An in vitro study of ceftazidime and vancomycin concentrations in various fluid media: implications for use in treating endophthalmitis.

PURPOSE: To investigate the precipitation process of a mixture of vancomycin and ceftazidime by equilibrium dialysis and determine its subsequent effect on the level of free antibiotics for treatment of endophthalmitis. METHODS: Concentrations of vancomycin and ceftazidime in an equilibrium dialysis chamber were measured during the equilibrium process by high-performance liquid chromatography. Normal saline (NS), balanced salt solution (BSS), and vitreous were used separately as the medium of dialysis. RESULTS: Precipitation of ceftazidime occurred at 37 degrees C but not at room temperature…

Outbreak of infection in two UK hospitals caused by a strain of Klebsiella pneumoniae resistant to cefotaxime and ceftazidime.

During an 8-month period, Klebsiella pneumoniae resistant to cefotaxime and ceftazidime were isolated from 18 elderly patients in two closely-situated UK hospitals. Amongst these 18 patients, the organisms were isolated from urine samples of 17, from blood cultures of two and from a wound swab of one. The infected patients were located in nine different wards and several of the patients had been transferred between wards, within and between the two hospitals. All the bacterial isolates belonged to serotype K62, were non-typable or reacted only weakly with bacteriophage, showed similar plasmid …

In vitro activity of ceftazidime and other beta-lactam antibiotics against nosocomial strains of Pseudomonas aeruginosa.

The in vitro sensitivity of 300 nosocomial strains of Pseudomonas aeruginosa to ceftazidime was compared with their sensitivities to eleven other beta-lactam antibiotics. Concentrations of 2 mg/l of ceftazidime were sufficient to inhibit all of the strains tested including the carbenicillin-resistant ones. Ceftazidime shows considerably greater activity against Ps. aeruginosa than the other beta-lactam antibiotics.

Population pharmacokinetics and dosing regimen optimization of ceftazidime in plasma and cerebrospinal fluid of neonatal sepsis patients.

INTRODUCTION: Ceftazidime is a mainstay in the treatment of neonatal sepsis. However, the lack of pharmacokinetic (PK) data regarding its blood-cerebrospinal fluid (CSF) penetration in neonates hinders accurate exposure assessment and dose optimization for patients with concomitant central nervous system infections. METHODS: A population PK (PPK) model was established using plasma and CSF samples from 69 neonates using NLME software. Model predictive performance was validated using goodness-of-fit plots, bootstrapping, visual predictive checks, and normalized prediction distribution errors. M…

Ceftazidime-Induced Overexpression of MexJK-OprM Confers Resistance to Ceftolozane/Tazobactam in an Isolate of Pseudomonas aeruginosa.

OBJECTIVE: The emergence of resistance to ceftolozane/tazobactam (CTLZ/TAZ) in Pseudomonas aeruginosa poses a significant clinical challenge … microbial exposure assays were conducted to evaluate resistance acquisition and the impact of prior ceftazidime (CAZ) exposure. RESULTS: Pa-R and Pa-S were genetically identical and lacked carbapenemase genes. No mutations or overexpression of ampC were observed; however, mexJK expression was approximately 30-fold higher in Pa-R. RNA interference using mexJK …

Turbitest: an innovative rapid test for determining in vitro susceptibility to ceftazidime-avibactam plus aztreonam in Enterobacterales.

Treating carbapenem-resistant Enterobacterales, mainly those producing metallo-β-lactamases (MBL) is a subject of major concern. The combination aztreonam-avibactam (AZA) was developed in an attempt to overcome this challenge, as avibactam inhibits KPC and other serine-β-lactamases, enabling aztreonam (ATM) to act against MBL. However, in scenarios where AZA is not licensed so far, the combination of ATM with ceftazidime-avibactam (CZA) is an alternative that has been proved in vivo efficacy and in vitro synergic activity. We propose the TurbiTest, an alternative method based on turbidity meas…

Impact of the double deletion ΔG242-T243 in KPC-2 in the effectiveness of ceftazidime-avibactam and imipenem-relebactam.

