nailing. Its application to femoral and tibial axial, rotational, lengthening, and shortening osteotomies.

Interlocking nailing is an alternative method of internal fixation following corrective osteotomies for malunions or after correction of leg length inequality. Of 13 osteotomies (six femoral, seven tibial) for angular or rotational malunion, all healed following dynamic locked nailing. Eleven were considered anatomic and two had mild residual deformity. One-stage femoral lengthening was performed in 17 patients. The preferred operative technique includes a long Z-shaped osteotomy, static interlocking nailing, primary cancellous bone grafts, and one or two supplemental screws at the osteotomy s

Simultaneous maxillary and mandibular distraction osteogenesis with a semiburied device.

Distraction osteogenesis is a technique utilizing natural healing mechanisms to generate new bone; it is commonly used to lengthen the hypoplastic mandible. Distraction of the maxilla and mandible as a unit is an obvious extension of the technique. We describe the application of a semiburied distractor to simultaneously lengthen the mandible and maxilla and level a canted occlusal plane in three cases. The indications for bimaxillary distraction are reviewed, including its advantages, disadvantages and limitations.

Human WDR5 promotes breast cancer growth and metastasis via KMT2-independent translation regulation.

INTRODUCTION: The authors have applied the method of progressive lengthening to correct upper limb discrepancy resulting from arm reimplantation in a young female adult. MATERIAL: Following a motor vehicle accident, a twenty one years old female had her right arm sectioned. Four hours after the accident, the limb was reimplanted with 10 cm shortening of the humeral shaft, to allow direct nerve suture. Sixteen months later, she had excellent functional recovery, but was annoyed by the aesthetic appearance of her short arm. Humeral lengthening was undertaken. METHOD: The authors used a special

Lengthening in congenital posteromedial bowing of tibia: a follow-up series at skeletal maturity.

PURPOSE: Congenital posteromedial bowing (CPMB) of tibia is a rare, usually self-resolving condition that is present at birth. The data on the long-term outcome of lengthening, choice of fixator, outcome in early and late age groups, and complications is sparse, hence the need for this study. METHODS: This retrospective study included 93 patients with 94 affected legs from 1991 to 2023. The patients were categorized into early (under 10 years) and late (10 years or above) intervention groups. Eighteen patients with nineteen lengthening episodes who attained skeletal maturity made the basis of

Plantar approach for treatment of brachymetatarsia: A novel approach with an aesthetic consideration.

In the past, open osteotomy was always performed through a dorsal approach in the surgical treatment of brachymetatarsia, which created scar formation on the dorsal skin, subsequently resulting in dissatisfaction with cosmetic results. In this study, we provided a plantar approach to avoid forming scars on the dorsal side. A retrospective review was conducted in nine patients (13 feet) with brachymetatarsia treated with an open osteotomy and gradual bone lengthening through a plantar approach. Visual analogue scale (VAS) was used to evaluate the satisfaction of foot appearance, and we designed…

Technique tip: Simultaneous first metatarsal lengthening and metatarsophalangeal joint fusion for failed hallux valgus surgery with transfer metatarsalgia.

BACKGROUND: Failed hallux valgus surgery may result in residual or recurrent hallux valgus, and as well transfer metatarsalgia. The present technical tip concerns the combination of fusion of the first metatarsophalangeal (MTP) joint and lengthening of the first metatarsal (MT) through a scarf osteotomy. MATERIALS AND METHODS: Six patients underwent the presented technique, all for the indication of failed hallux valgus surgery with shortening of the first MT and degenerative changes in the 1st MTP joint. RESULTS: Follow-up at six months revealed all patients had complete healing of the oste

A 50-year experience with bone graft epiphysiodesis in the treatment of slipped capital femoral epiphysis.

A "millennium" update of all cases of slipped capital femoral epiphysis treated by bone graft epiphysiodesis between 1950 and 2000 was conducted. All cases were followed for at least 1 year to evaluate the occurrence of re-slippage, avascular necrosis (AVN), chondrolysis, or complications secondary to the surgical approach. This study adds 109 patients and 133 hips to the initial report for a total of 268 patients and 318 hips. The patients include 43 patients with 45 acute slips and 225 patients with 273 chronic slips. The results include three cases of AVN, six cases of re-slippage, one case

C-Type Natriuretic Peptide Derivatives: Orthopaedic Implications in Achondroplasia.

