Recidivism and concomitant criminality in pathological firesetters.

The purpose of this study was to explore dangerousness and fire setting recidivism of mentally disordered firesetters in comparison with mentally healthy arsonists. In Germany, all trials are registered centrally by the Federal Central Register. The records of this Register for the three year period from 1983 until 1985 were used in this follow-up study of arsonists to identify three samples of subjects: (1) All persons convicted of arson who have been found not guilty by reasons of insanity (n = 186), (2) all person convicted of arson who have been found guilty of diminished responsibility (n

The Relationship Between Serious Mental Illness and Criminal Offending in Persons Experiencing Homelessness: The Role of Substance Use Disorder.

Individuals who live with mental illness are encumbered by related risk factors that increase the probability of legal involvement. The goal was to determine how homelessness and substance use disorder are intervening factors in the relationship between symptoms of serious mental illness (SMI) and criminal offending. A sample of 210 chronically homeless adults receiving SAMHSA-funded outreach and psychiatric rehabilitation services between 2014 and 2016 was recruited in a study of interventions to address housing in homeless persons with a SMI. Participants were interviewed and data collected

Self-Strangulation Through A Sphygmomanometer: An Uncommon Suicide.

Suicide by asphyxia is quite a common event in forensic practice and may be implemented in different ways. The authors report a unique case of a 16-year-old youth who committed suicide by means of a standard mercury sphygmomanometer. This manner of suicide has never been described in the literature reviewed. A complete forensic investigation led to the conclusion that the cause of death was mechanical asphyxia, ascribed to self-strangulation by means of an atypical item. The victim suffered from attention-deficit/hyperactivity disorder (ADHD) syndrome and was assisted by support teachers. He h

Determinants of suicidal ideation: the role of substance use disorders.

BACKGROUND: This study tested the hypothesis that the amount of psychoactive substance consumed (frequency and/or quantity), life problems resulting from this use, and a DSM-IV diagnosis of substance abuse/dependence are independent risk factors associated with increased suicidal ideation in a population of psychiatric inpatients with major depressive disorder. METHOD: 891 hospitalized patients with a primary diagnosis of nonpsychotic major depressive disorder (MDD) received a standardized, psychiatrist-administered assessment battery. To examine the relationship between admission suicidality

Glioblastoma and new-onset criminal behaviors in a geriatric patient: a forensic-psychiatric case report from Switzerland.

Organic brain disorders (OBD), including rapid-growth cancerous tumors, can have significant neuropsychiatric effects and in some circumstances have led to the manifestation of deviant behaviors that conflict with societal norms. This report describes the case of a geriatric male patient in Switzerland with no prior history of delinquency who in later life repeatedly committed stalking offences and aggressive acts. An initial forensic-psychiatric evaluation diagnosed this individual with persistent delusional disorder based on pronounced symptoms and rigid personality traits; during this asses

Clinical characteristics as a function of referral status among substance users in residential treatment.

In the United States, substance users who voluntarily (VO) elect to receive treatment and substance users who are court-mandated (CM) to receive treatment typically obtain care within the same facilities. Little is known about the clinical characteristics that differentiate these individuals. The current study provides rates of specific DSM-IV Axis I and Axis II psychiatric and substance use disorders, comorbidities, childhood trauma, motivation, and other clinical and demographic characteristics as a function of referral status, among individuals in residential substance use treatment (463 pa

Persistent interferon-ß-1b-induced psychosis in a patient with multiple sclerosis.

Interferon-ß is used in patients with multiple sclerosis to reduce autoimmunity; although other psychiatric side-effects are common, in contrast to interferon-alpha, psychosis has been reported only once. A patient with multiple sclerosis developed auditory hallucinations, paranoid delusions, and increased aggressiveness after 16 months of treatment with interferon-ß-1b, 250 mg every other day. He responded after about one month to antipsychotic treatment, but tended to relapse upon dose reduction, and after 2 years still needs antipsychotics to control his symptoms. Because there was no chang

Criminality of the mentally ill in sheltered care: are they more dangerous?

The sample of mental patients in sheltered care has a lower arrest rate than the general population in California in all categories of crimes, except for violent crimes. For violent crimes, the sheltered-care population is likely to be arrested at 1.33 times the rate of the state population, even when the heterogeneity of aggravated assault was taken into consideration. This indicates empirically that the mentally ill in sheltered care are more dangerous than the general population. For prediction of criminality, four factors are found to be significant predictors of resident criminality after

[Hebephrenic symptoms as an expression of an XYY chromosome abnormality? Case report of a patient with suspected sexual abuse of his own 2 minor children].

A 48 year old patient was hospitalised because of parasuicidal behaviour and suicidal ideation. He was under suspicion of having sexually abused his 4-year old daughter and his 4-year old son. At the age of 17, he was hospitalised in a psychiatric ward under the diagnosis of hebephrenic schizophrenia. He successfully received an insulin coma therapy. Because of his increased height (1.89 m), mental retardation and other psychical disorders in his youth, we now suspected him of having an extra Y chromosome which was confirmed by chromosome analysis. The non-uniform symptomatology of XYY-individ

Long-term health, criminal justice, and socioeconomic outcomes among people subject to compulsory mental health care in Norway: a nationwide registry cohort study.

