Endoscopic anterior transmaxillary "transalisphenoid" approach to Meckel's cave and the middle cranial fossa: an anatomical study and clinical application.

OBJECTIVE Multiple approaches have been designed to reach the medial middle fossa (for lesions in Meckel's cave, in particular), but an anterior approach through the greater wing of the sphenoid (transalisphenoid) has not been explored. In this study, the authors sought to assess the feasibility of and define the anatomical landmarks for an endoscopic anterior transmaxillary transalisphenoid (EATT) approach to Meckel's cave and the middle cranial fossa. METHODS Endoscopic dissection was performed on 5 cadaver heads injected intravascularly with colored silicone bilaterally to develop the appro

Diagnosis and Outcome of Biopsies of Indeterminate Lesions of the Cavernous Sinus and Meckel's Cave: A Retrospective Case Series in 85 Patients.

BACKGROUND: When clinical presentation, laboratory studies, or imaging cannot diagnose cavernous sinus (CS) and/or Meckel's cave (MC) lesions, biopsy may be necessary. OBJECTIVE: To review our institutional series of biopsies of indeterminate CS and MC lesions. METHODS: Records from January 1994 to June 2016 were searched for biopsied indeterminate CS and MC lesions. We defined indeterminate as having an atypical imaging appearance or a broad differential and the need for tissue for definitive diagnosis. We defined primary tumors as originating from cells inherent or near the CS and MC. RES

Discovery of a new Wolbachia supergroup in cave spider species and the lateral transfer of phage WO among distant hosts.

Wolbachia are widespread intracellular bacteria infecting the major classes of arthropods and some filarial nematodes. In arthropods, Wolbachia have evolved various intriguing reproductive manipulations, including cytoplasmic incompatibility, parthenogenesis, feminization, and male killing. Sixteen supergroups of Wolbachia have been identified, named A-Q (except G). Though Wolbachia present great diversity in arthropods, spiders, especially cave spiders, are still a poorly surveyed group of Wolbachia hosts. Here, we report a novel Wolbachia supergroup from nine Telema cave spiders (Araneae: Te

New Australopithecus robustus fossils and associated U-Pb dates from Cooper's Cave (Gauteng, South Africa).

Australopithecus robustus is one of the best represented hominin taxa in Africa, with hundreds of specimens recovered from six fossil localities in the Bloubank Valley area of Gauteng Province, South Africa. However, precise geochronological ages are presently lacking for these fossil cave infills. In this paper, we provide a detailed geological background to a series of hominin fossils retrieved from the newly investigated deposit of Cooper's D (located partway between Sterkfontein and Kromdraai in the Bloubank Valley), including uranium-lead (U-Pb) ages for speleothem material associated wit

[Cave canulam et dilatationem. Cardiopulmonary resuscitation and surgical repair of a vessel perforation following central venous catheterization].

A 58-year-old female patient was scheduled for minimally invasive cardiac surgery. For drainage of venous blood to the heart-lung machine insertion of a central venous large diameter catheter was a prerequisite. During insertion into the right internal jugular vein by the anesthesiologist a life-threatening perforation of the subclavian artery occurred. The patient suffered hemorrhagic shock and advanced cardiac life support was carried out for approximately 18 min. Immediate surgical repair was successful and the originally planned operation was then performed. Concurrently, mild therapeutic

A high-resolution record of holocene climate change in speleothem calcite from cold water cave, northeast iowa.

High-precision uranium-thorium mass spectrometric chronology and (18)O-(13)C isotopic analysis of speleothem calcite from Cold Water Cave in northeast Iowa have been used to chart mid-Holocene climate change. Significant shifts in dagger(18)O and dagger(13)C isotopic values coincide with well-documented Holocene vegetation changes. Temperature estimates based on (18)O/(16)O ratios suggest that the climate warmed rapidly by about 3 degrees C at 5900 years before present and then cooled by 4 degrees C at 3600 years before present. Initiation of a gradual increase in dagger(13)C at 5900 years bef

Brief Communication: An Incipient Coronal Caries Lesion on a Neandertal Molar Tooth From El Sidrón Cave (Northern Spain).

OBJECTIVES: Here, we present the findings of an incipient caries lesion on a permanent upper left second molar attributed to an adult male Neandertal individual from El Sidrón Cave. This study presents new data regarding the antiquity of human caries lesions, their development, and the possibility of the para-masticatory behavior component as a contributory factor in their origin, at least in Homo neanderthalensis. MATERIALS AND METHODS: The tooth was examined using a μCT scan to confirm the lesion and to evaluate its extent. Element chemical analysis was performed to check the decalcificatio

Oceanographic and hydroclimatic data explain depressed water level in the coastal karst hosting the decorated Paleolithic Cosquer cave (France).

