Lateral Decubitus Positioning Increases Pleura-Plexus Distance in Ultrasound-Guided Supraclavicular Brachial Plexus Targeting.

INTRODUCTION: To evaluate whether lateral decubitus positioning increases the pleura-plexus distance at the supraclavicular level and to characterize its role as a modifiable anatomical factor relevant to ultrasound-guided interventions. METHODS: This prospective observational ultrasound study included 90 adults comprising individuals with complex regional pain syndrome, cervical spine stenosis, and healthy controls. Pleura-plexus distance was measured at end expiration in three standardized positions: semi-reclined supine, 45° lateral tilt, and lateral decubitus. Positional effects were asse

Thoracic outlet syndrome.

Diagnosis and treatment of thoracic outlet syndrome (TOS) involves neurologists, physiatrists, family physicians, orthopedic surgeons, vascular surgeons, thoracic surgeons, neurosurgeons and sometimes psychiatrists. It is generally accepted that TOS is caused by compression of brachial plexus elements or subclavian vessels in their passage from the cervical area toward the axilla and proximal arm either at the interscalene triangle, the costoclavicular triangle, or the subcoracoid space. Cervical ribs, anomalous muscles, and fibrous bands may further constrict these areas. Patients with thorac

Hemodynamic alterations in the paravertebral venous plexus after spinal injury.

PURPOSE: To assess the prevalence of altered venous hemodynamics after spinal cord injury. MATERIALS AND METHODS: The authors performed venography in 200 ambulant patients and 107 patients with spinal injury. The hemodynamic characteristics of the paravertebral venous plexus were documented after pedal injection of contrast material. The hemodynamic and clinical data were correlated in patients with spinal cord injury. RESULTS: Abnormal venous return via the paravertebral venous plexus in the absence of iliac or inferior vena caval occlusion was shown in 73 (68%) of the 107 patients with spi

Delayed Onset of Subclavian Artery Pseudoaneurysm With Brachial Plexus Compression Following Gunshot Wound Injury.

Early diagnosis of brachial plexus injuries is crucial to prevent long-term morbidity and improve outcomes. We present a unique case of delayed onset of brachial plexus compression two months following a traumatic gunshot injury causing multiple injuries including a T1 vertebral body comminuted fracture and pneumothorax. The patient experienced significant pain and progressive neurological examination changes during follow-up visits, and thus duplex ultrasound and computed tomography (CT) angiography were performed, which demonstrated a left subclavian artery pseudoaneurysm. This was managed o

Recurrent arterial and new-onset neurogenic thoracic outlet syndrome as a complication after previously inadequately excised first and cervical ribs.

Thoracic outlet syndrome commonly presents with the neurogenic subtype and can be caused in rare circumstances by an anatomic abnormality such as a cervical rib, for which surgical excision is the mainstay of management. An inadequately excised first or cervical rib can result in recurrent symptoms. We have reported the case of a 30-year-old woman who had presented with symptoms of right recurrent arterial and neurogenic thoracic outlet syndrome. She underwent repeat right-sided first rib and cervical rib resection with brachial plexus neural lysis and right carotid-axillary bypass via a combi

A Unique Case of Erdheim-Chester Disease With Cervical and Lumbosacral Nerve Involvement: FDG PET/CT Finding.

Cervical and lumbosacral nerve roots and plexus involvement in Erdheim-Chester disease (ECD) is rare. A 50-year-old man with progressive edema in bilateral lower extremities, numbness, weakness, and muscle atrophy of upper limb for 2 years had been misdiagnosed of neuritis or cervical spondylosis until he presented bone pain. In addition to bilateral symmetric involvement in long bones, bilateral hypermetabolism in cervical and lumbosacral nerve roots was found by FDG PET/CT scan. ECD was diagnosed by bone biopsy. This case reported the rare peripheral neural involvement and highlighted the ad

Direct repair of iatrogenic thoracic duct injury through lymphovenous anastomosis (LVA): A case report.

This is a case report of a 64-year-old male with cancer with an unknown primary and bilateral cervical lymph node metastases. Twelve months after chemo-, radio-, immunotherapy, and radical neck dissection, he presented with recurrent cervical metastases. The patient underwent radical revision neck dissection including the deep neck muscles of the cervical plexus and reconstruction with a free anterolateral thigh flap. During tumor resection, parts of the thoracic duct were removed which resulted in a large lymph leak. This was addressed by creating a lymphovenous anastomosis to a branch of the…

Intraoperative Shoulder Traction as Cause of C5 Palsy: Magnetic Resonance Imaging Study.

OBJECTIVE: During surgery, shoulder traction is often used for better fluoroscopic imaging of the lower cervical spine. Traction on the C5 root has been implicated as a potential cause of C5 palsy after cervical spine surgery. Using magnetic resonance imaging, this study was undertaken to determine the impact of upper extremity traction on the C5 root orientation. METHODS: In this study, 5 subjects underwent coronal magnetic resonance imaging of the cervical spine and left brachial plexus. Using a wrist restraint, sequential traction on the left arm with 10, 20, and 30 lb. was applied. Measur

Head and Neck Schwannomas: A Surgical Challenge-A Series of 5 Cases.

