"He Bore it Like a Scarlet Letter": Medical Student Reflections on Substance Use Disorders.

OBJECTIVE: Substance abuse in the context of the opioid crisis presents a major public health concern. Despite some evidence that medical students' attitudes towards substance use disorders worsen during medical school, very few studies have examined how students' early clinical experiences with substance use disorders shape their views of this clinical population. This study uses student reflective essays to explore these formative educational experiences. METHODS: Using content analysis, the authors analyzed a collection of 802 medical student reflective essays written during core clerkship

Validation of hospital register-based diagnosis of Parkinson's disease.

INTRODUCTION: Denmark has a long-standing tradition of maintaining one of the world's largest health science specialized register data bases as the National Hospital Register (NHR). To estimate the prevalence and incidence of diseases, the correctness of the diagnoses recorded is critical. Parkinson's disease (PD) is a neurodegenerative disorder and only 75-80% of patients with parkinsonism will have idiopathic PD (iPD). It is necessary to follow patients in order to determine if some of them will develop other neurodegenerative diseases and a one-time-only diagnostic code for iPD reported in

Ethical analysis of childhood vaccination information processes among Turkish parents in the United States.

INTRODUCTION: Highly educated, established immigrant populations remain understudied in vaccine-hesitancy research, which has focused primarily on refugee and low-literacy samples. This study examines how foreign-born Muslim Turkish parents in the US experience informed consent for childhood vaccination within a legal context that treats vaccination as a mandatory condition of school enrollment. METHODS: A qualitative descriptive design was employed using semi-structured in-depth interviews with 13 foreign-born Turkish Muslim parents recruited via snowball sampling across five U. S. states. Qualitative analysis of a health systems science (HSS) course incorporating early clinical exposure.

Health systems science (HSS) is increasingly recognized as a core component of contemporary medical education. Although early clinical exposure (ECE) has been shown to enhance students' understanding of clinical practice, evidence regarding its role in shaping HSS-related perspectives in Korean medicine (KM) education remains limited. This study explored how participation in an HSS-based ECE program influenced KM students' perceptions and learning experiences. Between 2023 and 2025, a qualitative study was conducted with 65 third-year students enrolled in a KM college. Following HSS-oriented l

A qualitative study on emergency department nurses' experiences of coping with moral distress based on self-efficacy theory.

OBJECTIVE: Grounded in Bandura's self-efficacy theory, this study aims to identify the key factors influencing how emergency department (ED) nurses cope with moral distress, providing an evidence base for targeted interventions to enhance their coping capacity and psychological well-being. METHODS: A qualitative descriptive design was adopted. Between November and December 2025, 17 ED nurses from a tertiary general hospital were recruited through purposive sampling with maximum variation. One-to-one semi-structured interviews were conducted and analyzed using thematic framework analysis. To e

Prospective multi-phase observational study evaluating local field potentials to guide deep brain stimulation programming in dystonia at a UK Tertiary Neurosciences Centre (LFP-DYT): a protocol.

INTRODUCTION: Deep brain stimulation (DBS) for dystonia is effective but programming optimisation can take months. Local field potentials (LFPs) recorded by the Medtronic Percept device may provide biomarkers to guide stimulation. This study will prospectively evaluate whether chronic LFP profiles correlate with clinical outcomes and can inform DBS programming strategies. METHODS AND ANALYSIS: LFP-DYT is a single-centre, multi-phase observational study at Newcastle upon Tyne National Health Service (NHS) Foundation Trust. An internal pilot (Cohort 1) will refine recording workflows, followed

Humanistic care in the ICU: effects on patient dignity, delirium and family satisfaction - a non-randomised clinical trial protocol.

INTRODUCTION: Critical care patients face varying degrees of physical and psychological stress; in recent decades, the increasing focus on technologies and clinical processes has led to dehumanisation. In contrast, humanistic care has been introduced as an essential care paradigm, emphasising comprehensive care and considering all stakeholders in the care process (patient, family and staff). In addition to improving clinical outcomes, this approach focuses on family satisfaction, preserving the human dignity of patients and reducing the incidence of acute cognitive disorders such as delirium;

International migration of Africa-born health workers: a scoping review of drivers and policies, 2010-2024.

BACKGROUND: Africa faces a critical shortage of human resources for health (HRH), crucial for improving population health. We described drivers of and policies to manage the migration of Africa-born HRH from source (African) countries in Africa to destination (non-African) countries. METHODS: We conducted a scoping review, using the Arksey and O'Malley framework, using data from peer-reviewed articles (2010-2024) and gray literature from institutions like Africa Centers for Disease Control (CDC) and World Health Organization (WHO). We used push-pull theory to identify economic, professional,

Haemodynamic response to peripheral and inspiratory resistance training in patients with coronary artery disease (HEMODYRET): study protocol of a prospective, single-centre, randomised crossover trial.

