Additional therapy for cholesterol lowering in ezetimibe-treated, statin-intolerant patients in clinical practice: results from an internal audit of a university lipid clinic.

OBJECTIVE: The aim of our study was to evaluate the tolerability and efficacy of alternative approaches to improve cholesterolemia control in patients with statin-related myalgia treated with ezetimibe. RESEARCH DESIGN AND METHODS: We retrospectively evaluated 3534 Clinical Report Forms (CRFs) filled in the period June 2012-June 2015 for first visits to the lipid clinic of the University of Bologna. For this study, we selected 252 CRFs based on the following criteria: statin-related myalgia, previous failed treatment with at least two low-dosed statins, well tolerated treatment with ezetimibe

Management and Outcomes of Diabetic Foot Complications Requiring Surgical Intervention at a Sudanese First-Level Hospital: A Prospective Clinical Audit.

BACKGROUND AND AIMS: Diabetic foot complications, encompassing ulcers, abscesses, and tissue necrosis, are common and serious complications of diabetes mellitus that can significantly impact quality of life and potentially lead to amputation, particularly in the presence of risk factors and inadequate care. This prospective clinical audit evaluated the management and outcomes of diabetic foot complications at a first-level hospital in Hilaliya, Sudan. METHODS: A prospective clinical audit of 15 patients requiring surgical intervention was conducted from August to November 2023. Data on demogr…

Clinical audit of plasma methylmalonic acid (MMA) requests following publication of NICE guidelines on Vitamin B12 deficiency in over 16s.

BackgroundVitamin B12 deficiency can cause anaemia, fatigue, and neurological sequelae. Total B12 assays may not be reflective of tissue B12. Methylmalonic acid (MMA) can assess tissue B12 more accurately. NICE published clinical guidelines on Vitamin B12 deficiency in adults in 2024. They outlined MMA testing in two scenarios: in patients where nitrous oxide use is possible or who have an indeterminate total B12 result (180-350 ng/L) alongside symptoms or signs of B12 deficiency. MMA testing is costly, not readily available, and time consuming, so increased use will have implications.MethodsT

Barriers to cure in advanced non-seminomatous germ cell tumors: a clinical audit from a resource-limited region in India.

PURPOSE: Non-seminomatous germ cell tumors (NSGCT) are considered highly curable malignancies in developed nations. However, outcomes in low- and middle-income countries remain suboptimal, and literature describing outcomes and barriers to cure in advanced NSGCT from resource-limited settings is scarce. METHODS: A retrospective analysis was conducted among patients with advanced NSGCT treated at a tertiary cancer center in central India. Primary endpoint was overall survival (OS), while secondary endpoints included progression-free survival (PFS), and treatment-related toxicity. RESULTS: Among

Improving adherence to venous thromboembolism prophylaxis guidelines in surgical inpatients: a closed-loop clinical audit in a resource-limited setting.

BACKGROUND: Venous thromboembolism (VTE) is a major cause of preventable morbidity and mortality among hospitalized surgical patients. Although evidence-based guidelines exist, adherence remains suboptimal, particularly in low- and middle-income countries. This audit assessed adherence to National Institute for Health and Care Excellence (NICE) guidelines for VTE prophylaxis among surgical inpatients and evaluated the impact of a targeted educational intervention. MATERIALS & METHODS: A prospective clinical audit was conducted at a tertiary care hospital. Adult surgical inpatients were assess…

Improving admission clerking standards and physical health assessments in febrile patients: A prospective clinical audit at a tertiary care hospital.

INTRODUCTION: Clinical audit is an effective tool to improve patient care, which in turn has an impact on clinical outcome.Patients who present with fever usually are generally admitted based on competency-based criteria, where admission is determined on the severity of the illness assessed through clinical and laboratory findings. Bacterial infections were linked to high sepsis-related mortality. This highlights the importance of comprehensive and standardized admission clerking and physical assessment for patients presenting with fever. This audit aimed to evaluate the documentation of histo…

Retrospective audit reviewing accuracy of clinical diagnosis of geographic atrophy in a single centre private tertiary retinal practice in Australia.

Accurate diagnosis of both age-related macular degeneration (AMD) and inherited retinal diseases (IRD) with macular atrophy is important because treatments for both conditions are emerging. Phenotypical similarities between macular atrophy associated with AMD (geographic atrophy, GA) and IRD-associated atrophy exist, which can make accurate diagnosis challenging in clinical practice. Misdiagnosis may lead to inappropriate treatment strategies and missed opportunities for disease-specific interventions. A retrospective clinical review of medical records of people diagnosed with AMD between 1995

A Prospective Clinical Audit of Sterile Surgical Glove-Donning Practices Among Young Surgeons at a Tertiary Care Hospital in Sudan.

Background Adherence to aseptic protocols and proper sterilization is critical for optimal patient recovery post-surgery. The standard procedure for donning sterile surgical gloves helps manage infection risk and maintain surgical site cleanliness by following aseptic principles. This study evaluates adherence to these protocols among young surgeons at a tertiary care hospital in Sudan. Methods This prospective audit included 42 young residents and house officers at a tertiary care hospital in Sudan, following ethical clearance. Compliance with sterile surgical glove-donning practices was asse

Prospective clinical audit of two neuromodulatory treatments for fecal incontinence: sacral nerve stimulation (SNS) and percutaneous tibial nerve stimulation (PTNS).