UNLABELLED: The recent development of C-type natriuretic peptide (CNP) analogs to increase growth velocity in achondroplasia is now being used alongside limb lengthening procedures. Animal studies have shown that CNP analogs may affect bone density and the speed of callus consolidation during fracture healing. Although achondroplasia is primarily associated with a defect in endochondral ossification, pharmaceutical treatments for the condition may also influence intramembranous ossification. The availability of these nonsurgical treatments warrants further investigation into their potential im

Plating after lengthening in treating phalangeal and metacarpal deficiency: An alternative method.

OBJECTIVES: This study aims to investigate whether plating after lengthening in patients with phalanges and metacarpals deficiency could significantly shorten the duration of external fixation and decrease bone healing index. PATIENTS AND METHODS: Between February 2010 and December 2018, 11 phalanges in nine patients (6 males, 3 females; mean age: 28.4±4.4 years; range, 22 to 35 years) and nine metacarpals in six patients (2 males, 4 females; mean age: 21.0±2.9 years; range, 16 to 25 years) were lengthened at a rate of 0.25 mm in two increments. A unilateral external fixator was applied in al

The opening base wedge osteotomy and subsequent lengthening of the first metatarsal: an in vitro study.

UNLABELLED: Traditionally, the opening base wedge osteotomy has been indicated in a moderate to severe hallux abducto valgus deformity with a short first metatarsal. This in vitro study aimed to address the question of how much lengthening is inherent to the geometric design of an opening wedge in the first metatarsal. The preosteotomy length of a first metatarsal segment was compared with postosteotomy length after performing transverse and oblique basilar osteotomies while maintaining the opening wedge with a prefabricated spacer. In the current bench study, it was found that the opening bas

Monobloc craniomaxillofacial distraction osteogenesis in a newborn with severe craniofacial synostosis: a preliminary report.

Severe craniofacial synostosis can be a devastating problem for a newborn infant. Reasons for early surgical intervention include cranial stenosis, hydrocephalus, inadequate globe and corneal protection, compromised airway patency, and feeding problems. In this preliminary report, we describe the management of severe craniofacial synostosis in a newborn infant by means of cranial and midfacial distraction osteogenesis.

Preliminary studies of mineralization during distraction osteogenesis.

Distraction osteogenesis by the Ilizarov method was performed on 20 dogs. Mineralization at the site of the left tibial metaphyseal lengthening was measured by weekly quantitative computer tomography (QCT) using the contralateral tibia as a control. Four dogs each were killed on Days 7, 14, 21, and 28 of distraction in order to correlate QCT with microradiology, nondecalcified histology, quantitative calcium analysis, and scanning electron microscopy. It was consistently found that intramembranous ossification proceeded centripetally from each corticotomy surface toward the central fibrous int

Comparison of orthofix external fixator combined with vascularized fibula graft versus single segmental femoral lengthening for treating femoral defects ≥ 6 cm: a retrospective study.

BACKGROUND: Bone lengthening and free vascularized fibula graft (FVFG) represent two prevalent modalities for bone defect repair. The objective of this study was to compare the efficacy disparities between FVFG and single - segmental bone lengthening (BL) when the Orthofix external fixator was utilized in the treatment process. METHODS: The follow - up data of 72 patients with femoral defects, who received treatment from January 2009 to May 2019, were collected for retrospective analysis. Among them, 60 comparable patients were stratified into two groups: one group of 30 patients received fre…

[Thumb replacement operation using metacarpal distraction osteotomy as a secondary intervention after thumb amputation].

After thumb amputation distal to the MCP-1 joint, good results can be achieved with lengthening of the first metacarpal bone by an external fixateur if a correct technique is applied. In all our patients the lengthened thumb was mechanically stable and sensitive, thus improving the patient's ability to use the hand to grasp. Another advantage of this procedure is the relative simplicity of the method. Furthermore, the original nerve and vascular supply can be preserved.

What Are the Risks and Functional Outcomes Associated With Bilateral Humeral Lengthening Using a Monolateral External Fixator in Patients With Achondroplasia?

BACKGROUND: Many patients with achondroplasia experience functional impairments because of rhizomelic upper extremities (proximal limb shortening). Bilateral humeral lengthening may overcome these functional limitations, but it is associated with several risks, such as radial nerve palsy and insufficient bone regeneration. Only a few studies have reported on patient satisfaction and functional outcome after humeral lengthening in patients with achondroplasia. Furthermore, the reported numbers of adverse events associated with lengthening procedures using external fixators vary widely. QUESTIO
