A male aged 30 suffered from toxic shock syndrome after septorhinoplasty with positioning of a tampon. Initial treatment consisted of removing the tampon and supportive care, as a result of which the patient recovered. The patient was a carrier of Staphylococcus aureus which produced toxic shock syndrome toxin-I (TSST-I). Anti-TSST-I antibodies were already found in the serum in the initial phase of the disease.

In a trial of 2 grams oral single dose of amoxycillin on 70 patients with acute gonococcal urethritis, 64 patients had full bacteriological cure, 4 patients were lost to follow up and presumed cured, two patients, still had gonococcal urethritis one week after treatment but one admitted to re-exposure and subsequently recovered with a second 2 gram single dose of "amoxil". CONCLUSIONS: In distinction to the typical bacteria of acute rhinosinusitis, S. viridans is the leading cause of rhinosinusitis complications. It is not merely a commensal organism of the upper respiratory tract.

We report a rare case of pyomyositis of the iliacus muscle in a 29-year-old woman. After 2 weeks of adequate treatment, secondary septic sacroiliitis occurred, a complication that had not been described previously. Pyomyositis of the iliacus muscle must be considered in the differential diagnosis of acute pain in the hip region.

A case of purulent spinal epidural abscess in a 20-day-old girl is presented. The patient had symptoms of fever, localized back pain and swelling over the interscapular area. The diagnosis was made by magnetic resonance imaging. Purulent material grew Staphylococcus aureus from the lesion. Because of severe cord compression surgical laminotomy and drainage were performed, and antibiotics were administered. One year later she was doing well without neurologic sequelae.

Ersipelas, a common disease of swine, is caused by Erysipelothrix rhusiopathiae. The organism was isolated in a blood culture taken from an infected captive dolphin. The dolphin showed typical subacute symptoms of square- and diamond-shaped skin lesions as seen in swine. It was surmised, in retrospect, that the disease was secondary to a primary pneumonia. The symptoms, clinical pathology and other special examinations, treatment and response are discussed.
