Ultrasonographic assessment of bypass capacity after revascularization surgery in moyamoya disease: a systematic review and single-arm meta-analysis.

PURPOSE: Moyamoya disease (MMD) is a chronic cerebrovascular disorder characterized by progressive arterial stenosis and fragile collateral formation, elevating stroke risk. Revascularization is the standard treatment, yet up to 27% of patients experience ischemic events within a year due to bypass insufficiency. While digital subtraction angiography (DSA) remains the gold standard for assessing bypass function, it is invasive and time-consuming. This study evaluates ultrasonography (US) as a noninvasive, cost-effective tool to assess bypass capacity post-revascularization in MMD. METHODS: A

The evolution and future directions of bypass surgery.

Cerebral bypass surgery is one of the most complex and elegant procedures in neurosurgery. It involves several meticulous steps that test the skills of even the most prepared neurosurgeons. This surgery has transcended its traditional role in arterial stenosis and atherosclerosis, expanding its applications to include flow preservation techniques for complex conditions such as aneurysms, tumors, and vascular malformations. The decreased number of procedures performed across many hospitals reflects the development of newer endovascular therapies but is also due to the results of the extracrania

How I do it: combined bypass for adult moyamoya disease with maximal consideration of cosmetic aspects.

BACKGROUND: Combined bypass, including direct and indirect procedures, has been recognized as the maximal revascularization to prevent further hemorrhagic or ischemic stroke in adult moyamoya disease (MMD). It is also important to consider cosmetic aspects when planning combined bypass for MMD. However, there are few reports that have described the cosmetic considerations in bypass surgery for MMD. METHODS: We demonstrate our surgical techniques aimed at achieving extended revascularization as well as excellent cosmetic outcomes with figures and video. CONCLUSION: Our combined bypass procedu

Collateral circulation after revascularization in moyamoya disease: influencing factors and underlying mechanisms.

Cerebral revascularization is a primary treatment for moyamoya disease and is generally considered to improve cerebral perfusion while potentially reducing the risk of stroke recurrence. However, the efficiency of postoperative collateral circulation formation varies significantly among individuals, and some patients continue to experience adverse events, such as recurrent ischemia or cerebral hemorrhage, which may be related to inadequate collateralization. Therefore, identifying the factors influencing the formation of collateral circulation after cerebral revascularization is critical for t…

Current status of revascularization surgery for Moyamoya disease: special consideration for its 'internal carotid-external carotid (IC-EC) conversion' as the physiological reorganization system.

Moyamoya disease is a chronic cerebrovascular disease with unknown etiology, which is characterized by bilateral steno-occlusive changes at the terminal portion of the internal carotid artery and an abnormal vascular network formation at the base of the brain. Moyamoya disease is known to have unique and dynamic nature to convert the vascular supply for the brain from internal carotid (IC) system to the external carotid (EC) system, as indicated by Suzuki's angiographic staging established in 1969. Insufficiency of this 'IC-EC conversion system' may result in cerebral ischemia, as well as in i

Effect of Revascularization on Headache Associated with Moyamoya Disease in Pediatric Patients.

Episodic headache is common in childhood moyamoya disease (MMD). The onset, mechanism, cause of headache and the effect of revascularization surgery on headache are not yet clear. We studied 10 cases of children (7 boys and 3 girls) younger than 18 years who underwent revascularization for MMD between 2009 and 2013. We evaluated frequency of headache and cerebral blood flow changes by single photon emission computed tomography brain imaging with [I123]-labeled iofetamine (IMP-SPECT) before and after surgery. Patients' ages ranged from 0 to 15 years at onset and 2 to 17 years at the time of sur

[Combination of double and indirect two-sided revascularization of the brain in the treatment of moyamoya disease].

An adult patient with progressive chronic cerebral ischemia associated with moyamoya disease who underwent combined revascularization of both cerebral hemispheres in step-by-step fashion is reported in the article. The feature of this case is a large volume of revascularization procedures with double-barrel extra-intracranial anastomoses combined with indirect synangioses. This surgical approach ensured early postoperative development of extensive collateral network and complete compensation of impaired cerebral circulation. Particular attention is paid to description and discussion of preoper

Rupture of a cerebral aneurysm following carotid endarterectomy.

