BACKGROUND: Genetic cardiomyopathy is a rare disease in childhood.
AIMS: To analyse clinical and genetic aspects of a paediatric cardiomyopathy population, and to establish genotype-phenotype correlations.
METHODS: We performed a retrospective study of all patients with idiopathic cardiomyopathy aged<18years in Southeast France. Secondary causes of cardiomyopathy were excluded. All data (clinical, echocardiography, genetic testing) were collected retrospectively. Patients were classified into six groups: hypertrophic cardiomyopathy; dilated cardiomyopathy; restrictive cardiomyopathy; left ve

Exosomes, a subset of small extracellular vesicles that play a crucial role in intercellular communication, have garnered significant attention for their potential applications in the diagnosis and treatment of cardiomyopathies. Cardiomyopathies, which encompass a spectrum of heart muscle disorders, present complex challenges in diagnosis and management. Understanding the role of exosomes in the etiology of cardiomyopathies such as dilated cardiomyopathy (DCM), restrictive cardiomyopathy (RCM), arrhythmogenic cardiomyopathy (AC), and hypertrophic cardiomyopathy (HCM) may open new possibilities

Variants in the filamin C (FLNC) gene are increasingly recognized as causes of cardiac and skeletal muscle disease. Cardiomyopathy-associated FLNC variants span dilated cardiomyopathy (DCM), nondilated left ventricular cardiomyopathy (NDLVC), arrhythmogenic cardiomyopathy (ACM), hypertrophic cardiomyopathy (HCM), and restrictive cardiomyopathy (RCM), but their clinical interpretation depends strongly on variant class and evidence of pathogenicity. FLNC truncating variants (FLNCtv), including nonsense, frameshift, and canonical splice-site variants, usually cause haploinsufficiency through nons

Within the heart muscle, the largest sarcomeric protein is titin (TTN). The heart expresses two principal isoforms, N2B and N2BA, which arise from alternative splicing of the TTN gene. These isoforms span four distinct regions of the sarcomere: the Z-line, I-band, A-band, and M-line. Titin, encoded by the extensive TTN gene consisting of 364 exons, plays a critical role in the structural integrity, development, mechanical properties, and regulation of both cardiac and skeletal muscles. The purpose of this review is to provide a comprehensive understanding of the critical role TTN mutations pla

Cardiomyopathies are a small percentage of heart disease in pregnant women, but usually cause severe complications. It is not know the exact incidence of cardiomyopathy associated with pregnancy in Europe. In these patients, there is a higher probability of death due to changes in hemodynamic, metabolic and hemostatic that occur in pregnancy. Pregnant mortality is 2.4% vs. 0.007%--in the normal population. The most common cause of severe maternal complications is peripartum cardiomyopathy (PPCM). In dilated cardiomyopathy (DCM) and restrictive (RCM) is usually observed significant clinical det

Loeffler's endocarditis is a rare form of restrictive cardiomyopathy associated with eosinophilia and endomyocardial fibrosis. It manifests most commonly as diastolic dysfunction or valvular abnormalities due to eosinophilic infiltration and degranulation. Herein, we chronicle a case of left ventricular involvement with Loeffler's endocarditis. We emphasize the utility of multimodality imaging including two-dimensional echocardiography and cardiac magnetic resonance imaging in providing diagnostic information, as there is no standardized diagnostic criteria to date.

In this article, we describe a 14-year-old boy with a confirmed diagnosis of Friedreich ataxia who underwent cardiac transplantation for left ventricular failure secondary to dilated cardiomyopathy with restrictive physiology. His neurological status prior to transplantation reflected early signs of neurological disease, with evidence of dysarthria, weakness, mild gait impairment, and limb ataxia. We review the ethical issues considered during the process leading to the decision to offer cardiac transplantation.

Löffler's syndrome is defined by prolonged and profound eosinophilia and restrictive cardiomyopathy. Doppler echocardiography is useful in both the diagnosis and management of this entity. On the other hand, diastolic dysfunction is assessed better by transesophageal echocardiography than by transthoracic echocardiography, mainly in the analysis of pulmonary vein Doppler flow. We describe a patient with Löffler's syndrome, whose pulmonary vein flow, obtained by transesophageal echocardiography, was helpful for better management of the disease.

