Diabetes overtreatment and flags for deprescribing glucose-lowering treatment in geriatric inpatients with type 2 diabetes: Prognostic insights from a longitudinal cohort.

AIMS: The study investigated diabetes overtreatment (excessively intensive therapy) and flags for considering deprescribing glucose-lowering treatment (GLT) (clinical features identifying patients most likely to benefit from reducing treatment) in older adults with type 2 diabetes (T2D) and their association with five-year survival in geriatric inpatients. METHODS: Single-centre retrospective cohort study including geriatric inpatients hospitalised between 2008 and 2015, aged ≥75 years with T2D on GLT. Diabetes overtreatment, according to the 2019-Endocrine Society guidelines, was retrospecti

Frailty-Informed Deprescribing of Glucose-Lowering Therapy in Older Adults with Type 2 Diabetes: A Pragmatic Clinical Framework.

BACKGROUND: Older adults with type 2 diabetes are often subjected to intensive glucose-lowering therapies with a high likelihood of hypoglycaemia, falls, functional decline, and treatment burden. While international recommendations advocate for individualised glycaemic targets, they provide little clarification on when, or how, to de-intensify therapies as health status changes. METHODS: We undertook a targeted evidence synthesis of randomised controlled trials, observational cohort studies, target trial emulation studies, and international clinical guidelines focusing on glucose-lowering age

Multidisciplinary Team Delivered Deprescribing Intervention for Patients Receiving Maintenance Hemodialysis: A Pilot Randomized Controlled Trial.

PURPOSE: Inappropriate polypharmacy is highly prevalent among patients with chronic kidney disease (CKD) and could be reduced through targeted deprescribing interventions. We aimed to evaluate the feasibility and preliminary effectiveness of a multidisciplinary team-delivered deprescribing intervention for CKD patients undergoing hemodialysis (HD). METHODS: A parallel-group pilot randomized controlled trial (RCT) was conducted among adult HD patients. Participants were randomized into either intervention or control groups. The intervention group received a structured deprescribing program usi

Attitudes of Australian patients receiving inpatient mental health care towards deprescribing: a cross-sectional survey.

BACKGROUND: Psychotropic polypharmacy is common, increasing, and associated with higher risks of adverse effects, hospitalisations and mortality. This study aimed to explore the attitudes and beliefs of people receiving inpatient mental health care toward deprescribing (discontinuing a medication when the current or potential risk outweighs the current or potential benefit) and determine any patient characteristics associated with these attitudes and beliefs. METHODS: A cross-sectional survey of patients admitted to two open acute psychiatric inpatient units was conducted over a 6-month perio

Current Practices on Prescribing and Deprescribing for Patients on Long-Term Antibiotic Treatment for Chronic Pulmonary Conditions: An Umbrella Review by the European Society of Clinical Pharmacy (ESCP).

PURPOSE: Chronic pulmonary conditions require complex treatment strategies involving long-term antibiotic treatment, which carries the highest risk of antimicrobial resistance and adverse drug events (ADE). Specific guidance on prescribing and deprescribing can help reduce these risks and improve therapy effectiveness. The aim of the study was to determine prescribing and deprescribing practices for long-term antibiotic treatment (≥30 days) in preventing exacerbations of stable chronic pulmonary conditions in adult patients across all healthcare settings. PATIENTS AND METHODS: This umbrella r

Anticholinergic medicines use among older adults before and after initiating dementia medicines.

AIMS: We investigated anticholinergic medicines use among older adults initiating dementia medicines. METHODS: We used Pharmaceutical Benefits Scheme dispensing claims to identify patients who initiated donepezil, rivastigmine, galantamine or memantine between 1 January 2013 and 30 June 2017 (after a period of ≥180 days with no dispensing of these medicines) and remained on therapy for ≥180 days (n = 4393), and dispensed anticholinergic medicines in the 180 days before and after initiating dementia medicines. We further examined anticholinergic medicines prescribed by a prescriber other than

Statistical analysis plan of the study titled "A deprescribing programme aimed to optimize blood glucose-lowering medication in older people with type 2 diabetes mellitus - the OMED2 study: a randomized controlled trial".

BACKGROUND: The OMED2 (Optimization of Medication in Elderly with Diabetes) study addresses the effect and implementation of integrating a deprescribing programme (DPP) in general practice. The aim of the DPP is to reduce glucose-lowering medication (SU/insulin) in overtreated older patients. The protocol for this study has been published previously. This statistical analysis plan (SAP) contains a more elaborate outline of the (statistical) methods we plan to use for data analysis. METHODS: The OMED2 study is a randomized mixed-methods study with a 2-year follow-up period that compares the ef

Feasibility of Measuring Physical Function by Wearable Devices During Deprescribing of Anticholinergic and Sedative Medications.

Cumulative exposure to anticholinergic and sedative medications has been associated with worsening physical function in older adults. We evaluated the feasibility of measuring physical function using wearable devices and explored the impact of reducing the anticholinergic and sedative medication burden in a pilot study of community-dwelling adults aged 60 years and older. Evaluations included the 10-meter walk test (10MWT), the Short Physical Performance Battery (SPPB), and the mini-BESTest. Two participants/month were recruited in one clinic in 2022. The five participants had a median age of

Gitelman-like syndrome: A rare complication of using aminoglycosides in tuberculosis - A case report.

