Behavioral contributions to acquisition of gonorrhea in patients attending an inner city sexually transmitted disease clinic.

To begin to define the behaviors that influence gonorrhea acquisition, a case-series of patients attending a sexually transmitted disease clinic was surveyed. Although gonorrhea rates were similar, men and women differed in sexual behaviors predictive of infection. Men with a new or casual partner were more likely to have gonorrhea than men with no such partners (adjusted odds ratio = 2.7); this finding did not hold true for women. Condom use in the previous month reduced the chances of gonorrhea acquisition for both men and women. More than 33% continued to engage in sexual activity after ons

Sexual histories of heterosexual couples with one HIV-infected partner.

Ninety-eight heterosexual couples enrolled in a HIV transmission study, at least one of whom was HIV-infected, were interviewed about sexual behavior. Although males and females were interviewed separately, there was agreement between them on the number of sexual contacts, the practice or anal intercourse, and condom use. These findings of strong reliability are encouraging, but do not necessarily imply that the data are valid.

Use of contraceptive methods and contraceptive recommendations among health care providers actively involved in contraceptive counseling -- results of an international survey in 10 countries.

BACKGROUND: This study was conducted to determine the personal choices of contraceptive methods among an international sample of contraception health care professionals (HCPs) and to determine if these choices are concordant with their recommendations to women. STUDY DESIGN: In an anonymous online survey, 1001 HCPs actively involved in contraceptive counseling [obstetrician/gynecologists (OB/GYNs), general practitioners (GPs) and midwives (only in Sweden)] from 10 countries (Australia, Brazil, Canada, France, Germany, Korea, Mexico, Spain, Sweden and the United Kingdom) were asked about their

Contraceptive use and consistency in U.S. teenagers' most recent sexual relationships.

CONTEXT: Most U.S. teenage pregnancies are unintended, partly because of inconsistent or no use of contraceptives. Understanding the factors associated with contraceptive use in teenagers' most recent relationships can help identify strategies to prevent unintended pregnancy. METHODS: Data on 1,468 participants in Waves 1 and 2 of the National Longitudinal Study of Adolescent Health who had had two or more sexual relationships were analyzed to assess factors associated with contraceptive use patterns in teenagers' most recent sexual relationship. Odds ratios were generated through logistic re

Does structured counselling influence combined hormonal contraceptive choice?

OBJECTIVE: To assess the effect of structured counselling on women's contraceptive decisions and to evaluate gynaecologists' perceptions of comprehensive contraceptive counselling. METHODS: Belgian women (18-40 years old) who were considering using a combined hormonal contraceptive (CHC) were counselled by their gynaecologists about available CHCs (combined oral contraceptive [COC], transdermal patch, vaginal ring), using a comprehensive leaflet. Patients and gynaecologists completed questionnaires that gathered information on the woman's pre- and post-counselling contraceptive choice, her pe

Effect of price and access on contraceptive use.

The Family Planning Program in Bangladesh has been very successful. The contraceptive prevalence rate (CPR) has increased from 13% in 1979 to 49% in 1996. Now that the program has matured and demand for family planning has been created, the Ministry of Health and Family Welfare (MOHFW) of the Government of Bangladesh is concerned with increasing its financial sustainability. Options to increase financial sustainability include cost sharing and a gradual transition from doorstep to static clinic delivery of contraceptives. Many of these alternatives would involve additional travel time or charg

Development and validation of a condom self-efficacy scale for college students.

This study proposed to develop and validate a scale for the college population that measures self-efficacy in using condoms. The Condom Use Self-Efficacy Scale (CUSES) was derived from several sources and consisted of 28 items describing an individual's feelings of confidence about being able to purchase condoms, put them on and take them off, and negotiate their use with a new sexual partner. This scale was administered to a sample of 768 college students. It was found to possess adequate reliability (Cronbach's alpha = .91; test-retest correlation = .81) and correlated well with the Attitude

Evidence-based selection of candidates for the levonorgestrel intrauterine device (IUD).

BACKGROUND: Recent evidence-based guidelines expanded the definition of appropriate candidates for the levonorgestrel-releasing intrauterine system (LNG-IUS). We investigated correlates of evidence-based selection of candidates for the LNG-IUS by physicians who offer insertion. METHODS: We conducted a mixed-mode (online and mail) survey of practicing family physicians and obstetrician-gynecologists in Seattle. RESULTS: A total of 269 physicians responded to the survey (44% response rate). Of the 217 respondents who inserted intrauterine devices, half or fewer routinely recommended the LNG-IU

Surgical sterilization among women and use of condoms--Baltimore, 1989-1990.