Nonischemic cardiomyopathies encompass a broad spectrum of myocardial disorders with mechanical or electrical dysfunction without evidence of ischemia. There are five broad variants of nonischemic cardiomyopathies; hypertrophic cardiomyopathy (Variant 1), restrictive or infiltrative cardiomyopathy (Variant 2), dilated or unclassified cardiomyopathy (Variant 3), arrhythmogenic cardiomyopathy (Variant 4), and inflammatory cardiomyopathy (Variant 5). For variants 1, 3, and 4, resting transthoracic echocardiography, MRI heart function and morphology without and with contrast, and MRI heart functio

Heart failure is a global epidemic that drastically cuts short longevity and compromises quality of life. Approximately 6 million Americans ≥20 years of age carry a diagnosis of heart failure. Worldwide, about 40 million adults are affected. The treatment of HF depends on the etiology. If left untreated it rapidly progresses and compromises quality of life. One of the newer technologies still in its infancy is stem cell therapy for heart failure. This review attempts to highlight the clinical studies done in ischemic cardiomyopathy, dilated cardiomyopathy and restrictive cardiomyopathy. A comb

INTRODUCTION: Diabetic cardiomyopathy (DbCM) is characterized by subclinical abnormalities in cardiac structure/function and is associated with a higher risk of overt heart failure (HF). However, there are limited data on optimal strategies to identify individuals with DbCM in contemporary health systems. The aim of this study was to evaluate the prevalence of DbCM in a health system using existing data from the electronic health record (EHR).
METHODS: Adult patients with type 2 diabetes mellitus free of cardiovascular disease (CVD) with available data on HF risk in a single-center EHR were i

| Cardiomyopathy in children is a rare but serious disease leading to heart failure with a high mortality rate. Dilated cardiomyopathy (DCM) and hypertrophic cardio-myopathy (HCM) are more common than restrictive cardiomyopathy (RCM). Arrhythmogenic right ventricular dysplasia (ARVD) is extremely rare in pediatric age groups. Treatment should be targeted at the cause of the cardiomyopathy, if identified. Nutritional deficiencies, metabolic disorders, and inborn errors of metabolism should be corrected if possible. Immunosuppressive therapy may be considered in patients with lymphocytic myocard

Cardiomyopathies comprise a group of disorders wherein the primary defect is in cardiac myocytes. The common forms of pediatric cardiomyopathies, classified according to their morphological and functional manifestations, include dilated cardiomyopathy (DCM), restrictive cardiomyopathy (RCM), hypertrophic cardiomyopathy (HCM), and others. Cardiac gene mutations caused abnormal myofibril Ca2+ sensitivity may be involved in the underlying molecular mechanisms of cardiomyopathies. Thus far, no effective treatment for cardiomyopathies has been developed, especially for HCM and RCM in which diastoli

Cardiac amyloidosis is caused by amyloid fibrils that deposit in the myocardial interstitium, causing restrictive cardiomyopathy and eventually death. The electromechanical, inflammatory, and autonomic changes due to amyloid deposition result in arrhythmias. Atrial fibrillation is by far the most common arrhythmia. The rate control strategy is generally poorly tolerated due to restrictive filling physiology and heart rate dependance, favoring adoption of the rhythm control strategy. Anticoagulation for stroke prophylaxis is warranted, irrespective of CHA2DS2-VASc score in patients with a favor

Despite many decades of research into the underlying pathophysiology of thromboembolic disease in cats with cardiomyopathy, findings across the literature are still limited with inconsistencies across study findings. The aim of this scoping review is to describe the literature evaluating thromboembolic disease in cats with cardiomyopathy, to identify coagulation pathways that are involved, and their respective findings and limitations. All studies except two had a healthy control cat group for comparison, and intervention groups varied with regards to severity of cardiomyopathy, and/or the pre

Endomyocardial fibrosis (EMF) is a rare and often underdiagnosed form of restrictive cardiomyopathy. Prognosis is generally unfavorable. Early diagnosis, along with surgical and medical intervention, is crucial for improved outcomes. We report the case of a 15-year-old girl from Guinea who presented with suspected Ebstein's anomaly and severe right heart failure. Multimodal cardiac imaging revealed right ventricular EMF. Despite counseling on prognosis, the family declined surgery. The patient experienced cardiac arrest two months later.

Tropical endomyocardial fibrosis (FET) is a leading cause of heart failure and the most common restrictive cardiomyopathy worldwide. Extensive fibrosis of the ventricular endocardium causing architectural distortion, impaired filling and valvular insufficiency define the disease. Confined to peculiar and limited geographical areas, the aetiology remains blurred and it carries a grim prognosis. The changing burden reported recently in some endemic areas and the refinement of diagnostic tools have emphasized alternative routes for understanding and treatment of the disease.
