Mortality prediction and long-term outcomes for civilian cerebral gunshot wounds: A decision-tree algorithm based on a single trauma center.

Gunshot wounds (GSW) are one of the most lethal forms of head trauma. The lack of clear guidelines for civilian GSW complicates surgical management. We aimed to develop a decision-tree algorithm for mortality prediction and report long-term outcomes on survivors based on 15-year data from our level 1 trauma center. We retrospectively reviewed 96 consecutive patients who presented with cerebral GSWs between 2003 and 2018. Clinical information from our trauma database, EMR, and relevant imaging scans was reviewed. A decision-tree model was constructed based on variables showing significant diffe

[Salvage from Extracorporeal Membrane Oxygenation due to Acute Deteriorated Heart Failure;Treatment Decision-tree Based on the Risk Factors].

BACKGROUND: Salvage rates for patients requiring extracorporeal membrane oxygenation (ECMO)due to acute cardiogenic shock remain poor due to difficulties in decision making on optical timing of ECMO removal or conversion to ventricular assist devices( VAD). METHOD: From 2005 to 2018, 37 patients supported with ECMO due to acute circulatory deterioration were referred to our department for implantation of VAD. Their outcomes were analyzed using multi-variate analysis to assess the risk factors of VAD implantation, and we adopted a new decision-tree to improve the outcomes. RESULTS: Four patie

Who benefits, who is left behind? Intersectional inequities in unmet health care needs before and after the 2010 Swedish choice in primary health care reform using decision trees.

In 2010, Sweden implemented the Choice in Primary Health Care reform, allowing private healthcare providers to establish themselves at self-selected locations. While intended to improve efficiency and responsiveness through increased competition, concerns were also raised about its potential to reinforce inequities in healthcare access across different population groups. This study examines intersectional inequities in unmet healthcare needs (UHCN) in relation to the reform's implementation. Using data from the Health on Equal Terms survey (2007-2014, N = 69,644), we applied a decision tree-ba

From complex algorithms to clinical practice: a multicenter machine learning model and simplified decision tree for predicting cachexia risk in gastric cancer.

BACKGROUND: Cachexia is a frequent, specific metabolic syndrome that severely compromises survival in gastric cancer (GC). While early diagnosis is paramount, existing screening methods are limited by complexity and suboptimal accuracy. There is an urgent need for an efficient, data-driven tool derived from routine clinical parameters. METHODS: In this multicenter retrospective study, we analyzed data from three independent hospitals. Variable selection was performed using univariable and multivariable analyses. We constructed and compared multiple machine learning (ML) models to predict cach

Inflammatory activity levels on patients with anti-TNF therapy: most important factors and a decision tree model based on REGISPONSER and RESPONDIA registries.

BACKGROUND: The effectiveness of anti-tumour necrosis factor (TNF) therapy in spondyloarthritis is traditionally associated with factors such as age, obesity and disease subtypes. However, less-explored aspects, such as mental health, socioeconomic status and work type may also play a crucial role in determining inflammatory activity and therapeutic response. OBJECTIVES: To identify the most significant factors explaining inflammatory activity levels in patients treated with anti-TNF therapy and to develop an interpretable machine-learning model with good performance and minimal overfitting.

Development and Validation of Machine Learning-Based Prediction for Dependence in the Activities of Daily Living after Stroke Inpatient Rehabilitation: A Decision-Tree Analysis.

BACKGROUND AND PURPOSE: Accurate prediction using simple and changeable variables is clinically meaningful because some known-predictors, such as stroke severity and patients age cannot be modified with rehabilitative treatment. There are limited clinical prediction rules (CPRs) that have been established using only changeable variables to predict the activities of daily living (ADL) dependence of stroke patients. This study aimed to develop and assess the CPRs using machine learning-based methods to identify ADL dependence in stroke patients. METHODS: In total, 1125 stroke patients were inve

Towards closed-loop deep brain stimulation: decision tree-based essential tremor patient's state classifier and tremor reappearance predictor.

Deep Brain Stimulation (DBS) is a surgical procedure to treat some progressive neurological movement disorders, such as Essential Tremor (ET), in an advanced stage. Current FDA-approved DBS systems operate open-loop, i.e., their parameters are unchanged over time. This work develops a Decision Tree (DT) based algorithm that, by using non-invasively measured surface EMG and accelerometer signals as inputs during DBS-OFF periods, classifies the ET patient's state and then predicts when tremor is about to reappear, at which point DBS is turned ON again for a fixed amount of time. The proposed alg
