Intracranial vascular stenosis of haemodynamic significance is underappreciated both clinically and diagnostically. It is thought to be the cause of cerebral ischaemia in 8% of patients. Now it is possible to perform dilatation of the intracranial arterial stenosis by modern intraarterial angioplasty. Two cases are presented of successful dilatation of middle cerebral artery trunk stenosis. Diagnostic problems, follow up and pharmacological treatment as well as technique of intravascular treatment is described.

OBJECTIVES: To present a patient with monomicrobial anaerobic vertebral osteomyelitis secondary to a previously undescribed source of infection.

Subchorionic vascular aneurysms of the placenta are rare lesions and may present confusion with chorioangioma or focal mesenchymal dysplasia on sonography. To our knowledge, the findings of placental aneurysms have not been reported in the ultrasound literature. We present a case with detailed sonographic evaluation, including spectral and color Doppler and pathological analysis, that was mistaken for chorioangioma prenatally. Knowledge of this benign entity may allow the sonologist to recommend conservative management in similar cases.

OBJECTIVES: Percutaneous balloon mitral valve commissurotomy (BMC) is an alternative to surgical commissurotomy. Complications following BMC includes mitral regurgitation, iatrogenic atrial septal defect, residual mitral stenosis, and pericardial hemorrhage. In this study, 42 consecutive symptomatic patients (median age 60 (30-78) years, 86% females) with mitral valve stenosis (57% had severely deformed and/or calcified mitral valves) were treated with (Inoue) balloon dilatation during the period August 1989-June 1994. The patient was an infant girl who had undergone repair of a giant omphalocele. Acute gastric dilatation (AGD) is a well-known, acute, life-threatening disease in dogs but its feline counterpart is much less known. This paper provides descriptions of 2 cases of AGD in cats that the authors have seen. Including these a total of 10 cases of feline AGD have been reported. A 37 year-old female patient underwent catheter-balloon dilation for discrete subaortic stenosis. During the procedure severe mitral regurgitation was produced requiring emergency surgery. Transesophageal and intraoperative findings were posteriormedial papillary muscle rupture. Retrospective viewing of the cineangiogram during balloon inflation revealed distal balloon indentation, corresponding to the injured papillary muscle. This severe complication of the left ventricular outflow tract angioplasty should be avoided by careful positioning of the guidewire and balloon before inflation.

Familial occurrence of extrahepatic biliary atresia (EHBA) or biliary dilatation has been reported; however, the genetic influences on these disorders are still obscure. Gastroperitoneal adhesions, which developed after tube gastrostomy in a 3-year-old dog, caused an inverted L configuration of the pyloric antrum and duodenum, resulting in periodic episodes of gastric dilatation. Toxic megacolon occurs in colitis of differing aetiology. This report describes 15 patients with chagasic megacolon with this complication. The clinical signs and symptoms in all patients were pain and progressive abdominal distension accompanied by fever, severe toxaemia and shock.
