Endoscopic diagnosis and extraction of biliary ascaris.

A woman was admitted to hospital after several episodes of severe upper right abdominal pain and vomiting. Previously she had had cholecystectomy for gallbladder stones, and later endoscopic papillotomy to treat common duct stone. Endoscopic retrograde cholangiography diagnosed an Ascaris worm in the common bile duct. One end of the 20 cm long living worm was caught in a Dormia basket when it protruded through the papillotomy opening, and the worm extracted with the endoscope.

Impacted common bile duct stones.

The impacted common bile duct stone can be managed either surgically or radiologically using methods such as percutaneous basket extraction or endoscopic papillotomy. At operation, if an impacted common bile duct stone cannot be removed by the usual methods, duodenotomy and sphincteroplasty are indicated. If the patient's condition is good, sphincteroplasty should not be avoided in the belief that other means of stone removal are superior. However, if the patient's condition is not ideal, sphincteroplasty should be deferred in favor of delayed removal by percutaneous or endoscopic techniques.

Endoscopic Ultrasound-Guided Hepaticogastrostomy via a Retrosternal Gastric Tube Conduit for Obstructive Cholangitis Caused by Ampullary Carcinoma: A Case Report.

Subtotal esophagectomy with retrosternal gastric tube reconstruction alters upper gastrointestinal anatomy and can create two obstacles to endoscopic retrograde cholangiopancreatography (ERCP): displacement and rotation of the pylorus and proximal duodenum, which impede side-viewing duodenoscopy, and ampullary carcinoma, which may obscure the bile duct orifice and prevent selective cannulation. When endoscopic drainage fails, endoscopic ultrasound-guided hepaticogastrostomy (EUS-HGS) offers an alternative route for internal biliary decompression. Reports of EUS-HGS performed from within a retr

Rapidly Progressive Coexistence of Gastric Antral Vascular Ectasia (GAVE) and Portal Hypertensive Gastropathy (PHG) Presenting With Iron Deficiency Anaemia: Successful Use of PuraStat in a Patient With Implantable Cardioverter-Defibrillator (ICD).

Gastric antral vascular ectasia (GAVE), or "watermelon stomach", is an uncommon but important cause of chronic gastrointestinal blood loss and iron deficiency anaemia (IDA). Although associated with cirrhosis and portal hypertension, it is regarded as distinct from portal hypertensive gastropathy (PHG), and the two conditions rarely coexist. We report an 85-year-old woman with cirrhosis and multiple comorbidities who presented with severe symptomatic IDA (Hb 60 g/L). Remarkably, oesophago-gastro-duodenoscopy (OGD) performed just five months earlier had been entirely normal, yet repeat gastrosc

Magnified endoscopic observation using narrow-band imaging of periampullary adenoma in a patient with familial adenomatous polyposis.

BACKGROUND: Adenoma of the major papilla carries a relatively high risk of malignant transformation to carcinoma, the leading cause of death in patients with familiar adenomatous polyposis (FAP) after colectomy. CASE REPORT: A 35-year-old man had undergone prophylactic colectomy for FAP 3 years earlier. On the forward-viewing and side-viewing endoscopy done for surveillance, the overlying mucosa of the major papilla showed even granularity. On magnifying duodenoscopy using a narrow-band system (NBI), which uses modified optical filters and yields clear images of fine surface structures on the

Photodynamic therapy for malignant tumours of the ampulla of Vater.

Ten patients with ampullary carcinoma, not suitable for surgery, were treated with endoscopic photodynamic therapy (PDT) to evaluate the feasibility and safety of treatment. Patients received 4 mg kg-1 of haematoporphyrin derivative intravenously. Two days later, a duodenoscopy was performed and red (630 nm) light delivered to the tumour at fixed energy densities of 50 J or 200 J cm-1 per application, depending on the type of optical fibre used. The tumours were treated by three or four light applications at each session. Treatment was repeated up to five times at intervals of three to six mon

Surgical resection combined with CHOP chemotherapy plus rituximab for a patient with advanced mesenteric diffuse large B cell lymphoma.

Herein is described a mesenteric diffuse large B cell lymphoma (DLBCL) case with a bulky mass which had achieved a complete remission by the combination therapy of a surgical resection and the CHOP chemotherapy with rituximab. A 78 year old man was referred to the Rakuwakai-Otowa Hospital due to a left lower abdominal tumor. Abdominal CT and MRI showed a bowel-like mass in the left lower abdominal cavity. Abdominal US revealed lymph node swellings at the paraaortic region. By the gastro-duodenoscopy, DLBCL of non-Hodgkin's lymphoma was proven at the 2nd portion of the duodenum. On laparotomy,

Low grade MALT lymphoma of the stomach: treatment outcome with radiotherapy alone.