Since 1980, surgical sterilization among women has become the most common contraceptive method used among women aged greater than 30 years in the United States and is used by 28% of women aged 15-44 years. A previous report of women in drug treatment suggested that women who have been surgically sterilized were less likely to report condom use--an effective measure for prevention of human immunodeficiency virus (HIV) infection and sexually transmitted diseases (STDs)--than were nonsterilized women. This report summarizes a study of the relation between surgical sterilization, risk status for S

Intrauterine contraceptive device use in patients with acute salpingitis.

The frequency of intrauterine contraceptive device (IUD) use among 144 hospitalized women with acute salpingitis was compared with that of 229 control women, who were symptomless sexual partners of men examined for non-gonococcal urethritis. The relative risk for salpingitis was higher for IUD than in the group of basal level of disease (p < 0.02). This was mostly caused by the high relative risk in the women with no deliveries. Oral contraceptives showed a contrary picture, the relative risk was significantly lower than on the basal level (p < 0.01). Non-gonococcal and gonococcal infections w

A mixed-methods study of factors influencing postpartum intrauterine device uptake after family planning counseling among women in Kigali, Rwanda.

INTRODUCTION: Rwanda has high unmet need for family planning (FP), especially in the postpartum period when women are advised to space pregnancies at least two years for improved maternal-child health. Despite interest in the copper intrauterine device (IUD), a highly cost-effective method, access and uptake remain low. This study aimed to determine factors associated with postpartum IUD (PPIUD) uptake after postpartum family planning (PPFP) counseling as well as provider perceptions of facilitators and barriers to clients' PPIUD uptake. METHODS: Postpartum women who received PPFP counseling

HIV Prevention: Opportunities and Challenges.

Preexposure prophylaxis (PrEP) with tenofovir disoproxil fumarate (TDF)-based regimens has been shown to be effective in preventing acquisition of HIV infection, with protective efficacy being dependent on adherence to treatment. Data from the PROUD (Preexposure Option for Reducing HIV in the UK) and IPERGAY (Action to Prevent Risk Exposure By and For Gay Men) studies, the later of which employed event-driven PrEP, showed a high rate of protective efficacy of PrEP with TDF and emtricitabine among men who have sex with men. Data from the ASPIRE (A Study to Prevent Infection With a Ring for Exte

Clinical acceptability of an ethylene-vinyl-acetate nonmedicated vaginal ring.

In an open study the acceptability of a newly designed vaginal ring was evaluated. The ring being developed for contraceptive purposes was made of ethylene-vinyl-acetate copolymer (EVA). In this study the ring did not contain active medication. Three groups of 20 healthy female volunteers were asked to use rings--with a diameter of 54 mm--for 21 consecutive days; each group was assigned rings with a different cross-sectional thickness of 3, 3.5, or 4 mm. Subjects were given diary cards to report on insertion and removal of the ring and on complaints. Of the 65 women who participated, 59 comple

Free condoms in the schools of Copenhagen, Denmark.

Attitudes of ninth grade students and their classroom teachers toward using condoms as a preventive measure against contracting AIDS were studied. Students (N = 438) and their teachers (N = 28) in the council schools of the Municipality of Copenhagen were surveyed using a structured questionnaire. Overall, results demonstrated that the message regarding "safe sex" had been well understood. However, 42% of pupils reported not using a condom during their last sexual intercourse. Many pupils indicated difficulty in using condoms. A continuing need exists for advice to the young. An information ca

Dyspareunia.

Dyspareunia--painful intercourse--is a common cause of sexual difficulty. The following paper discusses, for both males and females, the common causes of dyspareunia and in many instances the appropriate treatment. Painful intercourse for the male is related either to the prepuce, glans penis, penile shaft, testicles, or urethra and prostate gland, whereas painful intercourse for the female is related to the vaginal opening, clitoris, vagina, or uterus, tubes, and/or ovaries. Each of the above is discussed.

New condoms for men and women, diaphragms, cervical caps, and spermicides: overcoming barriers to barriers and spermicides.

Barriers and spermicides are being developed for the prevention of unwanted pregnancy and sexually transmitted infection. The inconvenience of current methods limits their acceptability and leads to higher user failure rates. New methods, such as female condoms, nonlatex male condoms, and new cervical caps are being developed: the Reality (Wisconsin Pharmacal, Jackson, WI) polyurethane vaginal pouch has already received conditional Food and Drug Administration approval for use in the United States. The material used in a Food and Drug Administration-approved nonallergenic glove is being tested
