[LEFT HIS BUNDLE BRANCH BLOCK ASSOCIATED WITH LEFT VENTRICULAR TORSION AND REDUCED EJECTION FRACTION].

The influence of left His bundle branch block (LBBB) on left ventricular (LV) torsion in patients with cardiomyopathy remains to be elucidated. The aim of this study was to evaluate LV torsion associated with LBBB and hemodynamic consequences of possible changes. We studied 64 patients with ischemic and dilatation cardiomyopathy (LV ejection fraction less than 40%) divided into 2 groups, with narrow and middle (153 ms) duration QRS complexes. Despite similar LV contractility, patients with LBBB had much less pronounced LV rotation and torsion. Torsion in patients with LBBB and narrow QRS compl

Direct His-Bundle Pacing Improved Left Ventricular Function and Remodelling in a Biventricular Pacing Nonresponder.

The optimal pacing modality after atrioventricular junction (AVJ) ablation remains unclear. Herein, we describe the case of a heart failure patient who had AVJ ablation for chronic atrial fibrillation and received a cardiac resynchronization therapy defibrillator device. Because of the lack of clinical response to biventricular pacing, the device was revised with the addition of direct His bundle pacing, which resulted in significant improvement in functional status and left ventricular indices. This case illustrated direct His bundle pacing as an alternative for conventional biventricular pac

Multiple gap phenomena associated with dual His bundle pathways having a lower common tract.

We report a case showing 4 types of aberrant conduction and 2 types of gap phenomena produced by single atrial extrastimulus during sinus rhythm. One of the gap phenomena was associated with dual His bundle pathways, demonstrating a fast-to-slow jump-up phenomenon. Of note, a sharp potential considered as retrograde His bundle activations was recorded, which indicated that the activation wave front descended the slow His bundle pathway and turned up the fast His bundle pathway.

Reanalysis of the "pseudo A-A-V" response to ventricular entrainment of supraventricular tachycardia: importance of his-bundle timing.

BACKGROUND: The sequence of atrial and ventricular electrograms following termination of ventricular pacing during supraventricular tachycardia has been shown to reliably differentiate atrial tachycardia from atrioventricular nodal reentrant tachycardia (AVNRT) and atrioventricular reentrant tachycardia (AVRT). However in patients with long HV intervals, this may be misleading due to a pseudo "A-A-V" response. The aim of the present study is to see if inclusion of the timing of the His-bundle in the electrogram response (ER) following ventricular pacing would reliably identify the mechanism of

A family with myotonic dystrophy associated with diffuse cardiac conduction disturbances as demonstrated by His bundle electrocardiography.

In a family with myotonic dystrophy, three siblings showing ECG abnormalities were subjected to His bundle ECG studies. The duration of the disease ranged from 9 to 24 years. All three siblings had prolonged HV intervals (His-Purkinje conduction times) of 70 to 80 msec. The sister with the shortest duration of the disease had second-degree atrioventricular (AV) nodal block as well as left posterior fascicular block, the younger brother had left anterior fascicular block, and the elder brother with the longest history had first-degree AV block without bundle branch or fascicular block. With reg

Pullout His bundle/electrophysiologic studies using leads previously inserted via the subclavian vein.

Conventional electrophysiologic studies involve the insertion of several catheter electrodes, positioned to pace and record from the high right atrium, His bundle region, and the right ventricular apex. The present study was undertaken to determine whether similar information could be obtained during removal of temporary pacing leads previously inserted via the subclavian vein, without the need for further invasive studies. Attempts to record a His bundle potential were successful in 157 of 200 patients (79%). A His bundle potential could be recorded during low right atrial pacing on 24 of 58

[Atrioventricular conduction disorders following total correction of tetralogy of Fallot. Analysis of the electrogram of the bundle of His].