Combinations of β-lactam-diazabicyclooctane inhibitors (DBOs) like ceftazidime-avibactam (CZA) and imipenem-relebactam (IMR) have shown efficacy in treating KPC-2-producing Klebsiella pneumoniae. However, CZA-resistant K. pneumoniae strains have been identified, often linked to substitutions and/or insertions/deletions in three different loops of KPC: (i) the Ω-loop region (amino acids 164-179), (ii) the 237-243 loop; and (iii) the 266-275 loop. This study investigates the impact of the double deletion ΔG242-T243 present in KPC-14. Our results demonstrate that the lower effectiveness of CZA ag…

Structural insights into the molecular mechanism of high-level ceftazidime-avibactam resistance conferred by CMY-185.

β-Lactamases can accumulate stepwise mutations that increase their resistance profiles to the latest β-lactam agents. CMY-185 is a CMY-2-like β-lactamase and was identified in an Escherichia coli clinical strain isolated from a patient who underwent treatment with ceftazidime-avibactam. CMY-185, possessing four amino acid substitutions of A114E, Q120K, V211S, and N346Y relative to CMY-2, confers high-level ceftazidime-avibactam resistance, and accumulation of the substitutions incrementally enhances the level of resistance to this agent. However, the functional role of each substitution and th…

The primary pharmacology of ceftazidime/avibactam: microbiology from clinical studies, and development of resistance during treatment.

As one of a series of thematically linked reviews of the primary pharmacology of the β-lactam/β-lactamase inhibitor combination, ceftazidime/avibactam, this article reviews the microbiological findings in drug-exposed patients. Earlier articles in the series focused on basic in vitro and in vivo translational biology (J Antimicrob Chemother 2022; 77: 2321-40 and 2341-52) and the development and mechanisms of resistance in vitro (J Antimicrob Chemother 2023: Epub ahead of print. doi: 10.1093/jac/dkac449). In clinical trials of ceftazidime/avibactam, combined favourable microbiological responses…

Ceftazidime-Avibactam plus Aztreonam for the Treatment of Infections by VIM-Type-Producing Gram-Negative Bacteria.

This is a retrospective single-center study of 24 patients who received ceftazidime-avibactam plus aztreonam (CZA/ATM) for the treatment of VIM-type-producing Gram-negative bacillus (GNB) infections. The bacteria isolated were Enterobacterales in 22 patients and Pseudomonas aeruginosa in 2. Sixteen out of 19 isolates showed synergistic activity. Two patients presented clinical failure at day 14, and the 30-day mortality was 17% (4/24). CZA/ATM could be considered an alternative therapy for VIM-type-producing GNB infections.

KPC-53, a KPC-3 Variant of Clinical Origin Associated with Reduced Susceptibility to Ceftazidime-Avibactam.

This study reports on the characterization of a Klebsiella pneumoniae clinical isolate showing high-level resistance to ceftazidime-avibactam associated with the production of KPC-53, a KPC-3 variant exhibiting a Leu167Glu168 duplication in the Ω-loop and a loss of carbapenemase activity. Whole-genome sequencing (WGS) revealed the presence of two copies of blaKPC-53, located on a pKpQIL-like plasmid and on a plasmid prophage of the Siphoviridae family, respectively. The present findings provide new insights into the mechanisms of resistance to ceftazidime-avibactam.

A successful antibiotic treatment by a new administration route: a case report of a subcutaneous administration of ceftazidime and tobramycin.

When intramuscular or intravenous administrations of parenteral drugs are not possible, the use of other routes (e.g., subcutaneous route) should be considered. We report a patient with Duchenne muscular dystrophy, who was hospitalized for acute pneumonia due to antibiotic-resistant strains of bacteria. Our patient was successfully recovered with antimicrobial therapy by subcutaneous administration of ceftazidime and tobramycin, for which no safety and efficacy data are available in humans. To the best of our knowledge, this case is the first supporting the subcutaneous administration safety a…

Treatment of a Klebsiella pneumoniae KPC cellulitis and gut decolonization with ceftazidime/avibactam in a migrant from Libya.

KPC-producing Klebsiella pneumoniae (KPC-Kp) is nowadays a global concern. Ceftazidime/avibactam is the most promising novel antibiotic combination available at the moment. We describe the case of a migrant with no risk factors for an infection due to multidrug resistant bacteria. He suffered from a KPC-Kp cellulitis and was treated with a ceftazidime/avibactam-meropenem-fosfomycin combination that not only eradicated the infection but also decontaminated his gut. Ceftazidime/avibactam-based treatment can be useful also in decontamination procedures.
