RESEARCH QUESTION: Is measurement of hyperglycosylated HCG (hHCG) superior to beta-HCG (HCG+β) for early pregnancy detection after IVF and embryo transfer?
DESIGN: Blood samples were collected on day 4 (+1), 7 (+1) and 11 (+2) after embryo transfer from women aged 18-45 years undergoing first or second fresh or frozen IVF embryo transfer cycles. Biochemical pregnancy was assessed on-site by HCG determination on day 11; clinical pregnancy was assessed by ultrasound on day 21 (+4/-3). Serum hHCG (immunochemiluminometric assay) and HCG+β (Elecsys® HCG+β assay) concentrations were measured. Perfo

BACKGROUND: Current guidelines recommend that resolution of a complete molar pregnancy (CMP) can only be confirmed once a negative β-human chorionic gonadotropin (β-hCG) has been maintained for six months following uterine surgical evacuation. However, multiple studies have found that the risk of developing gestational trophoblastic neoplasia (GTN) once a negative β-hCG had been obtained is negligible, which suggests that a shorter follow-up may be reasonable.
AIM: To determine the trend in β-hCG following diagnosis of a CMP and the incidence of GTN, in a single unit.

Current opinions differ as to the biological significance and treatment of pure seminoma associated with the serological establishment of beta-human chorionic gonadotropin. Between December 1987 and April 1990, 147 patients with malignant testicular tumors were treated. Of these patients 47 (32%) had a pure seminoma. In 35 of the 47 patients we measured the tumor markers beta-human chorionic gonadotropins and alpha-fetoprotein in the cubital vein blood and testicular vein blood. There were elevated beta-human chorionic gonadotropin levels in the cubital veins of 26% of the patients, in agreeme

The case of a 54-year-old man with a retroperitoneal seminoma and normal testes is reported. Diagnosis was established late, when the tumor had become large and inoperable and produced symptoms of subocclusion. Onset of gynecomastia, an uncommon occurrence, had been recorded two years earlier and should have given rise to suspicion. The tumor was a histologically unmixed seminoma with very significant beta-HCG secretion. A chemotherapy-radiotherapy combination produced dramatic improvement. From data from a review of the literature, pathogenesis, diagnosis and management of this uncommon type

Serial radioimmunoassay determinations of serum beta hCG and methotrexate were compared in two patients with nonmetastatic gestational trophoblastic neoplasia (NMGTN) treated with Goldstein's modification of Bagshawe's intermediate-dose methotrexate-citrovorum factor rescue-treatment program. Pretreatment beta hCG levels (mIU/ml) ranged within the outer limits of the 10(3) log level. Following intravenous methotrexate, sharp serum peaks between 10(-6) and 10(-5) M were observed. Plasma disappearance was rapid with a 3 log drop noted within 24 hr to levels incapable of inhibiting DNA synthesis.

The value of measuring the separate sub-units of human chorionic gonadotrophin (free alpha-hCG and free beta-hCG) instead of total hCG together with alpha-fetoprotein (AFP) and unconjugated oestriol (uE3) was examined to determine the effect on the performance of serum screening for Down's syndrome between 15 and 22 weeks of pregnancy. The study was based on stored serum samples relating to 75 singleton pregnancies with fetal Down's syndrome and 367 unaffected singleton pregnancies, matched for maternal age, gestational age, and duration of storage of the serum sample, supplemented by data fro

We studied the dynamics of serum human chorionic gonadotrophin (HCG) and its free alpha (HCG alpha) and beta (HCG beta) subunits in 49 early pregnancies achieved by in-vitro fertilization (IVF) and embryo transfer. Of the 49 early pregnancies, nine were normal singleton pregnancies, 11 were twin pregnancies, 11 were ectopic, eight ended in a clinical (spontaneous) abortion and 10 ended in a preclinical abortion. The HCG, HCG alpha and HCG beta concentrations in serum were measured on days 12, 19 and 26 after embryo transfer. Most ectopic pregnancies could be distinguished from singleton (and t

To determine the neuroendocrine (NE) features of thymic epithelial tumor, immunohistochemistry and electron microscopy studies were performed on eight NE tumors (thymic carcinoids) and 26 non-NE tumors (nine thymic carcinomas, five atypical thymomas, and 12 thymomas other than lymphocytic thymoma). Immunohistochemical studies were performed with antibodies against general markers for NE cells (synaptophysin, alpha subunit of a guanine nucleotide-binding protein, Go, and small-cell lung carcinoma cluster 1 antigen), and a broad panel of antibodies for hormonal substances. Thymic carcinoid showe

BACKGROUND: Bilateral tubal pregnancy (BTP) is a rare and diagnostically challenging form of ectopic pregnancy. This report emphasizes the critical importance of serial beta-human chorionic gonadotropin (β-hCG) surveillance following conservative tubal surgery.
METHODS: A 29-year-old patient underwent laparoscopic salpingostomy for a right tubal pregnancy. Serial β-hCG measurements were performed postoperatively to monitor trophoblastic activity.
RESULTS: Instead of the expected decline, β-hCG levels rose postoperatively (from 809.29 mIU/mL to 942.47 mIU/mL by day 3). This abnormal trend pro