Paleolithic decorated caves are home to a priceless heritage, but their preservation depends on hydroclimatic conditions within the cave. In coastal areas, changing sea levels pose a further threat to caves, as the sea floods the karst and obliterates Paleolithic artefacts. In this paper, we study the case of the Cosquer Cave, a half-submerged coastal cave located in southeastern France, home to Upper Paleolithic archeological remains. This is a very special case, where the sea represents both an opportunity and a threat for the preservation of an archeological site. The cave is confined, subm…

Meningeal architecture of Meckel's Cave and its surgical implications: A study using epoxy sheet plastination and three-dimensional reconstruction.

BACKGROUND: The architecture of Meckel's Cave (MC) dural walls and their associations with neighboring structures remain subjects of debate. Few studies have explored the thickness of the MC dural walls. The primary aim of this study was to elucidate the detailed meningeal architecture of MC and its relationship with the internal carotid artery, using sheet plastination technology to analyze the potential spreading pathways of tumors through the medial wall. METHODS: Ultrathin plastinated slices were prepared and examined in coronal, sagittal, and transverse planes. Three-dimensional reconstr

Cancer associated variant enrichment CAVE, a gene agnostic approach to identify low burden variants in chronic lymphocytic leukemia.

Intratumoral heterogeneity is an important clinical challenge because low burden clones expressing specific genetic alterations drive therapeutic resistance mechanisms. We have developed CAVE (cancer-associated variant enrichment), a gene-agnostic computational tool to identify specific enrichment of low-burden cancer driver variants in next-generation sequencing (NGS) data. For this study, CAVE was applied to TP53 in chronic lymphocytic leukemia (CLL) as a cancer model. Indeed, as TP53 mutations are part of treatment decision-making algorithms and low-burden variants are frequent, there is a

Cave-dwelling phlebotomine sand flies (Diptera: Psychodidae: Phlebotominae) in Thailand: population composition and pathogen detection of Bartonella and Trypanosoma.

BACKGROUND: Leishmaniasis is an emerging vector-borne disease that occurs in Thailand. Although Leishmania (Mundinia) parasites, the causative agents of the disease have been identified, the vectors of the disease remain unidentified. In the present study, we collected sand flies from three caves located in endemic areas of leishmaniasis, including Lampang and Chiang Rai in northern Thailand, and Songkhla in southern Thailand. METHODS: Female sand flies were identified on the basis of morphological characteristics and confirmed by cytochrome c oxidase subunit I (COI) sequencing. Sand fly DNA …

Genetic mapping of craniofacial traits in the Mexican tetra reveals loci associated with bite differences between cave and surface fish.

BACKGROUND: The Mexican tetra, Astyanax mexicanus, includes interfertile surface-dwelling and cave-dwelling morphs, enabling powerful studies aimed at uncovering genes involved in the evolution of cave-associated traits. Compared to surface fish, cavefish harbor several extreme traits within their skull, such as a protruding lower jaw, a wider gape, and an increase in tooth number. These features are highly variable between individual cavefish and even across different cavefish populations. RESULTS: To investigate these traits, we created a novel feeding behavior assay wherein bite impression

Early Pleistocene Horseshoe Bat Rhinolophus macrorhinus cimmerius subsp. nov. (Rhinolophidae, Chiroptera) from the Taurida Cave in Crimea.

Numerous remains (incomplete skull, cranial and mandibular fragments, and isolated teeth) of a large horseshoe bat of the Rhinolophus ferrumequinum group are described from the Lower Pleistocene depo-sits of the Taurida cave in the central Crimea. They are assigned to Rhinolophus macrorhinus cimmerius subsp. nov. In dental characters the new subspecies is less specialized than R. m. anomalidens Topál, 1979 from the Late Villafranchian of Central Europe, which implies the origin of the former from an earlier form morphologically close to R. m. macrorhinus Topál, 1963. The perfect preservation o

Robotic curative para-aortic lymph node dissection via INfra-mesocolon for gastric cancer: Robotic CAVING approach.

BACKGROUND: Two major surgical complications in D2 plus para-aortic nodal dissection (PAND) for gastric cancer (GC) have been pancreatic fistula and abdominal abscess [1]. The increase in these complications is due to the excessive mobilization of the pancreas. We previously reported a laparoscopic Curative PAND Via INfra-mesocolon for GC (CAVING), which minimizes mobilization of the pancreas [2]. Robotic surgery may be more comfortable than laparoscopic surgery for the surgeon performing this CAVING approach because robotic surgery has ergonomic benefits and advantages, such as native wrist-l

The Importance of STAT6 in a Schwannoma-like Grade III Solitary Fibrous Tumor/Hemangiopericytoma Located in the Cerebellopontine Angle and Meckel's Cave.

BACKGROUND: Hemangiopericytoma (HPC) is a rare mesenchymal tumor of the brain that occurs in the central nervous system as a solitary fibrous tumor. Surgical treatment of HPC is known to be beneficial, but results on the role of complete resection and adjuvant radiation are conflicting. However, it is often difficult to detect a central nervous system solitary fibrous tumor/HPC before surgery. We describe a presumed left trigeminal schwannoma that was histopathologically shown to be an HPC. CASE DESCRIPTION: A 65-year-old man presented with paresthesia of the second and third branches of the
