OBJECTIVES: To assess emergency physician prescribing for simple extremity fractures-specifically, distal radius fractures-and describe the opportunity for reducing opioid prescribing.
METHODS: An electronic survey was distributed to 1238 emergency physicians employed by a nationwide practice serving 220 sites in 20 states. The survey presented two plain film views of a simple Colles fracture and asked: "For the last patient you discharged . . . with the above injury, which pain medications did you prescribe or recommend?" Responses were collected using a clickable checklist of common opioid

The cause of osteoporosis, a condition in which bone mass is decreased to a point where structural failure may occur, is unknown; many facto … e vertebrae and the distal radius (the wrist). Vertebral fractures and wrist fractures (also called Colles' fractures) result. Type II, or senile osteoporosis, is age-related, occurs in men as well as women, and is characterized by fractures of the hip and humerus. There is a loss of cortical as well as trabecular bone.Estrogen deficiency, increasing age, smoking, …

INTRODUCTION: Intra-articular distal radial fractures in young subjects occur in severe trauma. Articular reduction needs to be anatomical. We report four cases with the particularity of having a 90° or 180° rotated lunate fossa. Our goal is to bring out the positive aspects of surgical procedure by volar medial approach and to assess long-term functionnal and radiological results.
MATERIAL AND METHODS: Our study focused on four men whose average age was 27 (age range from 19 to 43). The fractures were type IV according to Melone's classification. The associated lesions included: one fracture

Ligamentotaxis is the principle of molding fracture fragments into alignment as a result of tension applied across a fracture by the surrounding intact soft tissues. Uniplanar ligamentotaxis obtained by longitudinal traction does not always restore palmar tilt to the distal radius. Multiplanar ligamentotaxis extends the principle of uniplanar ligamentotaxis to include translation of the hand in the dorsal-palmar and the radial-ulnar planes to effect appositional and tilting alignment of the distal fragment(s) of a fractured radius. Use of an external fixator that allows adjustments in multiple

Age-associated bone loss is transiently accelerated by the menopause and can lower bone mass below the threshold of mechanical resistance. The appearance of fractures allows the clinical diagnosis of osteoporosis (OP). In addition to age and menopause, major causal factors of OP are low peak bone mass in young adults for genetic or nutritional reasons, and inadequate nutrition in advanced age. Endocrine and digestive diseases, corticotherapy, physical inactivity and low body weight are among other additional causes sometimes calling for specific treatment. Postmenopausal OP predominantly invol

INTRODUCTION: Kapandji described intra-focal pin fixation of distal radial fracture in which the pins pass through the fracture line and anchor to the far cortex. These pins aid reduction of the fracture and maintain reduction by buttressing the distal fragments.
MATERIALS AND METHODS: Between January 1996 to December 1997, 186 distal radial fractures were treated with intra-focal pin fixation. One hundred and seventy-seven patients were available for review after an average follow-up of 30.8 months, ranging from 24 to 36 months. In 133 fractures, a trans-styloid pin was inserted to supplemen

BACKGROUND: Bone stimulation represents a $500 million market in the United States. The use of electromagnetic stimulation in the treatment of fractures is common; however, the efficacy of this modality remains uncertain. We conducted a systematic review and meta-analysis of randomized controlled trials to evaluate the effect of electromagnetic stimulation on long-bone fracture-healing.
METHODS: We searched four electronic databases (MEDLINE, EMBASE, CINAHL, and all Evidence-Based Medicine Reviews) for trials of electromagnetic stimulation and bone repair, in any language, published from the

BACKGROUND: Delayed carpal tunnel syndrome after Colles' fracture is a common complication particularly following conservative treatment. The aim of the study was to verify the correlation of different radiological parameters of carpal alignment and the development as well as the severity of DCTS in elderly female patients within 6 months of distal radial fracture (DRF).
METHODS: This is a retrospective case-control study that included 60 female patients with DRF within 6 months treated conservatively (30 patients with signs and symptoms suggestive of DCTS and 30 asymptomatic patients as a co…

Thirty-three patients with pain and decreased range of motion after traumatic derangement of the distal radioulnar and ulna-carpal joint were treated with a modified Darrach distal ulnar resection. Their ages ranged from 22 to 75 years (average, 50 years). Twenty-seven patients had Colles' or other distal radius fractures. Seven patients had ulnar resection for treatment of distal ulnar-carpal derangement. Follow-up averaged 54.4 months. The patients showed an average increase in extension of 58% and flexion increased by 40% (p less than 0.01). Pronation increased by 40% and supination increas

