Caffeine, theophylline and theobromine are the most known methylxanthines as they are present in coffee, tea and/or chocolate. In the last decades, a huge experimental effort has been devoted to get insight into the variety of actions that these compounds exert in humans. From such knowledge it is known that methylxanthines have a great potential in prevention, therapy and/or management of a variety of diseases. The benefits of methylxanthine-based therapies in the apnea of prematurity and their translational potential in pediatric affections of the respiratory tract are here presented.

Better Than Chocolate investigates the inclusion of transgendered women in lesbian space. The film makes a strong statement that it is logical and natural to welcome trans women to the lesbian community, and that those who reject this inclusion are old-fashioned, violent, or both. It is my assertion that the film makes this statement at the expense of both those who have nuanced reservations toward trans inclusion and those transwomen who desire inclusion into lesbian communities but aren't hyperfeminine.

A case of severe methemoglobinemia and hemolytic anemia following ingestion of 5.6 gm of phenazopyridine with suicidal intent by a 16-year-old woman was successfully treated with intravenous methylene blue. Methemoglobinemia was suspected in this patient because of her intense central cyanosis, chocolate-colored blood, and minimal dyspnea. The patient's peripheral blood smear exhibited "bite cells." To the best of our knowledge, this is only the third such case reported in the literature. The pathophysiology, diagnosis, and therapy are reviewed.

Oculoglandular syndrome developed in a 9-year-old boy with a recent history of tick bites and minor eye trauma. Francisella tularensis was isolated on chemically supplemented chocolate agar from a swab of a conjunctival ulcer and an aspirate of a preauricular lymph node, confirming the diagnosis of tularemia. In addition, a rise in agglutination titer to the pathogen was detected in paired sera. The differential diagnosis of oculoglandular syndrome is discussed, with emphasis on the clinical characteristics which led to the diagnosis of tularemia.

Two cases of amylnitrite poisoning are presented. In both cases, severe methaemoglobinemia developed after ingestion of approximately 10 ml of amylnitrite. When admitted to hospital, both patients were deeply cyanosed, and arterial blood samples were noticed to be chocolate brown. They were intravenously treated with methylene blue. Within one hour the condition of both patients had improved dramatically, and blood gas-samples had normalised. In cases of cyanosis with no obvious genesis, poisoning with amylnitrite should be considered.

Bartonella quintana was isolated from 34 BACTEC nonradiometric aerobic resin blood cultures for 10 adults. Nine patients were initially diagnosed by routine acridine orange staining of routine cultures that had been incubated for 8 days. All subcultures grew on chocolate agar within 3 to 12 days (median, 6 days). The PLUS 26 high-volume aerobic resin medium, combined with acridine orange stain and subculture, is an effective system for detection and isolation of B. quintana from blood.

We report the finding of bleeding into right lung from a ruptured amoebic liver abscess. A 25-year-old male presented with pain in the right lower chest and cough associated with expectoration of chocolate-colored sputum. Radiographs of chest showed right-sided pleural effusion. Serial images of Tc99m-sulfur colloid study revealed progressive accumulation of radiotracer in the upper photopenic area extending into the right lung region suggestive of active bleeding from the abscess cavity.

The case is reported of a 19-year-old patient with recurrent Quincke edema with systemic symptoms. Based on observations of the course, diary entries and specific skin tests (scratch), sensitization to malt extract from germinated barley was diagnosed. The allergic reactions usually occurred after consumption of malt-containing chocolate drinks and malt-containing snack products. In cooperation with Prof. S. G. O. JOHANSSON, Stockholm, we have for the first time identified specific serum IgE antibodies against malt extract.

Methaemoglobinaemia is a rare but potentially dangerous haemoglobinopathy that is often underdiagnosed. It is one of the causes for unexplained cyanosis with dark-coloured blood, especially in the absence of cardiac or pulmonary pathology. In order to assess the risk of unintended exposure to food allergens, traditional deterministic risk assessment is usually applied, leading to inconsequential conclusions as 'an allergic reaction cannot be excluded'. TNO therefore developed a quantitative risk assessment model for allergens based on probabilistic techniques resulting in a more exhaustive risk assessment and more detailed information. By now, this approach is recognized as the future approach in allergen risk assessment. A case study (hazelnut proteins in chocolate spread) is presented as a proof of concept.

The literature has been reviewed to uncover the existence of head and neck pain syndromes caused by foods and food and drug combinations. Knowledge of the etiology of such syndromes may help in their treatment. At least twenty-five syndromes have been described. These include those induced by coloring and flavoring agents, alcoholic products, chocolate, coffee and tea, foods containing tyramine, vitamins, minerals, pesticides, and several others. Where possible, the active ingredients and the minimal amounts responsible for the induction of pain have been identified.
