PURPOSE: To clarify the clinical features and imaging characteristics of non-AIDS patients with pulmonary cryptococcosis.
METHODS: We retrospectively collected 15 HIV-negative patients with pathology-proved pulmonary cryptococcosis from Sep 1992 to Jan 2008. Their medical records and radiological data were reviewed and analyzed.

OBJECTIVE: To investigate the diagnosis and treatment of pulmonary cryptococcosis.
METHODS: A total of 9 cases of pulmonary cryptococcosis, diagnosed at Sir Run Run Shaw Hospital of Zhejiang University from January 2002 to August 2004, identified by pathological examinations, were retrospectively studied.

A 19-year old man was admitted to our hospital with fever and productive cough. Chest X-ray films revealed multiple nodular shadows accompan … occal organisms with the infiltration of monocytes. These findings yielded a diagnosis of pulmonary cryptococcosis with endobronchial lesion. After 6 months of treatment with fluconazole, the multiple nodular shadows disappeared. Comparison of bronchoscopic findings before and after fluconazole treatment clearly demonstrated the efficacy of the therapy.

Ten cases of primary pulmonary cryptococcosis are presented. The patients' ages ranged from 19 to 71 years and all were males. All patients underwent chest X-ray, tomography and CT (two cases were in additional to HRCT). We describe two siblings with X-linked hyper-IgM immunodeficiency. One patient developed disseminated cryptococcosis. Co-culture of this patient's T cells with normal B cells suppressed IgG and IgA production. The CD40 ligand gene of one patient was examined and contained a nonsense mutation at nucleotide 475. Cutaneous cryptococcosis usually is associated with concurrent systemic infection and actually may develop before clinical manifestations of cryptococcal meningitis become apparent. Fourteen cases of systemic cryptococcosis were admitted to Siriraj Hospital during 1987 to 1989. These included 5 male and 9 female patients aged 4-65 years. A 33-year-old man was treated with systemic steroids for a retinal inflammatory lesion before the diagnosis of cryptococcal retinitis and meningitis was suspected. He died from central nervous system disease despite treatment with parenteral antifungals. Histopathological studies demonstrated ocular and disseminated systemic infection with Cryptococcus neoformans. Direct cryptococcal involvement of the eye is rare and is usually associated with disseminated disease. Systemic steroids must be used with caution, and patients who take these drugs require frequent monitoring.
