Eighty-six leprosy patients (49 active lepromatous, 24 inactive lepromatous 7 borderline, and 6 tuberculoid) and nine healthy controls were examined for numerical changes in T cell subsets (T gamma and T mu), and complement levels in peripheral blood to determine the roles of T cell subsets and complement in the etiology of leprosy. Immunoglobulins (IgG, IgA, IgM and IgD) and complement components of both the classical and alternative pathways (C1q, C3 and C4, and factor B) were measured in the sera of 15 Nigerian patients with proven amoebic liver abscess and compared with 30 age-matched controls. IgA, C3 and factor B were significantly higher in amoebic liver abscess patients. The differences in IgG, IgM, IgD, C1q and C4 levels between patients and controls were not significant.

We performed hemolytic assays for C1, C4, C3 and C5 on tear samples from 7 normal subjects and 10 patients with corneal ulcers. Hemolytic activities in tears were determined by 50% hemolysis of sensitized sheep red blood cells. Absent or low hemolytic activities of C1, C4 and C3 were found in normal tears. C5 was detected in tear samples from 4 of 7 normal subjects. Tear samples from patients with corneal ulcers showed elevated levels of C1, C4, C3 and C5, but wide ranges in values were found. C5 had the highest hemolytic activities in both normal tears and tears from eyes with corneal ulcers.

We report on a girl with selective IgA deficiency and persistently low complement component 4 (C4) levels compatible with heterozygous C4 deficiency. Deterioration of her serum immunoglobulin levels and transition to common variable immunodeficiency were observed within a 5 year follow-up. She also developed Sjögren's syndrome, autoimmune hemolytic anemia and immune thrombocytopenic purpura. While these abnormalities have been described before in various combinations, to our knowledge, they have not been reported in a single individual.
