The transolecranon approach for the treatment of distal humerus fractures and nonunions is commonly used. A complication of the standard osteotomy is denervation of the anconeus muscle, which provides dynamic stability to the lateral side of the elbow by preventing varus and posterolateral rotatory instability. This article describes the anconeus flap transolecranon (AFT) approach, which utilizes an internervous plane to preserve the anconeus muscle and a chevron-shaped osteotomy for maximal joint exposure. The approach is straightforward to perform with limited complications.

Major disturbances of shoulder function secondary to axillary burn contracture require some form of surgical release if conservative therapy has been unsuccessful. Any area large enough to need extensive skin grafts should be considered for reconstruction using a parascapular fasciocutaneous flap. This flap, which may be elevated easily, provides an option for single-stage correction of the deformity without the need for long-term rehabilitation or splinting.

Our experience with 25 children in which cutaneous transureteroureterostomy had to be done because of neurogenic bladder is very gratifying. The procedure is very simple with few immediate and late complications. The cutaneous ureteral stoma works efficiently and so far has not offered important problems. We feel this method is superior to the bowel conduit methods.
