Proximal femoral reconstruction after aseptic loosening following proximal femoral replacement for Ewing sarcoma: a case report with one-year follow-up.

We report the case of a 30-year-old patient initially treated for a proximal femoral Ewing's sarcoma when 12 years old. Index treatment comprised tumour resection and total hip arthroplasty. Two years later revision for aseptic loosening was performed. Subsequently, six further surgical revisons were performed for varying causes. At the age of 23 years the proximal femur was resected and a proximal femoral endoprosthesis implanted.Eighteen years after initial diagnosis the patient presented with recurrent aseptc loosening. Both the proximal femur and acetabulum were reconstructed. For acetabul

Arthroscopic Bone Graft and Internal Fixation of Non-Union of the Proximal Pole of the Scaphoid: Surgical Technique and Outcomes.

Background  Fractures of the proximal pole of the scaphoid have been associated with delayed union, non-union, and avascular necrosis. This has been attributed to avascularity of the proximal pole of the scaphoid. While proximal pole non-unions can be successfully treated using open techniques, there is little information in the literature regarding arthroscopic bone graft and internal fixation of proximal pole non-unions. Description of Technique  After insertion of a 1.2-mm radiolunate K-wire, the scaphoid non-union was arthroscopically excised, bone grafted with iliac crest cancellous bone,

Gustilo Type III Open and Comminuted Tibial Fractures Managed by Simultaneous Combination of a Free Latissimus Dorsi Muscle Flap and Ilizarov External Fixation: A Case Report.

CASE: A 70-year-old woman who sustained Gustilo type III open and comminuted tibial fractures presented with extensive soft-tissue defect. Definitive surgery was performed using a free latissimus dorsi muscle flap for the extensive soft-tissue defect and Ilizarov external fixation (IEF) to stabilize the fractures and arthrodese the ankle. Ankle arthrodesis was accomplished by the wires penetrating the implanted muscle flap. CONCLUSION: The combined free flap and IEF management protocol described in this report was effective in achieving early weight-bearing and prompt bone healing in an elder

Grade III avulsion fracture repair on the UCL of the proximal joint of the thumb.

A technique is presented for repair of grade III avulsion fractures of the ulnar collateral ligament when the bony avulsion fragment in the thumb is too small or too comminuted to allow simple repair. The case presented demonstrates that a single suture-to-bone anchor can provide secure ligament fixation without the need for external pull-out wires or buttons and can make possible an early return to activity.

Dislocation of the lunate, triquetral, and hamate bones. Case report.

A patient sustained multiple injuries during a road traffic accident, including closed palmar luxation of the lunate together with dislocation of the triquetrum and hamulus of the hamate bone. Initially the injury was treated closed, but redislocation necessitated closed reduction and stabilisation with K-wires. Three years later movement was painless and slightly restricted despite persisting palmar dislocation of the lunate and triquetral bones as well as ulnar translocation.

Mother's fibula in son's forearm: use of maternal bone grafting for aneurysmal bone cyst not amenable to curettage - a case report with review of literature.

It has always been a challenge to reconstruct large bone gaps. The aim of this case report is to highlight the success of homologous maternal bone grafting in a large cystic lesion. A six and half years old boy presented to us with an aneurysmal bone cyst (ABC) of the right radius, not amenable to curettage. We excised the lesion in toto, which created an 11 cm bone loss. Considering the age of the patient, we reconstructed the bone gap with maternal fibular graft. Accordingly, 12 cm of fibular graft was harvested and fashioned to fit into the bone gap. It was fixed with an intramedullary K-wi

Salter-Harris II distal radial fractures treated with the intrafocal K-wire technique: a retrospective study of 20 cases.

We retrospectively reviewed 20 children with Salter-Harris II fractures of the distal radius treated with intrafocal pinning according to Kapandji. Unlike conventional crossed K-wire pinning, this method avoids crossing the physis with the pins and thus minimizes the risk of physeal damage. None of the children had fracture re-displacement, angulation or growth deformities. The age, sex, side of injury, fracture displacement or angulation, treatment delay or immobilization period had any significant impact on the functional outcome. At a mean follow-up of 49 months (range 47-60), plain radiogr

New sternal closure methods versus the standard closure method: systematic review and meta-analysis.