A study was made of the electric activity of the atrioventricular (AV) conduction system in basal conditions and with atrial stimulation in order to discover the type, severity and prognosis of the AV disturbance in cases of total correction of tetralogy of Fallot. The authors studied thirteen patients from the Paediatric Department of the National Institute of Cardiology who had undergone such surgery, 7 males, 6 females from 3 to 22 years old. All presented advanced RBBB and sinus rhythm; in the immediate postoperative period, one presented complete transitory A-V block which required a stan

Effect of His Bundle Pacing on Abnormal Myocardial Fatty Acid and Glucose Metabolism Induced by Right Ventricular Pacing.

BACKGROUND: Metabolic disorder is noted for pacing-induced cardiomyopathy. The benefits of His bundle pacing over right ventricular (RV) pacing in preventing pacing-induced cardiomyopathy from a metabolic perspective are yet to be fully understood. METHOD AND RESULTS: Three pig groups were established for this study: sham control, RV pacing (RV pacing for 6 months), and His pacing (RV pacing for 6 months, followed by His bundle pacing for 3 months). Complete atrioventricular block was created in the last 2 groups. Left ventricular function and dyssynchrony were assessed via echocardiography,

Novel approach to diagnosis of His bundle capture using individualized left ventricular lateral wall activation time as reference.

BACKGROUND: During nonselective His bundle (HB) pacing, it is clinically important to confirm His bundle capture versus right ventricular septal (RVS) capture. The present study aimed to validate the hypothesis that during HB capture, left ventricular lateral wall activation time, approximated by the V6 R-wave peak time (V6 RWPT), will not be longer than the corresponding activation time during native conduction. METHODS: Consecutive patients with permanent HB pacing were recruited; cases with abnormal His-ventricle interval or left bundle branch block were excluded. Two corresponding interva

Beneficial effects of upgrading to His bundle pacing in chronically paced patients with left ventricular ejection fraction <50.

BACKGROUND: Heart failure is common in patients with permanent pacemakers. Right ventricular (RV) pacing may worsen cardiac function, while the nonresponder rate of cardiac resynchronization therapy (CRT) for heart failure treatment remains 25%-30%. OBJECTIVE: The purpose of this study was to assess clinical outcomes of upgrading to permanent His bundle pacing (pHBP) in patients with heart failure who underwent device upgrade from right ventricular pacing or CRT nonresponse. METHODS: Eighteen patients with pacing-dependent heart failure and left ventricular ejection fraction (LVEF) <50% unde

[Treatment of junctional paroxysmal tachycardia, without patent Wolff-Parkinson-White syndrome, by sectioning an accessory Kent-His bundle].

The authors report the case of a patient suffering from a Bouveret's tachycardia without syndrome of Wolff-Parkinson-White. The analysis of the tachycardic spells however showed that during a reciprocal crisis, the circuit went through a left accessory ventriculo-atrial bundle, functioning only in the reverse direction. This accessory bundle was successfully cut by the surgeon, following the procedure of wide atrioventricular desinsertion as described by the authors of Duke University for the surgical treatment of the Wolff-Parkinson-White syndrome.

Long-term follow-up of selective and non-selective His bundle pacing leads in patients with atrioventricular block.

BACKGROUND: His bundle pacing (HBP) is a novel treatment with limited knowledge on long-term outcome. We aimed to assess long-term safety and effectiveness of HBP in patients with atrioventricular block treated with HBP and a back-up right ventricular pacing (RVP) lead. METHODS: We included 38 patients from a completed single-center, randomized controlled cross-over trial designed to compare left ventricular (LV) function after 12 months of HBP vs. RVP conducted between September 2007 and August 2011. Lead performance beyond the 2-year study period was assessed based on a retrospective review

Is the systematic use of mapping systems during His Bundle catheter ablation cost-effective? A single-center experience.

AIM: Three-dimensional (3D) nonfluoroscopic mapping systems (NMSs) are generally used during the catheter ablation (CA) of complex arrhythmias. We evaluated the efficacy, safety, and economic advantages of using NMSs during His-Bundle CA (HB-CA). METHODS: A total of 124 consecutive patients underwent HB-CA between 2012 and 2019 in our EP Laboratory. We compared two groups: 63 patients who underwent HB-CA with fluoroscopy alone from 2012 to 2015 (Group I) and 61 patients who underwent HB-CA with the aid of NMSs from 2016 to 2019 (Group II). Two cost-effectiveness analyses were carried out: the

Bundle branch reentrant tachycardia in patients with apparent normal His-Purkinje conduction: the role of functional conduction impairment.

