Modification of the stages in total reconstruction of the auricle: Part II. Grafting the three-dimensional costal cartilage framework for concha-type microtia.

Among the microtic disorders, those with the remnant ear, ear lobule, concha, acoustic meatus, tragus, and incisura intertragica are classified as concha-type microtia. From these characteristic factors, concha-type microtia has been thought to be a milder form of microtic disorder in comparison with lobule-type microtia. The constructed modified three-dimensional costal cartilage framework is grafted to the superior portion of the auricle and fixed to the inferior remnant cartilage of the auricle (conchal portion). This may appear to be a simple procedure in comparison with the other microtic

Temporoparietal Fascia Sandwich Technique in the Immediate Reconstruction of Human Bite Ear Amputation.

BACKGROUND: Traumatic ear amputation is a rare injury. Ear replantation is the gold standard in dealing with amputated ears. However, this is not always feasible. In this case series, the authors discuss the immediate reconstruction of traumatic ear amputation using temporoparietal fascia with the same skin from the amputated ear, which provided a high esthetic result and patient satisfaction. PATIENTS AND METHODS: The authors present a case series of 3 patients from July 2018 to May 2020 who suffered from traumatic ear amputation due to a human bite and were successfully reconstructed using

Biochemical properties of tissue-engineered cartilage.

OBJECTIVE: Microtia is treated with rib cartilage sculpting and staged procedures; though aesthetically pleasing, these constructs lack native ear flexibility. Tissue-engineered (TE) elastic cartilage may bridge this gap; however, TE cartilage implants lead to hypertrophic changes with calcification and loss of flexibility. Retaining flexibility in TE cartilage must focus on increased elastin, maintained collagen II, decreased collagen X, with prevention of calcification. This study compares biochemical properties of human cartilage to TE cartilage from umbilical cord mesenchymal stem cells (U

Salvage of Ear Framework Exposure in Total Auricular Reconstruction.

BACKGROUND: One of the most common complications of total auricular reconstruction is delayed wound healing, which results in skin necrosis and exposure of the ear framework. Various options exist for salvage of the exposed ear framework. METHODS: From January 2009 to May 2014, 149 patients underwent total auricular reconstruction using an autogenous cartilage framework or porous polyethylene framework (Medpor; Stryker, USA). An autogenous cartilage framework was used in 48 patients, and a Medpor framework was used in 101 cases. Three cases of framework exposure (3/48, 6.3%) were observed amo

Novel Concept of Attaching Endoscope Holder to Microscope for Two Handed Endoscopic Tympanoplasty.

UNLABELLED: The well established techniques in tympanoplasty are routinely performed with operating microscopes for many decades now. Endoscopic ear surgeries provide minimally invasive approach to the middle ear and evolving new science in the field of otology. The disadvantage of endoscopic ear surgeries is that it is one-handed surgical technique as the non-dominant left hand of the surgeon is utilized for holding and manipulating the endoscope. This necessitated the need for development of the endoscope holder which would allow both hands of surgeon to be free for surgical manipulation and

A pilot study comparing mechanical properties of tissue-engineered cartilages and various endogenous cartilages.

BACKGROUND: Mechanical properties of tissue-engineered cartilage and a variety of endogenous cartilage were measured. The main goal was to evaluate if the tissue-engineered cartilage have similar mechanical characteristics to be replaced with rib cartilage in microtia reconstruction. Such study lays the foundation for future human clinical trials for microtia reconstruction. METHOD: Atomic force microscopy and compression testing were used to measure the viscoelasticity of tissue-engineered cartilage (stem cell seeded on Poly lactic co-glycolytic acid nanofibers and Pellet) and endogenous car

Injectable chondroplasty: Enzymatic reshaping of cartilage grafts.

OBJECTIVE/HYPOTHESIS: To develop an injection-based enzymatic technique that selectively softens cartilage tissue for reshaping cartilaginous structures in the head and neck. MATERIALS AND METHODS: Two groups were formed using fresh rabbit ears: (1) whole rabbit ear group; (2) composite graft group (2.5mm×3.0cm specimens sectioned from the central region of the pinna). Subperichondrial injections using three enzymes (hyaluronidase, pronase, and collagenase II) in sequence were performed for the experimental specimens from both groups. In the control specimens, phosphate buffered saline was in

The fate of diced cartilage grafts of traumatized versus nontraumatized origin.

