(Neo)adjuvant approvals for solid tumors by the US Food and Drug Administration 2010-2023.

BACKGROUND: (Neo)adjuvant systemic treatments aim to improve overall survival (OS)_by reducing recurrence risk; however, there can be challenges, such as reliance on surrogate endpoints, censoring, loss to follow-up, and impact on quality of life (QOL). We examine the design, reporting, and outcomes of Food and Drug Administration (FDA)-approved drugs for solid tumors. METHODS: We searched FDA (neo)adjuvant approvals for solid tumors 2010-2023 with time to event-endpoints. Trial characteristics, endpoints, and censoring were evaluated descriptively. Statistical designs and sample size calcula

Should Health Canada Use Reports From Foreign Regulators to Approve New Drugs?

Health Canada is consulting on whether to use reports from foreign regulators as the basis for approving new drugs. This Data Matters article looks at three metrics: the time gap between submission of new drug applications to the Food and Drug Administration (FDA) and Health Canada; the time gap between FDA and Health Canada approval of new drugs; and the proportion of FDA-approved new drugs that are also approved by Health Canada. This article argues that there is no convincing evidence that such a change is necessary. Health Canada should pause its plans for using foreign regulator reports u

Repurposing approved drugs as ferroptosis modulators: a critical review of clinical trials in cancer and neurodegeneration.

Ferroptosis is a regulated form of cell death that depends on iron and is marked by lipid peroxidation and the inactivation of glutathione peroxidase 4. It has emerged as a pathway of broad relevance to cancer and neurodegenerative disease. Preclinical studies have identified potent ferroptosis inducers (System Xc⁻ and GPX4 inhibitors) and inhibitors (liproxstatin/ferrostatin derivatives, FSP1-CoQ10 activators) with promising therapeutic potential. However, clinical translation remains limited. Since dedicated therapeutics targeting ferroptosis are still in early development, current clinical

Extending the Targets for Coronavirus Antivirals Beyond That of Approved Drugs: Insights From Preclinical Research.

Antiviral drugs have been approved for the treatment of COVID-19. However, they present pharmacological limitations, a mixed efficacy profile and target just two coronavirus proteins. To extend the range of druggable coronavirus proteins, researchers explored small molecule N-glycan binders as inhibitors of SARS-CoV-2 spike protein interaction with the cell receptor. Other groups investigated lipopeptides as inhibitors of cell fusion by viral spikes. High throughput screening of chemical libraries yielded viral maturation inhibitors that targeted the viral M protein. Massive screening led to i

Repurposing FDA-approved drugs for treatment of amyotrophic lateral sclerosis using machine learning.

INTRODUCTION: Amyotrophic Lateral Sclerosis (ALS) is a neurodegenerative disease characterized by loss of motor neurons. Current medications are largely ineffective, associated with side effects, and hindered by a lack of agreement over treatment pathways. The time-intensive process and high costs further limit the development of therapeutics. Therefore, this research aimed to identify FDA-approved drugs that inhibit three proteins (Casein kinase 1, Protein tyrosine kinase 2, Ephrin type-A receptor 4) associated with ALS. METHODS: A machine learning (ML) model was trained for each protein to

Identification of FDA-Approved Drugs That Inhibit SARS-CoV-2 and Human Norovirus Replication.

In this study, to identify novel compounds that inhibit severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) replication by targeting its protease, we screened an U.S. Food and Drug Administration (FDA)-approved drug library to determine their effects on SARS-CoV-2 3CL protease (3CLpro) activity using a cellular- and green fluorescent protein (GFP) reporter-based 3CLpro assay, called the FlipGFP-3CLpro assay. Among the hit compounds, 5 compounds (auranofin, endoxifen, netupitant, pimozide, and regorafenib) were selected for further analysis. We found that 3 compounds (auranofin, endoxi

An FDA-approved drug structurally and phenotypically corrects the K210del mutation in genetic cardiomyopathy models.

Dilated cardiomyopathy (DCM) due to genetic disorders results in decreased myocardial contractility, leading to high morbidity and mortality rates. There are several therapeutic challenges in treating DCM, including poor understanding of the underlying mechanism of impaired myocardial contractility and the difficulty of developing targeted therapies to reverse mutation-specific pathologies. In this report, we focused on K210del, a DCM-causing mutation, due to 3-nucleotide deletion of sarcomeric troponin T (TnnT), resulting in loss of Lysine210. We resolved the crystal structure of the troponin

Consideration on Food Effect Studies for Anticancer Drugs Approved in Japan Between 2001 and 2022.

In developing a new drug, studies to evaluate the food effect (FE) on the drug's pharmacokinetics (FE studies) are generally conducted in healthy subjects in the early stages of clinical development. Conversely, for anticancer drugs, which have many adverse effects, it is assumed that FE studies cannot be conducted in healthy subjects. However, many unknowns exist about how FE on oral anticancer drugs is examined in clinical development. In this study, we aimed to examine the characteristics of conducting FE studies on anticancer drugs approved in Japan to date. Between 2001 and 2022, 70 new o

Repurposing of FDA-approved drugs by targeting SIRT2 to alleviate inflammatory response and kidney injury.