INTRODUCTION: His-Purkinje conduction delay, manifested by bundle branch block QRS complex configuration or by HV interval prolongation, is considered an essential condition for maintenance of bundle branch reentrant tachycardia (BBRT). METHODS AND RESULTS: Of 178 patients with different types of ventricular tachycardia (VT), 13 were found to have BBRT as the underlying electrophysiologic mechanism. Of these 13 patients (9 men and 4 women; mean age 64 +/- 13 years), 6 had an HV interval < or = 55 msec (group A), and 7 had a prolonged HV interval (> 55 msec; group B) during sinus rhythm (SR).

Clinical and electrocardiographic features and long-term results of electrical therapy in patients with isolated His bundle disease.

The clinical, ECG, and electrophysiologic findings of 35 consecutive patients with second- and third-degree intra-His block with normal QRS complexes were examined. The follow-up period varied between 12 and 120 months (mean 45). Seventy-seven per cent of the patients were women. Underlying heart disease was present in 43% of the patients. ECGs were characterized by both second-degree type I and type II atrioventricular block, normal or slightly prolonged PR interval of the conducted beats or of the first conducted beat of a Wenckebach sequence, and by subtle changes in the initial forces of t

Longitudinal dissociation and transition in thickness of the His-Purkinje system cause various QRS waveforms of surface ECG under His bundle pacing: A simulation study based on clinical observations.

AIMS: His bundle pacing (HBP) is a feasible and reliable alternative to conventional right ventricular pacing (RVP), but associated ECG (electrocardiogram) changes have not been well-studied. This study aimed to determine the mechanisms underlying ECG changes associated with HBP using patient-specific multiscale heart simulations. METHODS: ECGs were recorded in two patients who were treated by HBP under a native rhythm and HBP at high and low voltages. We created patient-specific multiscale simulation heart models of these patients and performed ECG simulation under these conditions. Using th

Congenitally corrected transposition of the great vessels: localization of the site of complete atrioventricular block using his bundle electrograms.

Complete atrioventricular block proximal to the bundle of His in a patient with congenitally corrected transposition of the great vessels was documented using His bundle electrograms. The spontaneous rhythnm probably originated from the bundle of His and was responsive to carotid sinus massage, atropine and isometric and treadmill exercise. These electrophysiologic observations are consistent with recent anatomic studies of congenitally corrected transposition of the great vessels.

[Significance of spike potentials in the sinus nodes and atrial conduction system during interpretation of bundle-of-his electrography].

The article deals with the issues of the His bundle electrogram (HBE) analysis, using the spike potentials of the sinus node and the atrial conductive tracts for the interpretation of the HBE. The HBE and the atrium spike potentials have been recorded in 40 dogs and in 307 patients. According to our data the onset of the A-wave on the IcECG begins in some msec earlier than the onset of the P-wave on the standard ECG leads. The spike potentials of the sinus node have been recorded directly at the sinus node area, the onset of which begins in 20-40 msec earlier than the onset of the P-wave. By o

Usefulness of His Bundle Pacing to Achieve Electrical Resynchronization in Patients With Complete Left Bundle Branch Block and the Relation Between Native QRS Axis, Duration, and Normalization.

His Bundle pacing (HBP) restores electrical synchronization in left bundle branch block (LBBB); however, the underlying mechanisms are poorly understood. We examined the relation between native QRS axis in LBBB, a potential indicator of the site of block, and QRS normalization in patients with LBBB. Data from patients (n = 41) undergoing HBP at 3 sites were studied (68 ± 13 years, 13 women). Study criteria included strictly defined complete LBBB and successful implantation of a permanent HBP lead. Preprocedure and postprocedure electrocardiograms were reviewed independently by 2 blinded reader