Aminoglycosides are known to cause electrolyte disturbances. Approximately 8-26% of patients who receive an aminoglycoside for several days develop mild renal impairment that is almost always reversible (Brunton et al., 2013). A 46 year old male with multi-drug-resistant pulmonary tuberculosis with resistance to kanamycin is being presented, who was on injectable Capreomycin, Levofloxacin, Ethionamide, Cycloserine, pyrazinamide, linezolid and clofazamine for a period of four months. He presented to us with generalised weakness and pain in the lower limb muscles. Investigation revealed hypokale

Intensity of Withdrawal Symptoms During Opioid Taper in Patients with Chronic Pain-Individualized or Fixed Starting Dosage?

OBJECTIVE: Controlled opioid withdrawal is recommended for patients with chronic noncancer pain (CNCP) with insufficient pain reduction or intolerable side effects while on opioid treatment. Few studies have investigated the management of opioid withdrawal (OW). Most common are protocols with an individualized starting dosage (ISD), calculated from the last opioid intake. After two cases of overdose, we introduced a novel withdrawal protocol using a low fixed starting dosage (FSD) for safety reasons. The present study compares the intensity of withdrawal symptoms using the Subjective Opioid Wi

Heparin bridge therapy and post-polypectomy bleeding.

AIM: To identify risk factors for post-polypectomy bleeding (PPB), focusing on antithrombotic agents. METHODS: This was a case-control study based on medical records at a single center. PPB was defined as bleeding that occurred 6 h to 10 d after colonoscopic polypectomy and required endoscopic hemostasis. As risk factors for PPB, patient-related factors including anticoagulants, antiplatelets and heparin bridge therapy as well as polyp- and procedure-related factors were evaluated. All colonoscopic hot polypectomies, endoscopic mucosal resections and endoscopic submucosal dissections performe

Perceived Pain Following Hypnotic Deprescribing in Older Adults.

BACKGROUND: Older adults with chronic insomnia often use benzodiazepine receptor agonists (BZRAs) despite known associated risks and non-pharmacological alternatives such as cognitive behavioral therapy for insomnia (CBTI). CBTI reduces insomnia severity and could potentially improve other outcomes such as the impact of pain on daily activities, even when BZRAs are deprescribed. Yet concerns that deprescribing may worsen pain (which is often comorbid with insomnia) can be a barrier to engagement in BZRA deprescribing. This study examined changes in pain outcomes associated with deprescribing B

Substitution Patterns After Discontinuation of CNS-Active Medications in Older Adults in Primary Care.

BACKGROUND: Little is known about substitution of alternative medications in the context of deprescribing. The objectives were to: (1) determine the frequency of medication substitutions among those who discontinued a central nervous system (CNS)-active medication, and (2) characterize substitutions as potentially inappropriate (as per the 2023 Beers Criteria) versus not. METHODS: We conducted a secondary analysis that combined data from the intervention and usual care arms from the STOP-FALLS deprescribing trial that tested a health-system-embedded intervention designed to reduce prescriptio

Symptom and Medication Burden in Home-Based Palliative Care Patients.

Home-based palliative care (HBPC) improves quality of life by addressing distressing physical, psychosocial, and spiritual symptoms. However, symptom and medication burdens remain prevalent challenges. This study sought to characterize symptom prevalence and severity, the associated medication burden, and determine opportunities for deprescribing in the setting of HBPC. Patients referred to a home-based palliative care practice over a two-year period were included; 109 patients met inclusion criteria. Mean age was 78 years (± 11.8), and average Charlson Comorbidity Index (CCI) was 6.4 (± 2.7).

Programmed cell death protein 1 inhibitor-induced recalcitrant mixed small and medium vessel vasculitis.

Pembrolizumab, a programmed cell death protein 1 (PD1) inhibitor, has been known to be associated with several adverse reactions, including immune related adverse events. In less than one percent of patients, PD1 inhibitors have been linked to the development of connective tissue disease. Patients with previously known connective tissue disease are hypothesized to be at increased risk of flares in as many as 40% of cases. A 70-year-old man with a past medical history significant for rheumatoid arthritis in remission and stage IV lung adenocarcinoma presented to the dermatology clinic after one

3,4-diaminopyridine base effectively treats the weakness of Lambert-Eaton myasthenia.

INTRODUCTION: 3,4-diaminopyridine has been used to treat Lambert-Eaton myasthenia (LEM) for 30 years despite the lack of conclusive evidence of efficacy. METHODS: We conducted a randomized double-blind placebo-controlled withdrawal study in patients with LEM who had been on stable regimens of 3,4-diaminopyridine base (3,4-DAP) for ≥ 3 months. The primary efficacy endpoint was >30% deterioration in triple timed up-and-go (3TUG) times during tapered drug withdrawal. The secondary endpoint was self-assessment of LEM-related weakness (W-SAS). RESULTS: Thirty-two participants were randomized to c