BACKGROUND: Compulsory mental health care aims to manage risk and treatment needs among people with severe mental illness, but evidence on long-term outcomes across health, justice, and socioeconomic domains is scarce. We aimed to compare long-term outcomes across adult groups defined by community treatment orders (CTOs), compulsory hospitalisation, compulsory observation, and voluntary mental health care, and the general population. METHODS: In this nationwide registry cohort study, we identified all individuals aged 18-65 years with specialist mental health-care contact in Norway from Jan 1

Supreme court of Canada's "Beautiful Mind" case.

The Supreme Court of Canada's (SCC) first case involving capacity and the refusal of involuntary psychiatric treatment involved a self described "professor" who had been referred to as "Canada's Beautiful Mind". He had been found not criminally responsible on account of mental disorder for uttering death threats. While considered incapable of making a treatment decision by psychiatrists and a review board, three levels of court, including the SCC, found him to be capable. "Professor" Starson therefore continued to refuse treatment for his psychosis and spent over seven years detained because h

Psychiatric and Forensic Differences Between Men Charged with Sex Offences and Men Charged with Other Offences.

Studies on differences between individuals convicted of sexual offences and nonsexual offences are sparse and there is an on-going debate as to whether sexual offenders differ from other offenders. The primary aim of this study was to determine demographic characteristics, prevalence of mental disorders, alcohol and drug use at the time of the crime and the criminal responsibility of individuals charged with sexual offences, compared to nonsexual crimes, with the aim of bringing awareness to the similarities and differences between men charged with sex offences and those charged with other cri

[Dissociation (conversion) - malingering - antisocial personality disorder: differential diagnostic reflection on the basis of a case study].

In this case report we refer to the big challenge of making a diagnosis in a deliberate malingering in the field of mental disorders. We specifically describe the difficulty regarding the differentiation between a conversion disorder and malingering of a serial delinquent. For such a person avoiding criminal persecution is one of the most frequent reason to deceitfully simulate a mental illness. In this field, symptoms of conversion disorders exceed the average; furthermore, a great number of organic-neurological illnesses may appear to be very similar to a conversion disorder or in many cases

CRIMSON [CRisis plan IMpact: Subjective and Objective coercion and eNgagement] protocol: a randomised controlled trial of joint crisis plans to reduce compulsory treatment of people with psychosis.

BACKGROUND: The use of compulsory treatment under the Mental Health Act (MHA) has continued to rise in the UK and in other countries. The Joint Crisis Plan (JCP) is a statement of service users' wishes for treatment in the event of a future mental health crisis. It is developed with the clinical team and an independent facilitator. A recent pilot RCT showed a reduction in the use of the MHA amongst service users with a JCP. The JCP is the only intervention that has been shown to reduce compulsory treatment in this way. The CRIMSON trial aims to determine if JCPs, compared with treatment as usu

Factors associated with disallowance of compulsory mental healthcare referrals.

OBJECTIVE: Norwegian studies report that a substantial amount of referrals for compulsory mental health care are disallowed at specialist assessment, at a rate that varies with referring agent. Knowledge on factors associated with disallowance could improve the practice of compulsory mental health care. This study aims to examine such factors, placing particular emphasis on the impact of referring agents. METHOD: This study utilized data from the prospective, longitudinal cohort study 'Suicidality in Psychiatric Emergency Admissions' conducted at a Norwegian psychiatric emergency unit which s

Patient perspectives and the risk of compulsory admission: the Amsterdam Study of Acute Psychiatry V.

OBJECTIVE: Compulsory admission to a psychiatric hospital is associated with a three- to fourfold increase in the risk of another compulsory admission. Given the rising numbers of civil detentions in The Netherlands and other European countries, it is important to understand the mechanism behind this association. Our aim is to study the links between opinions about prior psychiatric treatment, insight, service engagement and the risk of (new) civil detentions. METHODS: We took a random sample of 252 from the 2,682 patients consecutively coming into contact with two psychiatric emergency teams

Fetal syndromes and the charter: the Winnipeg glue-sniffing case.

The relationship between scientific knowledge and legal discourse is raised once again by a recent decision of the Supreme Court of Canada, a case involving a young Aboriginal woman who was pregnant and ordered by the court to remain in a drug treatment program at a health center until the baby was born. Her glue-sniffing habit was deemed dangerous to the normal development of the fetus. The Court held that her solvent-dependency did not justify the original court action, but both the Court and the various interveners disregarded the current state of our knowledge on the fetal syndromes. There

[AKuT outpatient clinic: cross-sectoral crisis intervention for borderline personality disorder and trauma-related disorders at the Central Institute of Mental Health in Mannheim].

BACKGROUND: Patients with borderline personality disorder or trauma-related disorders often experience psychosocial crises. Due to a lack of promptly available outpatient treatment options, acute care is often provided in a psychiatric emergency department or an inpatient setting. This impedes treatment continuity and fosters hospitalization. OBJECTIVE: Development and implementation of a hospital-wide treatment concept for crisis intervention at the Central Institute of Mental Health (CIMH) in Mannheim that overcomes sectoral boundaries. MATERIAL AND METHODS: Presentation of the new patient
