BACKGROUND: Although the mortality related to hip fracture and osteoporotic vertebral fracture have been reported, few studies have examined the mortality related to atlas and/or axis fractures. The aim of this study was to assess the association between mortality and atlas and/or axis fractures retrospectively and to elucidate the efficacy of surgical treatment.
METHODS: A total of 33 elderly patients who were treated for atlas and/or axis fractures at our institution between January 2012 and December 2018 were included in this study. These patients were divided into two groups: surgical tre

The clinical picture of the so-called subforaminal stenosis headache (Gutmann) and its surgical treatment (Roesner) have been presented. We have analyzed our findings in 119 patients and 55 operations. The clinical picture is characterized by increasing pain in the upper cervical and occipital region which in later stages may be accompanied also by other symptoms like disturbances of concentration, memory, libido, and potency, as well in some cases by symptoms similar to those of a secondary chronic myelopathy. The syndrome is caused by a subforaminal stenosis of the dural sac which regularly

Difficulty with singing is a rare but important complication following cervical spine surgery but there is little objective information regarding the cervical and head postural changes taking place during singing. The aim of this study was to identify postural changes in the cranio-cervical region associated with the demands of voice production in professional opera singing. The two Roentgen-cephalograms, one of which are taken whilst performing a specified singing task were taken from 18 professional opera students, 12 females (mean age 20.86 +/- 3.07 years) and six males (18.66 +/- 1.36 year

A patient presented with selective paralysis of the arms after having sustained a fall. X-ray of the cervical spine showed a type II odontoid fracture with posterior atlantoaxial dislocation. The diagnosis in the emergency room was cruciate paralysis, which is frequently associated with fractures of axis and/or atlas. However, magnetic resonance imaging (MRI) of the cervical spine revealed a lesion consistent with the acute central cord syndrome (CCS) at the C2-C6 level. The patient underwent posterior atlantoaxial arthrodesis to correct instability and was discharged, without much neurologica

Atlas injuries were produced in vitro and evaluated by radiographic examinations and anatomic studies from a clinical viewpoint. Ten cadaveric human upper cervical spine specimens were subjected to a high-speed axial impact. Injuries to the atlas consisted of six bursting fractures, two ruptures of the transverse ligament, one four-part fracture without a prominent bursting, and one posterior arch fracture. The major soft tissue injury involved the transverse ligament. There were five bony avulsions and three midsubstance tears. In this study, computed tomographic examinations clearly demonstr

The clinical findings of rotatory subluxation of the atlas are subtle and x-ray films involving the upper cervical segment are difficult to interpret. Nevertheless, patients with rotatory subluxation must be separated from those with "stiff necks," for they require hospital admission for cervical traction if complications are to be avoided. A case is reported and management and diagnosis are reviewed.

In a consecutive series of 30 hospitalized cervical injury patients, 18 proved to have widening of the prevertebral soft tissue space due to hematoma. The upper limit of normal width in 50 noninjury patients was used as a reference. Hematoma almost exclusively manifested at C1-C4 and proved to be closely related to fractures of anterior elements of the spine. Relatively small hematomas were encountered in odontoid fractures and compression fractures of vertebral bodies; relatively large hematoma in disruptive hyperextension injury. Large hematomas are explained by extensive anterior ligamentou

BACKGROUND: The curvature and range of motion of the cervical spine decrease after laminaplasty. However, to our knowledge these changes have not been studied prospectively. Also, the effect of laminaplasty on the mobility of the occipito-atlanto-axial joints has not been studied in detail. The purpose of our study was to prospectively evaluate the range of motion and curvature of the cervical spine, including the occipito-atlanto-axial joints, following laminaplasty.
METHODS: We conducted a prospective study of twenty-six patients who underwent cervical laminaplasty. They were followed for a

INTRODUCTION: Atlas ring fractures, which account for 1.3% of all spinal fractures, are predominantly managed conservatively. However, in certain cases, surgical treatment may be necessary depending on the type of fracture, degree of comminution, fracture location, and associated ligamentous injuries. Surgical stabilization frequently results in a posterior C1-2 or C0-2 fusion, which restricts movement, particularly craniocervical rotation. Coronal split fractures of the lateral mass need to be reduced and fixed due to dislocation, instability and secondary osteoarthritis. The preferred treatm

An occipitoatlantoaxial malformation and ventricular septal defect (VSD) were diagnosed in a 36-hour-old female camel. Physical examination revealed a firm protrusion of the dorsal aspect of the atlas and axis, tilting of the head to the left, and a grade V/VI systolic murmur. Neurological examination revealed proprioceptive deficits and ataxia of all 4 limbs. Radiographic examination and necropsy demonstrated malformation, fusion of the atlas to the occiput and hypoplasia of the dens of the axis, and subluxation of the atlantoaxial joint. Dorsoventral laxity of the atlantoaxial joint was also

Jefferson fracture is a very rare disease which occurs in only 2-13% of all cervical spinal fracture cases and in only 1.3% of total spinal fracture cases. A combination of an atlas-axis fracture occurs relatively frequently and with a higher incidence of neurological morbidity than isolated fractures. However, a Jefferson fracture, which is an isolated C-1 fracture, occurs very rarely. A 58-year-old woman was involved in a traffic accident and admitted to our hospital. She had a large scalp laceration in the parietal region and complained of nuchal pain. Neurological examination revealed noth

The C1 lateral mass screw technique is widely used for atlantoaxial fixation. However, C2 nerve dysfunction may occur as a complication of this procedure, compromising the quality of life of affected patients. This is a review of the topic of C2 nerve dysfunction associated with C1 lateral mass screw fixation and related research developments. The C2 nerve root is located in the space bordered superiorly by the posterior arch of C1 , inferiorly by the C2 lamina, anteriorly by the lateral atlantoaxial joint capsule, and posteriorly by the anterior edge of the ligamentum flavum. Some surgeons su