Chronic kidney disease (CKD) involves the gradual loss of kidney function, usually caused by inflammation and fibrosis. The Sirtuin 2 (SIRT2) is predominantly expressed in proximal epithelial tubular cells, and specific inhibition of SIRT2 activity has been shown to alleviate fibrotic kidneys. We focus on targeting SIRT2 to discover a potent FDA-approved drug as a nephroprotective treatment for patients with CKD. Candidate compounds with high affinity and inhibitory effect for SIRT2 protein were filtered using structure-based virtual screening and quantitative structure-activity relationships

In Silico identification and modelling of FDA-approved drugs targeting T-type calcium channels.

Studies have shown that inhibition of the Cav3.1 T-type calcium channel can prevent or suppress neurological diseases, such as epileptic seizures and diabetic neuropathy. In this study, we aimed to use in silico simulations to identify a U.S. Food and Drug Administration (FDA)-approved drug that can bind to the Cav3.1 T-type calcium channel. We used the automated docking suite GOLD v5.5 with the genetic algorithm to simulate molecular docking and predict the protein-ligand binding modes, and the ChemPLP empirical scoring function to estimate the binding affinities of 2,115 FDA-approved drugs t

Screening of the FDA-approved drug library identifies CCL17 inhibitors that block arthritic pain.

Rheumatoid arthritis (RA) is an inflammatory and destructive autoimmune polyarthritis that causes pain, swelling and deformity in the joints. While clinical trials targeting GM-CSF in RA are showing promise, the potential side effects of anti-GM-CSF therapy highlight the need for identifying downstream mediators of GM-CSF action. CCL17, a downstream inflammatory mediator of GM-CSF in monocytes and macrophages, has been shown to mediate GM-CSF-driven inflammatory arthritis in animal models. CCL17 shares its receptor, CCR4, with CCL22; however, unlike CCL17, CCL22 has been implicated in resolvin

Dual ON/OFF-switch chimeric antigen receptor controlled by two clinically approved drugs.

The ability to remotely control the activity of chimeric antigen receptors (CARs) with small molecules can improve the safety and efficacy of gene-modified T cells. Split ON- or OFF-switch CARs involve the dissociation of tumor-antigen binding from T cell activation (i.e., CD3ζ) on the receptor (R-) and signaling (S-) chains, respectively, that either associate or are disrupted in the presence of a small molecule. Here, we have developed an inducible (i)ON-CAR comprising the anti-apoptotic B cell lymphoma protein 2 protein in the ectodomain of both chains which associate in the presence of ven

Evaluation of the Canadian approved drug screening equipment cut-off levels for tetrahydrocannabinol (THC).

In 2018, Canada introduced roadside oral fluid (OF) screening devices, called Approved Drug Screening Equipment (ADSE), as an investigative tool in impaired driving investigations to detect tetrahydrocannabinol (THC), cocaine and/or methamphetamine in drivers. In this work, we compare the detection and concentration of THC in blood samples collected from suspected impaired drivers that tested positive at the roadside for THC on an ADSE. The two ADSEs that were utilized were the Dräger DrugTest® 5000 (DDT) and the Abbott SoToxa™ (SoToxa), both configured with a THC OF concentration cut-off conc

Identifying FDA-Approved Drugs that Upregulate Utrophin A as a Therapeutic Strategy for Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy (DMD) is a neuromuscular disease caused by mutations and deletions within the DMD gene, which result in a lack of dystrophin protein at the sarcolemma of skeletal muscle fibers. The absence of dystrophin fragilizes the sarcolemma and compromises its integrity during cycles of muscle contraction, which, progressively, leads to reductions in muscle mass and function. DMD is thus a progressive muscle-wasting disease that results in a loss of ambulation, cardiomyopathy , respiratory impairment, and death. Although there is presently no cure for DMD, recent advances have

Synthetic approaches and application of clinically approved small-molecule Anti-HIV drugs: An update.

Application of chemotherapeutic agents to inhibit the HIV replication process has brought about a significant metamorphosis in the landscape of AIDS. Substantial declines in morbidity and mortality rates have been attained, accompanied by notable decreases in healthcare resource utilization. However, treatment modalities do not uniformly inhibit HIV replication in every patient, while the emergence of drug-resistant viral strains poses a substantial obstacle to subsequent therapeutic interventions. Furthermore, chronic administration of therapy may lead to the manifestation of toxicities. Thes

Highlights in TMPRSS2 inhibition mechanism with guanidine derivatives approved drugs for COVID-19 treatment.

Transmembrane protease serine 2 (TMPRSS2) has been identified as a critical key for the entry of coronaviruses into human cells by cleaving and activating the spike protein of SARS-CoV-2. To block the TMPRSS2 function, 18 approved drugs, containing the guanidine group were tested against TMPRSS2's ectodomain (7MEQ). Among these drugs, Famotidine, Argatroban, Guanadrel and Guanethidine strongly binds with TMPRSS2 S1 pocket with estimated Fullfitness energies of -1847.12, -1630.87, -1605.81 and -1600.52 kcal/mol, respectively. Synergistic Combinations of FDA-Approved Drugs with Ceftobiprole against Methicillin-Resistant Staphylococcus aureus.

New strategies are urgently needed to address the public health threat of antimicrobial resistance. Synergistic agent combinations provide one possible pathway toward addressing this need and are also of fundamental mechanistic interest. Effective methods for comprehensively identifying synergistic agent combinations are required for such efforts. In this study, an FDA-approved drug library was screened against methicillin-resistant Staphylococcus aureus (MRSA) (ATCC 43300) in the absence and presence of sub-MIC levels of ceftobiprole, a PBP2a-targeted anti-MRSA β-lactam. This screening identi