BACKGROUND: Diced cartilage grafts have long been used in rhinoplasty. Along with their various reported advantages, they also have some disadvantages. The irregular resorption rate of the engrafted mass is one of the major issues noted with diced cartilage grafts. An explanation for the unpredictable resorption rate has not yet been elucidated. This study aimed to determine the role of traumatized versus nontraumatized cartilage as the source of diced cartilage grafts. METHODS: This study included the noses of 32 patients (19 traumatized noses and 13 nontraumatized noses) who underwent surge

Do combined otoplasty techniques really improve the surgical outcomes for prominent ear correction in adult patients?

OBJECTIVE: Prominent Ear is a genetic malformation of the human ear. It occurs due to malformation of cartilage during primitive ear development in intrauterine life. Multiple surgical techniques for the correction of protruding ears have been described which include: incisions, sutures, and cartilage scoring, isolated or in combination. To date, no ideal method that fulfills the aim of a complete surgical resolution has been described in adult patients. The aim of our study is to discuss the main surgical technique' described in order to identify technical association to obtain optimal surgic

Hypoxia Differentially Affects Chondrogenic Differentiation of Progenitor Cells from Different Origins.

BACKGROUND AND OBJECTIVES: Ear cartilage malformations are commonly encountered problems in reconstructive surgery, since cartilage has low self-regenerating capacity. Malformations that impose psychological and social burden on one's life are currently treated using ear prosthesis, synthetic implants or autologous flaps from rib cartilage. These approaches are challenging because not only they request high surgical expertise, but also they lack flexibility and induce severe donor-site morbidity. Through the last decade, tissue engineering gained attention where it aims at regenerating human t…

Intrinsic auricular muscles and auricular form.

Plical folding of the cartilagenous ear plate is largely determined by the insertions of four extrinsic and seven intrinsic auricular muscles. Dissection of ten external auricles from anencephalic fetuses of six or more months' gestation was done. The transversus and obliquus auricular muscles found in association with the antihelical fold were evaluated. Four auricles were of normal form and had the transversus and obliquus auricular muscles with typical insertions in the cartilage. Six of the ten auricles had a simplified form, with no antihelical folding in five and minimal development in o

Correction of upturned nasal tip with a costal cartilage graft in bilateral cleft lip patients.

We used a piece of costal cartilage as a cartilaginous strut to correct the upturned nasal tip in patients with bilateral cleft lip. The grafted cartilage provides more definition of the tip and improves the obtuse nasolabial angle. Neither the septal cartilage nor the ear cartilage has enough strength to shape the tip. This method of correction has consistently produced favorable, long-lasting results in adults and has improved the contour of the nasal tip in younger patients.

Correction of auricular deformity caused by high ear-piercing: case report.

In recent years, high ear-piercing and wearing earrings in the cartilaginous region of the auricle have become popular. The frequent occurrence of subsequent complications also has been pointed out. The present case is of a 17-year-old girl with an auricular deformity caused by infections occurring after piercing in the scapha. The authors performed an auriculoplasty by excision of the deformed cartilage and autologous cartilage graft from the ipsilateral concha. At present, 1 year and 5 months after the operation, no sign of recontracture is noted, and the auricle remains in a gratifying shap

Endoscopic autologous cartilage injection for the patulous eustachian tube.

Patulous eustachian tube (PET) can have a significant negative impact on a patient's quality of life. Several methods of surgical management can be an option to treat PET, and our objective is to evaluate the safety and efficacy of autologous cartilage injection in patients with PET. Thirty-three ears of twenty-five patients with chronic PET refractory to conservative treatment were enrolled to this study. Autologous tragal cartilage was harvested, and chopped into fine pieces to allow its injection using a 1 cc Bruening syringe. Endoscopic cartilage injection was performed submucosally into t

A new device for creating and positioning an autogenous cartilage framework during microtia reconstruction.

OBJECTIVE: This paper describes the fabrication and use of a three-dimensional appliance, known as the microtia surgical positioner, to more accurately position and better sculpt the autogenous rib cartilage graft during microtia reconstruction. STUDY DESIGN: The authors introduce a new device design and surgical application. METHODS: An impression and plaster cast were made from the patient's auricular defect. On this cast, the artist then created a wax baseplate and an esthetically pleasing wax sculpture of an ideal ear similar to the patient's normal contralateral ear. The surface contour

Relapsing polychondritis: case report and review of the literature.

A 65-year-old man who experienced the acute onset of pain, redness, and swelling of his right ear was treated with antibiotics for erysipelas. Subsequently, the other ear and both eyes became inflammed. Relapsing polychondritis was diagnosed and treatment with prednisone was successful. Relapsing polychondritis, a rare multisystem disease, presents most commonly with auricular chondritis. Other clinical manifestations include arthritis, nasal chondritis, scleritis, damage to tracheal and bronchial cartilage, and aortic, cardiac, and renal involvement. The cause is autoimmune. Recurrent episode