INTRODUCTION: Due to the documented benefits of peripheral resistance training (RT) and inspiratory resistance training, known as inspiratory muscle training (IMT), in patients with cardiovascular disease, both exercise forms are regularly used in cardiac rehabilitation. However, little is known about the haemodynamic responses to different intensities of dynamic RT, isometric RT, and IMT in patients with coronary artery disease (CAD). This study is designed to evaluate and compare the acute haemodynamic responses elicited by different RT and IMT modalities in patients with CAD. METHODS AND A

Multicentre pilot randomised control trial of a self-directed exergaming intervention for poststroke upper limb rehabilitation: research protocol.

INTRODUCTION: Technology-facilitated, self-directed upper limb (UL) rehabilitation, as an adjunct to conventional care, could enhance poststroke UL recovery compared with conventional care alone, without imposing additional resource burden. The proposed pilot randomised controlled trial (RCT) aims to assess whether stroke survivors will engage in self-directed UL training, explore factors associated with intervention adherence and evaluate the study design for an RCT testing the efficacy of a self-directed exer-gaming intervention for UL recovery after stroke. METHODS AND ANALYSIS: This is a

Study protocol of a randomised, double-blind, placebo-controlled, two-arm parallel-group, multi-centre phase 3 pivotal trial to investigate the efficacy and safety of recombinant human alkaline phosphatase for treatment of patients with sepsis-associated acute kidney injury.

INTRODUCTION: Sepsis, the leading cause of acute kidney injury (AKI), is associated with a high morbidity and mortality. Alkaline phosphatase (ALP) is an endogenous detoxifying enzyme. A recombinant human ALP compound, ilofotase alfa, showed no safety or tolerability concerns in a phase 2 trial. Renal function improvement over 28 days was significantly greater in the ilofotase alfa group. Moreover, a significant relative reduction in 28-day all-cause mortality of >40% was observed. A follow-up trial has been designed to confirm these findings. METHODS AND ANALYSIS: This is a phase 3, global,

Prospective evaluation of real-world safety, symptoms improvement and adherence to mavacamten among Iranian patients with hypertrophic obstructive cardiomyopathy (PERSIA-HOCM): protocol of a multicentre observational study.

INTRODUCTION: Hypertrophic cardiomyopathy (HCM) is a genetic cardiovascular disorder affecting approximately 1 in 200 to 1 in 500 adults, with nearly 70% exhibiting the obstructive phenotype Hypertrophic Obstructive Cardiomyopathy (HOCM). Randomised trials have demonstrated the efficacy of mavacamten, a first-in-class cardiac myosin inhibitor which significantly reduced outflow tract gradients and improved symptoms, exercise capacity and quality of life in the HOCM cases. Despite these findings, prospective real-world data on its safety and effectiveness remain limited, particularly in non-Wes

A journey through the history of Neurogenetics.

Since the late 19th century, when several inherited neurological disorders were described, the close relationship between Neurology and heredity were well documented by several authors in a pre-genetic era. The term Neurogenetics came to integrate two large sciences and clinical practices: Neurology and Genetics. Neurogenetics is the emerging field that studies the correlation between genetic code and the development and function of the nervous system, including behavioral traits, personality and neurological diseases. In this historical note, a timeline shows the main events and contributors

Narrative Identity in Third Party Reproduction: Normative Aspects and Ethical Challenges.

In the last few decades, assisted reproduction has introduced new challenges to the way people conceive and build their families. While the numbers of donor-conceived (DC) individuals have increased worldwide, there are still many controversies concerning access to donor information. Is there a fundamental moral right to know one's genetic background? What does identity in DC families mean? Is there any relationship between identity formation and disclosure of genetic origins? These questions are addressed by analysing core regulatory discourse (ethical recommendations and codes of practice).

Standards of Care for Assaulted Patients in Custody.

Patients who are incarcerated routinely experience physical and sexual violence, yet clinicians are often limited in their ability to address structural factors and ensure safety after discharge. This article explores the ethical and clinical challenges physicians face in treating incarcerated individuals, particularly dual loyalty conflicts between patient advocacy and institutional pressures. We argue that incarcerated patients should receive the standard of care afforded to other vulnerable populations, and that clinicians and health systems must embrace a broader role in violence preventio