Clinical and radiographic assessment of the results of 40 Swanson silicone ulnar head prosthesis implants in three different patient groups is presented. Twenty-eight were done for treatment of rheumatoid arthritis, eight for ulnoradial impingement after the Darrach procedure, and four for ulnocarpal impingement resulting from malunited Colles' fractures. Patients were reviewed 12 to 48 months after operation (mean, 29 months). Seventy-eight percent of patients reported relief of pain and improved function. Radiologic examination showed that all patients exhibited resorption of the distal ulna…

Distal radius fractures account for one in five bony injuries in both primary and secondary care. These are commonly the result of a fall on outstretched hands or high-energy trauma. On assessment, clinicians should determine the mechanism of injury, associated bony or soft tissue injuries, and neurovascular symptoms. Investigations should always include radiographs to evaluate for intra-articular involvement and fracture displacement. Owing to the heterogeneous injury patterns and patient profiles, the preferred management should consider the severity of the fracture, desired functional outco

BACKGROUND: Point of Care Ultrasound (PoCUS) is a safe, non-invasive tool for identifying distal radius fractures and can potentially be utilised to assist clinicians to reduce displaced fractures. We aim to test whether PoCUS is accurate to identify distal radius fractures and to determine how PoCUS performs as a tool to confirm a successful fracture reduction.
METHODS: A pragmatic prospective observational study was done in adult patients presenting with forearm injuries resulting in Colle's type distal radius fractures. Adults who presented to the emergency department (ED) with a suspected

The objective was to compare the prediction of bone mechanical properties provided by axial transmission to that provided by peripheral quantitative computed tomography (pQCT) at the distal radius. The distal radius is the location for Colles' fractures, a common osteoporosis related trauma situation. Measurements of the radial speed of sound were performed using three axial transmission devices: a commercial device (Sunlight Omnisense, 1.25 MHz), a bi-directional axial transmission prototype (1 MHz), both measuring the velocity of the first arriving signal (FAS), and a low frequency (200 kHz)…

Carpal fracture-dislocation associated to distal radius fractures is an uncommon injury of the wrist. Clinical assessment, instrumental diagnosis and treatment are all challenges for the surgeon. In addition, the prognosis in high-functioning patients is nearly always poor. The authors describe an early diagnosis of scapholunate dissociation and joint capsule tear associated with radial styloid and triquetral fractures in a 39-year-old professional piano player. A dorsal approach was used to reduce and fixate the fracture with k-wires, and to repair soft-tissue injuries with a Titanium micro-a

Twenty-five wrists with comminuted, displaced, intra-articular fractures of the distal radius were prospectively treated with an adjustable external fixator for an average of 51 days. Twenty fractures (80% [20 of 25] were available for follow-up) in 16 adults (mean age, 34 years) were treated with the Wrist Jack external fixator system (Hand Biomechanics Lab, Sacramento, CA) and evaluated at a mean follow-up period of 25 months. Ten patients (12 fractures) sustained high-energy trauma with multiple injuries, while 6 patients (8 fractures) sustained isolated distal radius fractures. Percutaneou

Osteoporosis international : a journal established as result of cooperation between the European Foundation for Osteoporosis and the National Osteoporosis Foundation of the USA1994eng
The criteria required for an effective screening strategy for osteoporosis are largely met in Caucasian women. The disease is common and readily diagnosed by the measurement of bone mineral with single- or dual-energy absorptiometry. Such measurements have high specificity but lower sensitivity, so that the value of the technique is greater for those identified as being at higher risk. Against this background there is little evidence that osteoporosis can usefully be tackled by a public health policy to influence risk factors such as smoking, exercise and nutrition. This suggests that it is ap

There is no clear consensus about the best management of intra-articular distal ulnar fractures associated with distal radial fractures in older adults. We describe a treatment wherein the distal radial fractures were securely fixed with a palmar plate, leaving the associated ulnar fractures unfixed. The wrists of 14 patients with a mean age of 74 years were reviewed at an average of 18 months after surgery. The results were excellent in 11 cases and good in three, according to the modified Gartland and Werley score. All fracture sites displayed union, and there was no instability of the dista
