Radiographic comparison of a fractured clavicle exhibiting a pseudo-arthrosis.

Bone remodeling is a natural process that is potentially problematic for radiographic comparisons because it can occur after antemortem (AM) imaging, thus interfering with the comparability of AM and postmortem (PM) radiographs from the same individual. While the effects of age-related remodeling have been studied, limited attention has been given to trauma-related remodeling with respect to radiographic comparisons. This report adds to the latter topic by presenting a case of AM clavicle fracture that developed into a pseudo-arthrosis over a 12-month period prior to the individual's death. Ev…

Excavated-type of rhomboid fossa of the clavicle: a radiological study.

The excavated type of rhomboid fossa of the clavicle is a relatively neglected anatomical structure that can potentially cause diagnostic problems. Its unilateral occurrence may be confused by the physician as avascular necrosis, osteomyelitis, or even a tumour. We studied 80 routine chest radiographs and identified the clavicles with excavated type of rhomboid fossa. The sex, sidedness, and handedness were recorded. An excavated type of rhomboid fossa was present in 43 clavicles (26.88%), appearing more frequently in males than in females. In addition, the incidence of the excavated type of r…

Congenital pseudarthrosis of the clavicle: a histopathological study in five patients.

Congenital pseudarthrosis of the clavicle is usually situated on the right clavicle. Pathological data on pseudarthrosis are scant. The histological analysis of five cases treated by 'en bloc' surgical resection are reported. Enchondral ossification was observed on both sides of the resected pseudarthrosis in two cases. Less characteristic enchondral features were noted in three cases. These findings confirm that the pseudarthrosis is due to the failure of coalescence of the two primary ossification centers of the clavicle.

Results of the Bosworth method for unstable fractures of the distal clavicle.

Eleven consecutive Neer's type II unstable fractures of the distal third of the clavicle were treated by open reduction and internal fixation, using a temporary Bosworth-type screw. In all cases, fracture healing occurred within 10 weeks. Shoulder function was restored to the pre-injury level. A Bosworth-type screw fixation is a relatively easy and safe technique of open reduction and internal fixation of type II fractures of the distal third of the clavicle.

Condensing osteitis of the clavicle: case report and review of the literature.

Osteitis condensans of the clavicle is a rare condition characterized by pain in the shoulder and often limitation of motion of the shoulder. The medical history and results of the physical examination, laboratory data, and radiographic studies (including computed tomography and magnetic resonance imaging), often establish the diagnosis. Patients who have slight or no pain usually require no treatment. Varying results have been reported for many different methods of treatment, including surgical excision, chemotherapy, antibiotics, nonsteroidal anti-inflammatory medications, radiation, local c…

Transitory osteonecrosis of the head of humerus after fracture of the clavicle.

The authors present a rare case of transitory osteonecrosis of the humeral head with anterior instability of the glenohumeral joint, which occurred in a young male aged 19 years, three months after fracturing his clavicle in a fall. Core decompression of the humeral epiphysis and anterior capsulomioplasty according to Symeonides was performed. Five months after surgery the patient was clinically cured. After having excluded the possibility of metabolic imbalance in the patient, we presumed a direct relationship between the trauma and degenerative pathology affecting the humeral epiphysis.

External fixation of the clavicle for fracture or non-union in adults.

A technique of osteosynthesis of the clavicle with Hoffmann external fixation was used to treat twenty patients for selected indications, including an open fracture, inability of a patient to tolerate prolonged conservative treatment, or a painful non-union. No vascular or pleural complication was observed. The average time that the external fixator was retained was fifty-one days. All of the clavicles united well. Mobility of the shoulder returned to normal in all patients.

Condensing osteitis of the clavicle: case report and review of the literature.

Osteitis condensans of the clavicle is a benign, often painful disorder of unknown etiology manifested by bony sclerosis of the clavicular head with an uninvolved sternoclavicular joint. The case presented demonstrates the characteristic scintigraphic findings of osteitis condensans of the clavicle. A review of the published pathologically proven cases reveals this disorder to have distinctive clinical and radiological features that allow differentiation from infection, neoplasia, and arthritides in most instances. The frequent observation, as in this case, of devitalized bone and marrow fibro…

[Rush pin osteosynthesis of the clavicles as an alternative to conservative treatment].

Fractures of the clavicle in the middle third usually undergo closed treatment by external fixation, because they heal spontaneously in most cases. However, a correct reposition and fixation by external manner is sometimes impossible, and consolidation in malposition with functional and esthetic disturbance occurs. External fixation can be painful and inconvenient to the patient, and congestion and paresthesia of the upper limbs may occur. Therefore the indication for internal fixation has been extended on fractures in persisting severe malposition despite external fixation and painful congest…

Non-union of the clavicle. Associated complications and surgical management.

Twenty-three patients who had a clavicular non-union were treated operatively at the Massachusetts General Hospital from 1974 to 1985. Twenty-one non-unions were the result of fracture and two, secondary to osteotomy. Twenty non-unions were located in the middle third of the clavicle, while three were in the lateral third. Radiographically, eighteen non-unions were atrophic and three, hypertrophic. Two non-unions resembled pseudarthrosis. Of the etiological factors that were reviewed the extent of displacement of the original fracture was the most significant. Associated complications of the n…

Step-Cut Clavicle Osteotomy and T-Shaped Iliac Crest Graft for Clavicle Nonunion Using 3-Dimensionally Planned Custom-Made Surgical Guides and Plate Fixation.

Clavicle fractures are common, with nonunion rates reaching up to 31% without surgery. Surgical treatment with plate fixation and iliac crest autograft enhances healing, yet accurate reconstruction remains challenging owing to clavicular morphology. This study presents a surgical technique using virtual surgical planning to create 3-dimensionally printed patient-specific surgical guides and a custom-made clavicle plate. This approach enables precise osteotomies, optimal graft shaping, and accurate anatomic reconstruction. The T-shaped autograft provides enhanced stability and increased bone co…

[Biplanar clavicle extension osteotomy : A case report of an extension osteotomy due to shortening of a healed clavicle fracture with sagittal split technique and corticocancellous graft].

Clavicle fractures healed in shortening or malposition are extremely rare, especially in adolescence. In extreme cases, they can result in massive pain and limitation of movement. A causal therapeutic approach is lengthening osteotomy using the sagittal split technique with tricortical iliac crest interposition. In the context of the case study of a young patient, the procedure, operation as well as documentation of the postoperative results are reported.

Absorbable Suture Fixation for Severely Displaced Type 4 Clavicle Fractures.

UNLABELLED: Type 4 clavicle fractures in skeletally immature patients are relatively uncommon and often undergo surgical intervention due to severe pain and uncertain remodeling of the displaced distal clavicle into the trapezius muscle. There remains no consensus on the optimal surgical technique for pediatric patients. Many commonly described methods for distal clavicle fractures in adults rely on rigid fixation or coracoid fixation. These techniques may increase the risk of implant complications or iatrogenic fractures in skeletally immature patients. The purpose of this paper is to demonst…

Bilateral Non-Displaced Clavicle Fractures: A Very Rare Case Report.

INTRODUCTION: Bilateral clavicle fractures are rare injuries, particularly when both fractures are non-displaced, accounting for less than 0.5% of all clavicle fractures. These injuries are typically associated with high-energy trauma, such as road traffic accidents or falls from height. Diagnosis may be challenging, as plain radiographs can miss subtle or non-displaced fractures, necessitating advanced imaging, such as computed tomography (CT) for accurate detection. CASE REPORT: We report the case of a 21-year-old male who sustained bilateral non-displaced midshaft clavicle fractures follow…
