The chronicity effect is derived from two other general principles that underlie clinical neuropsychological test interpretation. These are (1) localization principle: elementary functions are related to specific cortical areas; (2) Diaschisis: functions located at a distance from an acute lesion may be affected by that lesion. The chronicity effect states that as a nonprogressive lesion becomes chronic, diaschisis tends to recede. In an illustrative detailed case study, a man with encephalitis concentrated in the left temporoparietal area was tested five times over a 6-year period. In the fir

A total of 53 babies, 37 to 44 weeks' gestational age with a history of fetal distress in labour, and severe neurological abnormalities in the early newborn period, were followed up for between 2 and 5 years. Their progress was compared with an equal number of normal babies matched for gestational age, birthweight, sex, and social class, but without a history of fetal distress or delay in establishing spontaneous respiration at birth. In the set of babies with a history of fetal distress no perinatal condition was identified that could predict with certainty the type of neurological status in

The principal fields of application of the EEG are 1) the identification of transient functional disturbances - especially in cerebral seizures - which cannot always be reliably observed clinically, 2) the tracing of concomitant signs of substantial lesions which in the silent areas are not necessarily associated with isolated neurological defects and 3) the determination of the degree of severity of abnormalities when they do not agree with neurological or psychic disturbances. The discrepancies of the clinical findings themselves sometimes demand attention and benefit the patient when they l

Phenylketonuria, an autosomal recessively transmitted disorder of amino acid metabolism, is caused by a deficiency of hepatic phenylalanine hydroxylase converting phenylalanine to tyrosine. Thus, phenylalanine accumulates to plasma levels exceeding 1200 mumol/l. Untreated phenylketonuria is characterized by microcephaly, epilepsy, severe mental retardation and, in some cases, progressive supranuclear motor disturbances. These symptoms can largely be prevented by the early start of a phenylalanine-restricted diet. Neurological investigations of treated patients reveal only minor neurological si

The premise of cortical modularity is based on strong dissociations caused by focal lesions. These dissociations are rare, and their explanatory power and theoretical importance are vastly overrated. The effects of brain lesions must be considered in their totality, rather than in idiosyncratic selectivity. These effects are more consistent with a continuous, graded functional neocortical geometry, than with a modular neocortex. Distinction must be drawn between strong intrinsic modularity, and weak emergent modularity. Strong intrinsic modularity is more characteristic of the thalamus than of

Sleep complaints were obtained from 22 hospitalised patients with traumatic brain injury of recent onset (median 3.5 months after injury) and were compared with those of 77 discharged patients who had sustained brain injury about two to three years (median 29.5 months) previously. A high incidence of sleep complaints was noted in both groups (72.7% and 51.9% respectively). Disorders in initiating and maintaining sleep (DIMS) were the most common complaints among hospitalised patients (81.2%), whereas disorders of excessive somnolence (DOES) were common in discharged patients (72.5%). This diff

Four children surviving severe hypoxic-ischemic brain damage showed subtotal destruction of the cerebral hemispheres, with partial preservation of some infratentorial and supratentorial brain structures. True multicystic encephalopathy features were identified in two cases on CT scanning. The other patients showed more pronounced brain tissue destruction, with overall malacia of the hemispheres. An echographic study demonstrated that the brain damage occurred progressively, within about one month, and was preceded by oedema, white matter malacia and cavitation. The role of the compensatory mec

Although the perinatal brain is more resistant towards hypoxia-ischemia than at any later stage of development, perinatal asphyxia still contributes to permanent brain damage in survivors. This short review focuses on the discussion of mechanisms of late neuronal death, i.e. cerebral damage related to lack of oxygen but caused by mechanisms during the stage of reoxygenation. Evidence is presented to indicate that the transient, dense glutamatergic innervation of basal structures of the perinatal brain may induce brain damage via overflow of excitatory amino acids during oxygen lack, causing a

Eleven persons with chronic toxic encephalopathy caused by solvents had electronystagmography (ENG) and brain-stem response audiometry (BRA). It is concluded that the audio-vestibular examination of this category of patient should concentrate on cortical response audiometry (CRA) and computer ENG.

OBJECTIVE: Rescue benzodiazepine medication can be used to treat seizure clusters, which are intermittent, stereotypic episodes of frequent seizure activity that are distinct from a patient's usual seizure pattern. The NeuroPace RNS® System is a device that detects abnormal electrographic activity through intracranial electrodes and administers electrical stimulation to control seizures. Reductions in electrographic activity over days to weeks have been associated with the longer-term efficacy of daily antiseizure medications (ASMs). In this pilot study, electrographic activity over hours to d