Following carotid revascularization with either carotid endarterectomy or stenting, the impaired cerebral autoregulation can lead to cerebral hyperperfusion syndrome. This impaired autoregulation and increased flow may also put patients with unruptured aneurysms at risk for subarachnoid hemorrhage in the perioperative period. A patient is reported who underwent carotid endarterectomy for symptomatic carotid stenosis. A small anterior communicating artery aneurysm was identified preoperatively, which ruptured 2 days after carotid endarterectomy. Screening for cerebral aneurysms prior to carotid

Evaluating the role of graft angle on cerebral hemodynamics following direct cerebral bypass for moyamoya disease.

Direct cerebral bypass is a key treatment for moyamoya disease (MMD). This surgery grafts a donor vessel onto a recipient cerebral artery to boost blood flow to hypoperfused brain regions. Unlike coronary bypass, which restores downstream flow around a blockage, cerebral bypass for MMD reverses flow in the recipient vessel to perfuse the upstream network. Surgical decisions-such as donor vessel choice and anastomosis angle-significantly affect graft hemodynamics and outcomes. Yet these choices still rely on neurosurgeons' experience, lacking quantitative guidance. This study addresses that gap

Low hemoglobin is associated with postoperative cerebral infarction in moyamoya disease: development of a predictive model based on low hemoglobin.

BACKGROUND: Anemia is considered a risk factor for cardiovascular disease. However, there is little evidence regarding the relationship between hemoglobin (HB) and cerebral infarction after revascularization in patients with moyamoya disease (MMD). This study aimed to explore the relationship between postoperative cerebral infarction and HB in patients with MMD and to establish a predictive model. METHODS: Demographic information and different HB levels (the preoperative and postoperative HB, highest and lowest HB, and mean HB during hospitalization) of 112 patients with MMD were collected, o

Power Doppler evaluation of revascularization in childhood moyamoya.

Moyamoya disease is generally recognized in young children. One potential treatment is direct extra-intracranial bypass combined with indirect revascularization using encephalo-myo-synangiosis. Standard follow-up to assess neoangiogenesis includes repeat cerebral angiography, which is invasive. The authors studied whether noninvasive power Doppler imaging could evaluate the patency of the bypass and the degree of indirect revascularization. They found that transcranial power Doppler imaging is a valid noninvasive alternative to cerebral angiography.

Acetazolamide-Challenged Arterial Spin Labeling Detects Augmented Cerebrovascular Reserve After Surgery for Moyamoya.

BACKGROUND: Cerebrovascular reserve (CVR) inversely correlates with stroke risk in children with Moyamoya disease and may be improved by revascularization surgery. We hypothesized that acetazolamide-challenged arterial spin labeling MR perfusion quantifies augmentation of CVR achieved by revascularization and correlates with currently accepted angiographic scoring criteria. METHODS: We retrospectively identified pediatric patients with Moyamoya disease or syndrome who received cerebral revascularization at ≤18 years of age between 2012 and 2019 at our institution. Using acetazolamide-challeng…

Reconstruction of both M2 blood flow with a "Y" fashion anastomosis after excision of a large M1 bifurcation aneurysm: advantages and challenges.

BACKGROUND: Middle cerebral artery (MCA) M1 aneurysms with M2 branches originating from the aneurysm neck are difficult to treat because of blood flow reconstruction method selection, graft blood volume matching and various anastomoses. METHOD: We report a case of MCA M1 bifurcation aneurysm resection and reconstruction of the bifurcation using "Y" fashion anastomosis. Intraoperative DSA showed anastomotic stoma patency. This patient suffered transient left temporal ischemia and recovered well with a modified Rankin scale of 0 at discharge. CONCLUSION: This case demonstrates the application

"STA-MCA bypass with encephalo-duro-myo-synangiosis combined with bifrontal encephalo-duro-periosteal-synangiosis" as a one-staged revascularization strategy for pediatric moyamoya vasculopathy.

PURPOSE: Moyamoya vasculopathy progressively compromises cerebral blood flow resulting in chronic hypoperfusion. The middle cerebral artery (MCA) territory and the bifrontal areas are the regions most frequently affected. Although most techniques aim to only revascularize the MCA territory, augmentation of blood flow of the bifrontal areas is of importance in the pediatric moyamoya population since these regions play an important role in cognition, intellectual development, and in lower extremity and sphincter function. We recently described a one-staged surgical procedure combining revascular
