Treatment approaches to juvenile dermatomyositis.

Juvenile dermatomyositis (JDM) is a rare disease of childhood with significant morbidity. Although the mortality and morbidity has improved over recent decades, it is still a chronic disease for a significant proportion of children with JDM. There is a paucity of controlled trials in both adult and juvenile dermatomyositis. In this article, the different therapeutic agents used in clinical practice are outlined. Furthermore, our approach to the treatment of JDM with methotrexate and aggressively tapered steroids in an algorithmic fashion, is discussed.

Oropharyngeal dysphagia in polymyositis/dermatomyositis.

The nature of the oropharyngeal dysphagia in polymyositis/dermatomyositis (PM/DM) has been investigated by EMG methods. Nineteen patients with PM/DM were studied. The oropharyngeal phase of swallowing was evaluated by the electrophysiological methods measuring the laryngeal relocation time, pharyngeal transit time and the triggering of the pharyngeal phase of swallowing reflex. The EMG of cricopharyngeal muscle of the upper esophageal sphincter was also recorded in 10 patients. The patients have been compared with a group of 22 healthy controls matched with age and gender. Dysphagia limit was …

Hydroxyurea-induced dermatomyositis-like eruption.

Hydroxyurea is a cytotoxic chemotherapeutic agent used for myelodysplasia. The adverse cutaneous effects due to hydroxyurea include leg ulcers, hyperpigmentation of the skin and nails, a lichen planus-like eruption, lupus erythematosus, and a dermatomyositis-like eruption. We present a case of hydroxyurea-induced dermatomyositis-like eruption and review the features of this entity as previously reported.

[Clinical characteristics of juvenile dermatomyositis].

INTRODUCTION: Dermatomyositis, belonging to the group of the idiopathic inflammatory myopathies, is characterized by bimodal pattern of age-specific incidence of rates, with peaks in age group from 5 to 14 years (juvenile dermatomyositis) and in age group from 45 to 64 years (adult dermatomyositis). THE AIM OF THIS STUDY: Is to evaluate the clinical characteristics of 12 patients with juvenile dermatomyositis followed by the 3rd Department of Internal Medicine, University of Debrecen and the 2nd Department of Pediatrics, Semmelweis University, Budapest. METHODS: The authors analyzed the medi…

Juvenile dermatomyositis: recognition and treatment.

Juvenile dermatomyositis (JDM) is a multisystem disease characterized by acute and chronic lymphocytic inflammation of the skeletal muscle and skin. The disease is marked early in its course by the presence of a vasculopathy or vasculitis, and later by the development of calcinosis. Research has focused on the epidemiology, etiology, and pathogenesis of the disease with, until more recently, limited therapeutic interventions. This article highlights treatment regimens, both traditional and more recent interventions. Traditional treatment for JDM includes high dose corticosteroid treatment with…

Juvenile dermatomyositis in north India.

OBJECTIVE: To determine the clinical profile and therapeutic response of patients with juvenile dermatomyositis (JDM). DESIGN: Hospital based descriptive follow-up study. SAMPLE: 12 patients attending the Pediatric Rheumatology and Immunology Clinic over last five years. RESULTS: The patients were aged between 3 1/2 years to 12 years with a male to female ratio of 2:1. All patients had proximal muscle weakness at presentation. Distal muscle weakness and masseter atrophy was seen in 2 patients and neck flexor weakness and pharyngeal weakness was seen in 1 case. Muscle pain, tenderness or swa…

Cyclosporine treatment for dermatomyositis/polymyositis.

We report one case of dermatomyositis and one case of polymyositis and scleroderma in which the patients' condition improved after treatment with cyclosporine (cyclosporine A). Both patients' conditions were unresponsive to systemic steroid therapy. The addition of cyclosporine to the regimen enabled us to taper the steroid dosage; the lesions resolved and the results of laboratory tests returned to normal. In nine of thirteen studies of dermatomyositis treated with cyclosporine reported in the literature, definite benefit was achieved and no serious toxicity was noted.

Dermatomyositis associated with multifocal lipoatrophy.

A 10-year-old boy had juvenile dermatomyositis and multiple asymmetric lesions of lipoatrophy. The lipoatrophy had occurred without preceding clinical evidence of inflammation. Biopsy specimens of the skin confirmed a lobular panniculitis. This is the first report of the association of lipoatrophy and juvenile dermatomyositis.

Juvenile dermatomyositis: a clinical overview.

The child who develops the symptoms of the specific rash, proximal muscle weakness, and fatigue should seek medical care promptly. With the advances in physical and medical therapy, many of the consequences of the disease can now be ameliorated. There are suggestive data that JDMS and PM may each have a different pathophysiology, but more evidence is needed. The next few years should be exciting as there is increased effort to determine if there is, in fact, a causal relationship between Coxsackievirus B or other enterovirus and genetic factors that alter the susceptibility to or severity of t

Anaesthesia and acute dermatomyositis/polymyositis.

The anaesthetic management of two patients with severe muscle weakness--one suffering from acute dermatomyositis, the other from acute polymyositis--is described. Both patients presented for surgery for malignancy. Anaesthesia was induced with etomidate in one, thiopentone in the other. Alfentanil was used for analgesia and atracurium for muscle paralysis in both. Neuromuscular blockade was monitored using a peripheral nerve stimulator and no problems were experienced. Recovery of neuromuscular transmission and ventilatory function after operation were normal.