RATIONALE: Ectopic pregnancies tend to occur in the fallopian tubes. Secondary abdominal pregnancies (SAP) are much rarer. Laparoscopic salpingectomy is a commonly performed radical surgery. However, even if a patient has undergone radical surgery, we cannot take it lightly.
PATIENT CONCERNS: We report a relatively rare case here: an SAP on the hepatic diaphragmatic surface originating from a tubal abortion detected by a 2nd laparoscopy after primary salpingectomy. The patient was admitted to the local hospital for "ectopic pregnancy" half a month ago and underwent laparoscopic left salpingec

An ectopic pregnancy (EP) occurs when the fertilised ovum implants outside the endometrial cavity. An EP has an incidence of 1%, with the majority occurring in the fallopian tube. It has a maternal mortality of 0.2 per 1000, with about two-thirds of these deaths associated with substandard care. An interstitial pregnancy occurs when the EP implants in the interstitial part of the fallopian tube. An interstitial ectopic pregnancy (IEP) shows few early clinical symptoms, hence it is associated with serious or fatal bleeding and a mortality rate up to 2.5%. With the advent of transvaginal ultraso

An ectopic pregnancy (EP) accounts for 1-2% of all pregnancies, of which 90% implant in the fallopian tube. An abdominal ectopic pregnancy (AEP) is defined as an ectopic pregnancy occurring when the gestational sac is implanted in the peritoneal cavity outside the uterine cavity or the fallopian tube. Implantation sites may include the omentum, peritoneum of the pelvic and abdominal cavity, the uterine surface and abdominal organs such as the spleen, intestine, liver and blood vessels. Primary abdominal pregnancy results from fertilisation of the ovum in the abdominal cavity and secondary occu

Human chorionic gonadotropin (HCG) is an important indicator for the diagnosis of pregnancy. The authors report a unique case of cesarean scar ectopic pregnancy (CSEP) with negative urine and serum HCG levels, which was initially misdiagnosed as an intrauterine tumor despite the use of transvaginal ultrasound. Dilation and curettage was performed, which caused massive vaginal bleeding. Diagnostic hysteroscopy after uterine artery embolization and pathological examination of the surgical specimen confirmed the diagnosis of old CSEP. Postoperative follow-up showed that normal menstruation restar

Molar pregnancy, included in gestational trophoblastic disease, is a benign pathology with ability to metastasize, usually occurring with excessively high βhCG levels. Clinical scenario is usually a woman in extremes of reproductive age presenting with amenorrhoea, pain and vaginal blood loss; signs derived from high βhCG levels may be present (hyperthyroidism, hyperemesis). Diagnosis is based on a positive pregnancy test - usually a qualitative urinary test. The limitation of this test results from its inability to become positive in presence of markedly high levels of βhCG, saturating the an

INTRODUCTION: Beta-human chorionic gonadotropin (HCG-β) is considered to be a useful tumor marker for germ cell tumors (GCTs); however, various tumors other than GCTs, including cystic pituitary adenomas, Rathke's cleft cysts, and craniopharyngiomas, were reported to express HCG-β.
CASE REPORT: We herein present the case of a 5-year-old boy who presented with polyuria and had a solitary lesion in the neurohypophysis with a positive HCG-β titer in the cerebrospinal fluid. Under a preoperative diagnosis of germinoma, a biopsy was performed from the posterior pituitary lobe via the transsphenoid

BACKGROUND: Several studies have demonstrated that 25-77% of ectopic pregnancies spontaneously resolve with expectant management. However, expectant management is controversial and should be considered only for patients with small, unruptured gestational sacs, low beta-human chorionic gonadotropin (beta-hCG) levels and absence of symptoms. There is no consensus on how long to follow such patients.
CASES: Two patients with beta-hCG levels < 10 mIU/mL presented with ruptured ectopic pregnancy and hemoperitoneum.
CONCLUSION: While expectant management of a suspected ectopic pregnancy may allow

We present a 9-year-old boy with central precocious puberty and hydrocephalus caused by a human chorionic gonadotropin (beta-hCG) secreting pineal tumor. High levels of beta-hCG in the serum and cerebrospinal fluid were observed in this patient. The patient received radiotherapy and chemotherapy without surgical intervention. Subsequently, significant tumor regression was observed and the serum beta-hCG level normalized. There was no evidence of tumor recurrence at follow-up one year after treatment. The role of tissue biopsy to establish a diagnosis in pineal germ cell tumors remains controve

A rare case of primary choriocarcinoma of the jejunum occurred in a 45 year old Chinese man. This tumour was associated with massive melaena and a markedly raised serum human chorionic gonadotrophin (beta-hCG) concentration. Necropsy examination showed no primary lesion in other possible sites including testes, mediastinum, and pineal gland. Histologically, the tumour showed morphology typical of choriocarcinoma with no adenocarcinomatous element identified. The presence of choriocarcinoma in non-gonadal sites may be explained by retrodifferentiation or metaplasia of a conventional carcinomato
