BACKGROUND: Multiple penile condylomata acuminata (CA) often involve the inner foreskin and frenulum. This retrospective study compared conventional sleeve and disposable circular stapler circumcision, each combined with ablation of visible lesions, in selected patients with multiple penile CA.

We present a case of a 28 year old man presenting with obstructive urinary tract symptoms and cystoscopic findings of pan-urethral, occlusive condylomata acuminata (urethral genital warts). Despite different treatment modalities, resistance to treatment is observed in 4-15% of patients with anogenital condylomata acuminata. A total of 16 patients with resistant condylomata acuminata or extended infection affecting an area of more than 6 cm2 were treated with a prospectively defined therapy entailing laser treatment and adjuvant systemic interferon-alpha 2B (3 x 1 MU/day for 7 days). A group of 74 men who underwent carbon dioxide laser treatment of meatal condylomata were observed for an average of 18 months. The cure rate after 1 treatment of isolated meatal lesions was 78%; the presence of external lesions lowered the rate to 32% and additional external and urethral warts to 25%. Following multiple treatments all but 6 patients were cured; 83% of the recurrences developed within 3 months. One urethral and 6 meatal strictures occurred more than 3 months after treatment; 9 patients had a spraying stream many years after treatment and 2 complained of frequency.

Fifty-one patients, 16 in the second trimester and 35 in the third, were treated by cryotherapy for genital condylomata acuminata; 19 of them had cervical cryotherapy in the second and third trimesters for cervical involvement. To determine the correlation between deoxyribonucleic acid (DNA) loads of human papillomavirus (HPV) and recurrence of condylomata acuminata (CA), 31 cases of primary CA and 32 cases of recurrent CA were assayed for the HPV6/11 and HPV16/18 DNA loads by real-time fluorogenic quantitative PCR. Laser therapy is widely accepted as a safe and effective means of treating genital condylomata acuminata. In this report, single-session neodymium: yttrium-aluminum-garnet laser treatment of extensive condylomata of the penis resulted in deep coagulation necrosis and sloughing of penile shaft skin and urethra. We report on a 33-year-old patient with unusually extensive and predominantly cutaneous condylomata acuminata spreading from the penis over the mons pubis to the entire lower abdominal wall but almost completely sparing the genital mucous membranes. Single lesions were found on the left arm and on the buttocks. Southern blot hybridization detected HPV-6a DNA sequences within the fibroepithelial tumours. Carbon dioxide laser treatment resulted is cosmetically acceptable healing without recurrence.

Three patients with condylomata acuminata of the urinary bladder are reported. Human papillomavirus is a sexually transmitted virus that has been associated with intraepithelial neoplasia. The incidence and prevalence of the disease has risen dramatically, to epidemic proportions, within the last two decades.