BACKGROUND AND PURPOSE: Two types of neuromodulation are currently practised for the treatment of fecal incontinence (FI): sacral nerve stimulation (SNS) and percutaneous tibial nerve stimulation (PTNS). This study compares these therapies, as no data exist to prospectively assess their relative efficacy and costs. METHODS: The subjects of this study were two distinct cohorts undergoing SNS (between 2003 and 2008) or PTNS (2008-onwards) for FI. Clinical outcomes assessed at 3 months included incontinence scores and the number of weekly incontinence episodes. The direct medical costs for each

Should treatment distinguish anxiogenic from anxiolytic obsessive-compulsive ruminations? Results of a pilot controlled study and of a clinical audit.

In a small pilot controlled study over 8 weeks, 12 obsessive-compulsive ruminators listened for 2 h daily to their own audiotaped voice either (1) describing their anxiogenic thoughts (exposure) but omitting anxiolytic thoughts (mental/cognitive rituals), or (2) reading neutral prose or poetry. Taking all patients, both groups improved similarly. However, exposure patients who became anxious early in exposure slightly more improved. Consistent with this, in a clinical audit of 57 ruminators treated by trainee clinicians over 12 years, outcome improved significantly once practice changed so tha…

Improving the Quality and Completeness of Cesarean Section Operative Note Documentation Through a Structured Proforma: A Closed-Loop Clinical Audit.

Background High-quality operative documentation is essential for patient safety, continuity of care, clinical governance, and medicolegal accountability. Despite established professional guidance, deficiencies in cesarean section (CS) operative note documentation remain common in many healthcare settings. This audit aimed to determine the effect of implementing a structured operative note proforma, supported by educational sessions, departmental feedback, and visual reminder posters, on compliance with operative documentation standards. Methods A closed-loop clinical audit was conducted at Al-…

Preventing Participant Leakage in Reissued Clinical Artificial Intelligence Benchmarks: A Biomedical Informatics Evaluation Protocol and DAIC-WOZ/E-DAIC Audit.

BACKGROUND: Biomedical informatics increasingly reuses clinical artificial intelligence (AI) benchmarks, yet successor releases are often treated as independent external corpora. If the same participants reappear under new corpus names, features, or partition files, evaluation can become internal while reported as external. OBJECTIVES: The objective of this study is to define successor-release participant leakage as a benchmark-integrity threat, operationalize a preevaluation audit and leak-free protocol, and demonstrate its impact using the Distress Analysis Interview Corpus, Wizard-of-Oz co

Demographic prompt cues shift clinical recommendations in multimodal large language models: a multi-model audit of 25,380 dental radiograph assessments.

BACKGROUND: Multimodal large language models (LLMs) are increasingly being evaluated for clinical image interpretation, but whether patient demographic cues influence their recommendations remains unclear in dental radiology-a leading domain for AI-assisted diagnostics, and one marked by pronounced disparities in oral health. METHODS: We conducted a within-image, between-condition audit study using 705 panoramic dental radiographs from the publicly available DENTEX dataset. Each image was submitted to three commercial multimodal LLMs (Gemini 2.5 Flash, GPT-5.4, Claude Sonnet 4.6) under 12 exp

Disclosing possible nonmedically indicated cesarean sections in 5 high-volume urban maternity units in Tanzania: a criterion-based clinical audit.

BACKGROUND: Globally, the cesarean section rate has increased dramatically with many cesarean sections being performed on questionable medical indications. Particularly in urban areas of sub-Saharan Africa, the cesarean section rate is currently increasing rapidly. This potentially undermines the positive momentum of increased facility births and may be a central contributor to a growing "urban disadvantage" in maternal and perinatal health, which is seen in some settings. OBJECTIVE: To assess to what extent cesarean section indications follow evidence-based, locally co-created audit criteria

Nature and extent of outpatient podiatry service utilisation in people with diabetes undergoing minor foot amputations: a retrospective clinical audit.

BACKGROUND: People with diabetes are at high risk of foot complications that can lead to lower extremity amputations. National standards suggest that early assessment and management by a podiatry led multidisciplinary high-risk foot clinic (HRFC) helps to reduce complications. This review is a retrospective audit of the Central Coast Local Health District (CCLHD) podiatry department service utilisation in people with diabetes who had undergone a minor foot amputation. METHODS: All people with diabetes who had minor foot amputations in the calendar year 2017 in the CCLHD in New South Wales wer

Dropping risk stratification with subsequent treatment-risk paradox in non ST elevation acute coronary syndromes: a clinical audit in Iraq.

BACKGROUND: Risk stratification is the cornerstone in managing patients with Non-ST Elevation Acute Coronary Syndromes (NSTE-ACS) and can attenuate the unjustified variability in treatment and guide the intervention decision notwithstanding its impact on better healthcare resources use. This study sought to disclose real adherence to guidelines in risk stratification of NSTE-ACS patients and in adopting intervention decision in practice. METHODS: Multicentre prospective study recruited NSTE-ACS patients. Baseline characteristics were collected, TIMI (Thrombolysis in Myocardial Infarction) and

Patterns of prevalence and contemporary clinical management strategies in complicated acute biliary calculous disease: an ESTES 'snapshot audit' of practice.

BACKGROUND: Acute complications of biliary calculi are common, morbid, and complex to manage. Variability exists in the techniques utilized to treat these conditions at an individual surgeon and unit level. AIM: To identify, through an international prospective nonrandomized cohort study, the epidemiology and areas of practice variability in management of acute complicated calculous biliary disease (ACCBD) and to correlate them against reported outcomes. METHODS: A preplanned analysis of the European Society of Trauma and Emergency Surgery (ESTES) 2018 Complicated Biliary Calculous Disease a