OBJECTIVES: This study aimed to evaluate, by means of a systematic review, the efficiency of new methods for sternal closure in order to prevent sternal wound complications after sternotomy. METHODS: The method of study was a systematic review of randomized clinical trials. We included studies that used rigid plates, thermoreactive clips, cables and flat wires, in comparison with the standard closure method. Patients included adults, regardless of gender and race. RESULTS: Seven clinical trials were included involving 1810 patients. Five trials were carried out in the USA, 1 in Australia and

Volar locking plating versus percutaneous Kirschner wires for distal radius fractures in an adult population: a meta-analysis.

BACKGROUND: This paper compares volar locking plates (VLPs) and percutaneous Kirschner wire (K-wire) fixation for distal radius fractures in an adult population up to and beyond 12 months. The aim of this review is to compare functional and radiological outcomes along with complication rates. A meta-analysis was performed to investigate this. METHODS: A systematic review and meta-analysis was performed using the PRISMA guidelines. A search of major databases (MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials (CENTRAL) and CINAHL) was performed up to November 2017. Prospective an

Distal tibia nonunions.

Although tibia metaphyseal nonunion is rare, its treatment is often complex. The merits of related management techniques are discussed. These techniques include: intramedullary nailing, fine wire fixation, and blade plate reconstruction, which is the method preferred by the authors.

[Modified traction cerclage in fractures of the outer ankle].

The authors describe two techniques of osteosynthesis which can be used as the method of choice in suitable types of fractures. Both were elaborated as the author's modification in 1978 and used in the course of 10 years in 24 instances. They can be applied without special instrumental equipment in any department. The authors consider them very stable without requiring additional plaster immobilization and the authors use early rehabilitation immediately after operation. The authors never observed usuration of the bone beneath the cerclage or slower healing, pseudoarthrosis or any other compli

Nasal spine suspension for reduction and immobilization of mandibular fractures.

Successful treatment of mandibular fractures involves proper fracture reduction and immobilization for an adequate length of time. A simple wire fixation technique that can be used in many situations involves suspension wiring from the base of the anterior nasal spine to a pair of circummandibular wires. The technique may be used alone or adjunctively with other methods of fixation. It offers several advantages over other methods, particularly in the treatment of pediatric mandibular fractures.

Self-inflicted partial epiphyseolysis of the distal femur-a case report.

INTRODUCTION: Injuries to the epiphyseal plate are of great concern as they can affect bone growth. Although epiphyseal fractures are common in adolescents, fractures of the distal femoral epiphyseal plate are rare. CASE PRESENTATION: We present a case of a Salter-Harris type 1 fracture of the distal epiphysis of the femur that was self-inflicted by a patient with paraplegia due to spina bifida. The patient was brought to the pediatrician's attention during a routine checkup with an apparent swelling of the right thigh. Upon presentation, we performed a radiograph and an additional MRI, which

Suture osteosynthesis in the bony reconstruction of thumb duplication.

Metacarpal osteotomies are done to correct deviation deformity in thumb duplication. We describe a suture-only technique of metacarpal osteosynthesis, without using K-wires. Thirteen Flatt Type IV thumbs and five Wassel Type VII thumbs were reconstructed with this technique. The median follow-up was 23 months. After osteotomy, the metacarpal bone fragments were sutured together with 5-0 polyglactin or 4-0 polydioxanone sutures. Metacarpal fragment displacement was not observed on postoperative radiographs obtained at 1 and 2 weeks. Bony union was achieved at 6 weeks without loss of alignment.

Are bone bruises a possible cause of osteochondritis dissecans of the capitellum? A case report and review of the literature.

A 17-year old soccer-player sustained a fracture and dislocation of the ulnar epicondyle combined with a bone bruises at the radial head and the capitellum. An open reduction and internal fixation was performed using two K-wires. Initial recovery was uneventful. After the operation he was discharged home and reviewed on a regular basis. When bony union had occurred the two K-wires were removed. However, on follow up he continued to complain of pain on the radial aspect of the joint and did not regain his normal range of movement. A further MRI was performed. Now a grade II osteochondritis diss

Anatomical Structures at Risk After Medial Column Nailing.

BACKGROUND: Medial column nails have been introduced into Charcot's neuroarthropathy reconstruction as superconstruct fixation with high fatigue strength and pullout resistance. Similar to intramedullary nailing throughout other long-bone fixation, injury to neurovascular and musculotendinous structures secondary to percutaneous interlocking screw fixation may exist. We sought to identify structures at risk for injury during the interlocking of a medial column nail. METHODS: Medial column nails were inserted into ten cadaveric limbs. The proximal (talar), middle (first metatarsal), and distal
