Acute Megakaryoblastic Leukemia With Diffuse Periosteal Reaction of Bilateral Lower Extremities.

We report the case of a 3-year-old girl diagnosed with acute megakaryoblastic leukemia, who presented after >1 year of bilateral leg pain. At times the pain was severe enough to prevent ambulation, prompting visits to her primary care provider. However, it was not until acute respiratory failure occurred with subsequent hospitalization in the pediatric intensive care unit that severe anemia and thrombocytopenia were discovered and the diagnosis of acute myeloid leukemia was made. Bilateral lower extremity swelling was noted on admission and radiographs showed diffusely abnormal appearance of t

Management of lower limb deformities in children with osteogenesis imperfecta.

Osteogenesis imperfecta (OI) often leads to severe lower limb (LL) deformities due to recurrent fractures that significantly hamper ambulation. We describe our management experience of correction of LL deformities in four children with OI. Medical management consisted of peri and postoperative pamidronate therapy, calcium supplementation and rehabilitative care. Deformities were corrected with multiple osteotomies and intramedullary fixation by titanium elastic nails. At a mean follow-up of 30 months, all children have significantly reduced fracture incidence and have no evidence of recurrence

Deformity correction planning for hindfoot, ankle, and lower limb.

Many patients with foot and ankle deformities have concurrent deformities (osseous and soft tissue), with or without limb length discrepancies. Lower extremity deformities and limb length discrepancies typically result from trauma, congenital abnormality, avascular necrosis, previous surgery, nonunion, and malunion. Limb deformity correction requires extensive surgical experience because many considerations and factors apply to realignment. The considerations and factors regarding realignment are highlighted throughout this article.

The Fate of the Bent Rod in Children With Osteogenesis Imperfecta.

BACKGROUND: Osteogenesis imperfecta (OI) is a genetic disorder characterized by brittle bones and long bone deformity. Realignment and intramedullary rodding with telescopic rods are indicated for progressive deformity and can help prevent fractures. Rod bending is a reported complication of telescopic rods and a common indication for revision; however, the fate of bent lower extremity telescopic rods in the setting of OI has not been reported. METHODS: Patients with OI at a single institution who underwent lower extremity telescopic rod placement with at least 1-year follow-up were identifie

Reduced muscle strength (dynapenia) in women with obesity confers a greater risk of falls and fractures in the UK Biobank.

OBJECTIVE: This study aimed to determine the independent effects of obesity and dynapenia on falls risk, areal bone mineral density, and fracture risk (lower extremity or all other fractures). METHODS: A total of 16,147 women (aged 60-82 years) from the UK Biobank were categorized by handgrip strength (HGS; dynapenia status: HGS ≤ 21 kg) and body weight (BMI: normal weight, overweight, or obesity). Multiple logistic regression models examined the association among dynapenia and obesity and self-reported falls (previous 12 months), lower extremity fractures, and all other fractures (previous 5

[Contralateral replantation after bilateral traumatic lower leg amputation. Case report with 6 year follow-up].

A 66-year-old patient attempted suicide by jumping in front of a train. The lower extremities were amputated at different levels. On the right side, there was a complete amputation within the distal third of the lower leg. Proximal to the amputation site, there was an extensive soft-tissue and bone defect. On the left side, there was a crush injury of the tarsal and mid-tarsal bones. The left lower leg showed only few injuries. An ipsilateral (anatomical) replantation was not possible. In order to save one lower extremity, we decided to carry out a cross-over (contralateral) replantation of th

Land mine injury: functional testing outcome.

We present a case of a war patient treated with external fixation for a complex land mine injury involving the fracture of the tibial and fibular bones, which occurred at the front line during the war in Croatia and Bosnia and Herzegovina. Excessive destruction and foreign body penetration into the distal two-thirds of the right leg and foot endangered not only the patient's lower extremity but his life as well. Nevertheless, the patient's life, as well as extremity, was saved due to an intensive treatment. A 100-month follow-up showed a bridge callus between the tibia and fibula, tibial bone

Secondary radiological findings of osteoid osteoma as muscular atrophy and synovitis in paediatric and adult patients.

PURPOSE: Diagnosis of osteoid osteoma may be delayed if secondary radiological findings such as muscle atrophy, oedema in peripheric soft tissue and bone marrow, joint effusion, or synovitis are more severe than the lesion itself. In this article, the purpose was to demonstrate secondary radiological findings of osteoid osteoma in both paediatric and adult patients. This study is one of the very few with such a large case series. MATERIAL AND METHODS: Radiological images of 152 patients were reviewed retrospectively. Peri-nidus sclerosis, periosteal reactive bone formation, bone marrow and so

[Injuries of major blood vessels of the lower extremities as a complication of fractures and dislocations].

Surgical treatment was applied in 46 patients with simultaneous injury of great vessels of the lower extremities and fractures of the long bones or dislocations of the knee joint. The authors have determined the best in their opinion sequence of surgical procedure. They think that external fixation gives better bone stabilization and is more favorable than internal stabilization. They also recommend lavage of the peripheral vascular bed of the ischemic lower extremity, which prevents post-traumatic insufficiency of the kidneys. They think that early fasciotomy of the crus or fibulectomy is imp

Correlation of strain and loads measured in the long bones with observed kinematics of the lower limb during vehicle-pedestrian impacts.

The purpose of this study is to determine the loads in the long bones of the lower extremities during vehicle-pedestrian impact tests, and to correlate load data with observed kinematics in an effort to understand how stature and vehicle shape influence pedestrian response. In tests with a large sedan and a small multi-purpose vehicle (MPV), four post mortem human surrogates (PMHS) in mid-stance gait were struck laterally at 40 km/h. Prior to the tests, each PMHSwas instrumented with four uniaxial strain gages around the mid-shaft cross section of the struck-side (right) tibia and the femora b

[Treatment of comminuted fractures of the distal end of the radius by traction using transfixing Kirschner's wires and plaster cast].

Based on the analysis of 45 cases of comminuted fractures of the lower extremity of the radius, this study points out the insufficiency of conventional treatments. The authors advocate a technique using a Kirschner's wire passed through the metacarpal bones and another one through the ulna, both beind embeded in plaster cast. This method was tested on 19 cases with encouraging results. The technique was precisely described and the authors think that it should be used in fractures incorrectly reduced by conventional methods.

[Reconstruction of the distal lower extremity defects with distal pedicled fasciocutaneous flaps].

Soft-tissue coverage of defects in the distal lower extremity has its limitations because of minimal blood supply to the skin and the thin muscle layer. Split-thickness skin grafting is impossible in cases of defects of tendons or bones. Local flaps are applicable only for smaller defects. With the cross-leg flaps, it is necessary to immobilize the patient for a long time. Special microsurgical technique and knowledge are needed for the free flaps. Fasciocutaneous flaps of the lower extremity after Donski and Fodgestam are simpler and safer than the other methods. From 1987 to 1992, this metho

Telescoping intramedullary stabilization of the lower extremities for severe osteogenesis imperfecta.

Intramedullary extendable Bailey-Dubow nails were used for treatment of lower extremity deformities resulting from osteogenesis imperfecta. Sixteen patients had 56 nails placed in 48 long bones, including 18 revisions. All patients were braced postoperatively. The average duration of follow-up was 4.8 years. Nine patients who were nonambulatory preoperatively walked in braces postoperatively. Despite a high rate of complication, extendable nails provide correction of the angular deformities of osteogenesis imperfecta, decrease fracturing, and allow most previously nonambulatory children to wal
