Diazepam metabolism has been investigated in rat hepatocytes cultured for 3, 24, 48 and 72 hr under five different conditions. Although four of the treatments studied reduced markedly the spontaneous loss of cytochrome P450, they had different effects on the metabolism of diazepam (DZ) presumably by affecting the relative proportions of cytochrome P450 isozymes during the period of culture. Intravenously injected midazolam was associated with a greater degree of sedation, as assessed by 24-h retrograde amnesia, than the longer acting benzodiazepine, diazepam. Ultrasonic distress calls elicited in male rats by footshock under inescapable conditions were suppressed by diazepam. The suppression was blocked by pretreatment with Ro15-1788 or CGS8216, selective benzodiazepine receptor antagonists, while an opiate antagonist naloxone failed to modify the effect of diazepam. In contrast, naloxone antagonized but Ro15-1788 and CGS8216 did not antagonize beta-endorphin which also suppressed the ultrasonic vocalization. These findings suggest that benzodiazepine and opioid receptors may contribute separately to antianxiety activity.

This double-blind study of the effects of diazepam on painful cervical muscle spasms compared with those of phenobarbital and an inert placebo employed clinical, electrogoniometric and electromyographic recording methods for evaluation and computer analysis. The clinical results were equivocal but the technological recordings revealed a statistically significant greater desirable effect of diazepam compared to the other 2 treatments which would be expected to have only minor nonspecific effects.
