Topical Burn Wound Management.

Cutaneous burns challenge global health care systems with high patient morbidity and mortality rates.1 One recent study of adults admitted for 2 to 60 days to a US burn center reported that more than 7.9% of burn patients experienced at least 1 hospital-acquired infection (HAI), extending the hospital length of stay and increasing the likelihood of complications and death.2 Of these HAIs, most (35.8%) were skin and soft tissue infections, followed by respiratory (24.4%), bloodstream (18.1%), and urinary tract (17.8%) infections. A burn covering more than 5% of total body surface area (TBSA) mu

Epidemiology and management of respiratory burns in National Burns Centre, Singapore.

Respiratory burns constitute one of the two leading causes of burns mortality today, the other being infection. When cutaneous burns are complicated by co-existent respiratory or inhalational injury, the mortality remains high in most reported series. We began our prospective study of respiratory burns in 1982 as a multi-disciplinary team effort involving respiratory physicians, anaesthetists and our burns physicians. Between 1982 until February 1990, we treated a total of 240 patients with respiratory burns. During the same period, a total of 4041 patients were admitted to our Burns Centre at

Kidney failure trajectories and sub phenotypes in severe burn patients.

BACKGROUND: Acute kidney injury (AKI) is a common and severe complication in burn patients, significantly increasing morbidity and mortality. Recent classifications distinguish acute kidney disease (AKD) as persistent kidney dysfunction beyond seven days; however, its trajectory in burn patients remains poorly understood. This study aimed to identify renal injury phenotypes in critically ill burn patients and assess their associations with clinical outcomes. METHODS: We conducted a single-center retrospective cohort study from 2013 to 2023 of adult (>18 years) ICU burn patients with total bur

Ascariasis pneumonitis: a potentially fatal complication in smoke inhalation injury.

Ascaris pneumonitis in areas of endemic infestation is considered a benign condition. Smoke inhalation with any burn injury can be potentially fatal. A heavy infestation of Ascaris could further exacerbate the smoke-induced lung injury. After ingested eggs hatch in the small intestine, the larvae penetrate the mucosa and invade the blood stream and are then carried to the lungs. The larvae break out into the aveolar spaces as they are too large to cross the capillary bed and are carried up the bronchial tree and eventually swallowed. This study describes three cases of Ascaris infection in the

The predictive probability of mortality in the presence of full-thickness burns.

INTRODUCTION: Prognostication of burn injury mortality is challenging. Recent mortality prognostication tools have incorporated the percent of full-thickness surface area (FTSA). We hypothesize that the presence of full-thickness burn injury independently increases in-hospital mortality. METHODS: We performed a retrospective review of the National Trauma Data Bank (NTDB) from 2007 to 2019 of adults (≥16 years old) with burn injuries. Variables evaluated included basic demographics, presence of inhalation injury, percent TBSA, and percent FTSA burned. The primary outcome was in-hospital mortal

Restrictive Transfusion Strategy Is More Effective in Massive Burns: Results of the TRIBE Multicenter Prospective Randomized Trial.

OBJECTIVES: Studies suggest that a restrictive transfusion strategy is safe in burns, yet the efficacy of a restrictive transfusion policy in massive burn injury is uncertain. Our objective: compare outcomes between massive burn (≥60% total body surface area (TBSA) burn) and major (20-59% TBSA) burn using a restrictive or a liberal blood transfusion strategy. METHODS: Patients with burns ≥20% were block randomized by age and TBSA to a restrictive (transfuse hemoglobin <7 g/dL) or liberal (transfuse hemoglobin <10 g/dL) strategy throughout hospitalization. Data collected included demographics,

The Utility of the Social Vulnerability Index on Burn Injury Outcomes.

INTRODUCTION: Clinical predictors of burn outcomes are well established, but the impact of social vulnerability remains unclear. This study evaluated the utility of the Centers for Disease Control and Prevention's Social Vulnerability Index (SVI) in predicting outcomes at an urban, academic burn center. METHODS: We conducted a retrospective cohort study of patients (age ≥18) admitted to an American Burn Association-verified center (2020-2023). Subjects were categorized into high-risk (SVI ≥0.75) and low-risk (SVI <0.75) groups. Multivariate regression compared outcomes, controlling for age, s

Measures for preventing early postburn damage improve survival rate of burn patients.

AIM: To define the role of measures for preventing early postburn damage in improving the survival rate of burn patients. METHODS: 12568 burn cases admitted to our institute were chronologically divided into three groups (stages). Total burn surface area (TBSA), survival rate, incidence of burn shock, systemic infection and organ damage as well as the main treatments adopted in the recent decade were analyzed retrospectively. RESULTS: Incidence of burn shock, systemic infection and organ damage were significantly lower, and the total survival rate and the survival rate in patients with diffe

The Effect of Transfer on Outcomes in Burns.

Patients with burn injuries are often initially transported to centers without burn capabilities, requiring subsequent transfer to a higher level of care. This study aimed to evaluate the effect of this treatment delay on outcomes. Adult burn patients meeting American Burn Association criteria for transfer at a single burn center were retrospectively identified. A total of 122 patients were evenly divided into two cohorts-those directly admitted to a burn center from the field vs those transferred to a burn center from an outlying facility. There was no difference between the transfer and dire

An Acute Case of Exogenous Lipoid Pneumonia Which Developed in Tent Sauna.

The aspiration or inhalation of lipid substances can cause lung injury, including chemical pneumonitis and exogenous lipoid pneumonia. We herein report a case of acute lung injury caused by kerosene inhalation in a tent sauna. A patient developed dyspnea with respiratory failure two hours after igniting a wood-burning stove with kerosene. High-resolution computed tomography revealed widespread ground-glass opacities, and bronchoalveolar lavage revealed lipid-laden macrophages. Systemic corticosteroid treatment led to a rapid improvement. Although kerosene is generally safe at room temperature,

Total intravenous anesthesia for major burn surgery.

UNLABELLED: Total intravenous anesthesia (TIVA) is frequently used for major operations requiring general anesthesia in critically ill burn patients. We reviewed our experience with this approach. METHODS: During a 22-month period, 547 major burn surgeries were performed in this center's operating room and were staffed by full-time burn anesthesiologists. The records of all 123 TIVA cases were reviewed; 112 records were complete and were included. For comparison, 75 cases were selected at random from a total of 414 non-TIVA general anesthetics. Some patients had more than one operation during

Delayed asphyxia due to inhalation injury.

We report the case of a 53-year-old man who was found dead in his apartment 2 days after a little fire during which he had suffered from second to third degree burns at 2% of the body surface, also involving the skin around the mouth and the nose. At autopsy, severe acute emphysema and mucopurulent obstructive laryngotracheobronchitis were observed. The histology showed desquamative loss of the respiratory epithelium up to the middle bronchi; the bronchial lumen was filled with clumps of mucopurulent secretions mixed with necrotic epithelial cells. The cause of death was a delayed asphyxia due
