Confusion or delirium in patients with posterior cerebral arterial infarction.

OBJECTIVE: To identify the possible anatomic sites and risk factors for the development of confusion or delirium in patients with posterior cerebral arterial (PCA) infarction. MATERIALS AND METHODS: Twenty-nine patients aged 34-86 years with PCA infarction were divided into two groups: one with and the other without perturbed mentation. The clinical and laboratory data, including neuroimages, were retrospectively reviewed. Student-t, chi-square and Fisher's exact tests were performed for data analysis. RESULTS: Confusion or delirium tended to develop in the left (10/13) or bilateral (5/5) PC…

June 2001: 61 year old woman with confusion and obtundation.

The June COM. A 61 year old female presents with a three week history of increasing confusion, lethargy and headache. A neurological exam revealed disorientation, mild expressive aphasia, bilateral papilledema, and a right pronator drift. She had a craniotomy and resection of tumor. The tumor histologically was consistent with a solitary fibrous tumor displaying malignant features of hypercellularity, marked nuclear atypia, high mitotic activity, and a high proliferation index. This case is unique as the first malignant variant of solitary fibrous tumor to be reported intracranially.

[Non-convulsive status epilepticus--confusion and cognitive failure during seizures].

BACKGROUND: Non-convulsive status epilepticus is characterized by confusion and impaired consciousness, lasting at least half an hour. Seizure activity in the EEG confirms the diagnosis, but ictal discharges can be heterogeneous and difficult to classify. There is controversy regarding evidence of morbidity in humans. MATERIAL AND METHODS: We report three patients with non-convulsive status epilepticus. RESULTS: In two patients, the clinical findings during seizures were subtle cognitive impairment. One patient was confused during seizures. Localisation of seizure activity in the EEG was mai…

[Confusion syndrome in alcoholism. Faulty assessment can produce deleterious course].

Confusion is a frequent syndrome in alcoholism caused by a large number of neuropsychiatric and somatic disorders with severe alcohol intoxication, pathological intoxication, alcohol dementia and hepatic encephalopathy among them. Possible pathophysiologic mechanisms involved and therapeutic strategies are discussed.

Fear of senility: the nurse's role in managing reversible confusion.

1. Acute illnesses in the elderly often cause reversible confusion. 2. Agitated and unsafe behavior is an attempt by the confused elderly to maintain self-image and physical and psychological integrity. 3. Ebersole and Hess' humanistic framework can guide the nurse to prioritize and meet the needs of the confused elderly.

Primary intranasal Fusarium infection. Potential for confusion with rhinocerebral zygomycosis.

Fusarium species are saprophytic fungi that may colonize human skin and nails and may rarely cause invasive infections in traumatized tissue and in debilitated and immunocompromised patients. We report herein a case of invasive intranasal Fusarium oxysporum infection in a diabetic patient. This unusual presentation potentially can be confused with early rhinocerebral zygomycosis clinically and histologically. Distinguishing morphologic features and the possible role of diabetes in promoting this infection are discussed.

Severe confusion in a patient receiving electroconvulsive therapy and atenolol.

Atenolol is a beta-blocker antihypertensive agent that reportedly has little central nervous system action. A patient was given atenolol during a course of electroconvulsive therapy and developed an organic mental syndrome with paranoid delusions. The possible alteration of the permeability of the blood-brain barrier by electroconvulsive therapy may have enabled atenolol to cause this unusual side effect.

The "border zone" in evolving myocardial infarction: controversy or confusion?

Considerable controversy has arisen over the existence of a "border" zone of intermediate injuries during regional myocardial ischemia. Much of this controversy arises from the inappropriate use of terminology and the misquotation or misinterpretation of previously published studies. This article considers the nature of the interface between normal and ischemic tissues; proposes a series of definitions based on current knowledge of etiology of tissue injuries; reviews critically the existing evidence for an against the "border zone" concept; and considers the quantitative significance of a "bo

M3C: Resist Agnostic Attacks by Mitigating Consistent Class Confusion Prior.

Adversarial attack is a major obstacle to the deployment of deep neural networks (DNNs) for security-sensitive applications. To address thes … al defense. This paper sheds light on a generalizable prior under various attacks: consistent class confusion (3C), i.e., an AT classifier often confuses the predictions between correct and ambiguous classes in a highly similar pattern among diverse attacks. Relying on this latent prior as a bridge between seen and agnostic attacks, we propose a more genera …

Stereochemical Terminology in Chiral Drugs: Still Between Confusion and Misinterpretation.

The Viewpoint addresses the importance of correct designations of stereochemistry in chiral drugs. Sources of confusion typically arise from merging disparate concepts such as handedness, configuration, and chiroptical properties. This Viewpoint recommends the consistent use of configurational labeling, while avoiding ambiguous and context-dependent nomenclature.

Will Human-Animal Chimeras Cause Moral Confusion? Exploring Public Attitudes.

Recent medical research involving human-monkey chimeras, human brain organoids in rats, and the transplantation of a gene-edited pig heart and gene-edited pig kidneys in living human beings have intensified the debate about whether we should create human-animal chimeras for biomedical purposes and, if so, how we should treat them. Influential views in the debate frequently appeal to assumptions regarding how people will react to such chimeras. It has, for example, been argued that the most important objection against creating such chimeras is that this will result in inexorable moral confusion…

Pathogenic Variants in ATP1A3: Why Is There So Much Confusion?

Dominant pathogenic variants in ATP1A3 can occur anywhere in the coding sequence and can cause a wide range of clinical presentations. A practical problem is that sequencing services use one of 3 different mRNA transcripts with different lengths to number the candidate variants. Genetic reports often identify an ATP1A3 variant as a VUS when it is actually well validated. Only 1 transcript, the MANE Select transcript that encodes a protein of 1,013 amino acids, is well supported by evidence. Misidentification of variants in ATP1A3 is disadvantageous when it prevents a confident diagnosis. We il