BACKGROUND: Schwannomas, also known as neurilemmomas, are benign peripheral nerve sheath tumors. They originate from any nerve covered with schwann cell sheath. Schwannomas constitute 25-45% of tumors of the head and neck. About 4% of head and neck schwannomas present as a sinonasal schwannoma. Brachial plexus schwannoma constitute only about 5% of schwannomas. Cervical vagal schwannomas constitute about 2-5% of neurogenic tumors. METHODS: We present a case series of 5 patients of schwannomas, one arising from the maxillary branch of trigeminal nerve in the maxillary sinus, second arising fro

Anatomy of the spinal accessory nerve plexus: relevance to head and neck cancer and atherosclerosis.

The term spinal accessory nerve plexus may be defined as the spinal accessory nerve with all its intra- and extracranial connections to other nerves, principally cranial, cervical, and sympathetic. The term is not new. This review examines its applied anatomy in head and neck cancer and atherosclerosis. Over the centuries, general studies of neural and vascular anatomy and embryology formed a basis for the understanding upon which the plexus is described. During the past century, its anatomy and blood supply have come to be better understood. The importance of almost all of the plexus to head,

Quantitative assessment of brachial plexus MRI for the diagnosis of chronic inflammatory neuropathies.

OBJECTIVE: This study aimed at developing a quantitative approach to assess abnormalities on MRI of the brachial plexus and the cervical roots in patients with chronic inflammatory demyelinating polyneuropathy (CIDP) and multifocal motor neuropathy (MMN) and to evaluate interrater reliability and its diagnostic value. METHODS: We performed a cross-sectional study in 50 patients with CIDP, 31 with MMN and 42 disease controls. We systematically measured cervical nerve root sizes on MRI bilaterally (C5, C6, C7) in the coronal [diameter (mm)] and sagittal planes [area (mm2)], next to the ganglion

Cervical rib in a young individual from the late medieval cemetery of Corfinio (12th-13th century CE, Italy): a case report and review of the literature.

Cervical ribs constitute the most important variants among ribs, particularly because of their proximity to the brachial plexus nerve network and subclavian artery. Thus, cervical ribs may exert excessive pressure upon these structures, producing a variety of symptoms that may be considered as a neuro-vascular complex. Cervical ribs are not uncommon in the catalogue of anatomic abnormalities in modern medicine, but few cases have been reported from historic skeletal material. We conducted a review with the existent articles published in palaeopathological literature, the historical background

Unilateral thoracic outlet syndrome in a case with bilateral cervical ribs, does it always produce compression?

BACKGROUND: The cervical rib can be a rare finding in asymptomatic subjects. When it produces compression, it affects variably the vasculo-nervous structures of the brachial plexus; the clinical history is progressive. The imaging studies indicate the site of osteo-neuro-vascular conflict and electrophysiological studies indicate the degree of neural involvement. CASE REPORT: We present a patient with an eight-month history of paresis and right dysesthesias, with an image of bilateral cervical ribs with different anatomical characteristics. This case discusses how a variant of the anatomy pro

The scalenus medius band and the seventh cervical transverse process.

A case is reported of compression of the lowest trunk of the brachial plexus by a fibrous scalenus medius band. The band, when present, connects the transverse process of the seventh cervical vertebra, to the first rib. Its presence may on occasions be inferred from the abnormal shape of the seventh cervical transverse process. Similar case reports are reviewed. A study of the length and shape of the transverse process in symptoms-free patients was contrasted with the appearance in a patient with a scalenus medius band and symptoms of brachial neuritis. It is concluded that the shape rather th

Exclusively epidural arteriovenous fistula in the cervical spine with spinal cord symptoms: case report.

OBJECTIVE AND IMPORTANCE: We describe the case of an epidural arteriovenous fistula (AVF) in the cervical spine draining only into the epidural and paravertebral plexus. An entirely epidural AVF having such drainage is extremely rare. CLINICAL PRESENTATION: A 24-year-old man presented with a 4-month history of gradually progressive sensory and motor disturbances of the upper and lower extremities. Magnetic resonance imaging and magnetic resonance angiography revealed a peridural vascular lesion within the canal compressing the spinal cord from C5 to T2. Diagnostic angiography revealed a perim

An epidural arteriovenous fistula studied with time-resolved imaging of contrast kinetics (TRICKS) sequence.

We describe the use of time-resolved MR angiography in the diagnosis of cervical epidural arteriovenous fistula before final diagnosis and embolization was achieved by digital subtraction angiography. A 42-year-old woman was referred to us because of headache and dizziness, in addition to radiculopathy of the right superior limb. Angiographic examinations documented a direct high-flow arteriovenous fistula between the right vertebral artery and the cervical epidural venous plexus. The point of fistula was located in the upper third of the cervical segment below the C2 arch. Time-resolved MR an

Extraordinary features of the intraspinal extradural and intradural nerve root courses.

OBJECTIVES: Very few studies have reported occasional intradural and extradural communications between adjacent nerve roots. These studies mostly focus on lumbosacral regions followed by cervical regions, and rarely in the thoracic region. DESIGN: The aim of this work is to point out some extraordinary extradural and intradural features of the intraspinal nerve root courses and their possible participation in radiculopathy. SUBJECTS AND METHODS: The anatomical study was performed in 43 cadavers. All intradural and extradural rami communicantes between nerve roots were examined histologically