In order to evaluate the role of radiation therapy in the management of low-grade mucosa-associated lymphoid tissue lymphoma of the stomach (MLS), in patients with no evidence of Helicobacter pylori (H. pylori) or who had not responded to H. pylori eradication treatment, we analyzed the treatment outcome of patients who had received radiotherapy alone. Between Jan 1995 and May 2001, 6 patients with low- grade MLS were treated with radiotherapy alone. The median radiation dose was 30.6 Gy (range; 30 - 39 Gy) in a daily fractions of 1.5 - 1.8 Gy. Each patient had an endoscopic esophago-gastro-du

Nonoperative removal of giant common bile duct calculi.

Endoscopic sphincterotomy has allowed us to extract relatively large stones from the common bile duct as compared with other methods utilizing a T-tube tract or the percutaneous transhepatic route. Twenty-four patients with large stones over 20 mm in diameter were selected and reviewed from a series of 469 sphincterotomy patients. Eleven stones passed into the duodenum spontaneously, the maximal size of which was 30 by 43 mm. Passage occurred within 4 days after sphincterotomy in 27 percent, 5 to 7 days after the procedure in 55 percent, and 8 to 13 days after the procedure in 18 percent and w

Endoscopic retrograde cholangiopancreatography training using a silicone simulator fabricated using a 3D printing technique (with videos).

Endoscopic retrograde cholangiopancreatography (ERCP) training remains challenging. This study used 3D printing techniques to develop and optimize a portable ERCP training simulator and to implement basic and advanced practical techniques. Subsequently, we aimed to determine whether endoscopy trainees acquired proficiency in ERCP techniques and assess any improvements in their skill levels from using this model. An ERCP training model was generated using 3D printing techniques, including five distinct interchangeable and transparent ampullar-common bile duct (CBD) modules. A prospective study

Small Intestine Bacterial Overgrowth is associated with increased Campylobacter and epithelial injury in duodenal biopsies of Bangladeshi children.

Small intestine bacterial overgrowth (SIBO) has been associated with enteric inflammation, linear growth stunting, and neurodevelopmental delays in children from low-income countries. Little is known about the histologic changes or epithelial adherent microbiota associated with SIBO. We sought to describe these relationships in a cohort of impoverished Bangladeshi children. Undernourished 12-18-month-old children underwent both glucose hydrogen breath testing for SIBO and duodenoscopy with biopsy. Biopsy samples were subject to both histological scoring and 16s rRNA sequencing. 118 children we

Updates on the Management of Ampullary Neoplastic Lesions.

Ampullary neoplastic lesions (ANLs) represent a rare cancer, accounting for about 0.6-0.8% of all gastrointestinal malignancies, and about 6-17% of periampullary tumors. They can be sporadic or occur in the setting of a hereditary predisposition syndrome, mainly familial adenomatous polyposis (FAP). Usually, noninvasive ANLs are asymptomatic and detected accidentally during esophagogastroduodenoscopy (EGD). When symptomatic, ANLs can manifest differently with jaundice, pain, pancreatitis, cholangitis, and melaena. Endoscopy with a side-viewing duodenoscopy, endoscopic ultrasound (EUS), and mag

[Transorbito-maxillary percutaneous endoscopic gastrostomy].

Peroral, enteral feeding is often impossible in patients with head and neck cancer. Percutaneous endoscopic gastrostomy is a well-established, quick, minimally invasive, and safe procedure for providing long-term enteral feeding. Space-occupying tumour mass and altered anatomy due to surgery inhibit the gastroscope’s peroral introduction and the feeding tube’s placement in some instances. Various access routes and modified insertion techniques are recommended to overcome the feeding tube insertion challenges. We present a rare case of a 64-year-old head and neck cancer patient who was unable t

A Prolonged History of Denture Impaction for 12 Weeks In Situ.

Foreign body ingestion has serious consequences if left untreated. Impacted dentures for a prolonged period can lead to life-threatening complications. Therefore, prompt diagnosis and immediate intervention are lifesaving. Our patient presented to his local accident and emergency department after having swallowed his dentures during a meal. Initial investigations and workup detected no abnormalities and he was discharged back to the community. Twelve weeks following ingestion, he had developed dysphagia and weight loss which prompted an urgent referral for oesophago-gastro-duodenoscopy (OGD).
