Ebstein's anomaly with 'reversible' functional pulmonary atresia.

We present the case of an infant with prenatal diagnosis, at 32 weeks gestation, of Ebstein's anomaly without anterograde flow from right ventricular to pulmonary atresia (PA)-functional PA with flow reversal in the ductus arteriosus. Prostaglandin E1 was started after birth. Chest X-ray showed severe cardiomegaly and echocardiogram confirmed Ebstein's anomaly with a thickened non-opening pulmonary valve without anterograde flow but with mild regurgitation. Multidisciplinary team decision was to progressively reduce prostaglandins and have an expectant attitude. Peripheral oxygen saturation ab

360-Degree Cone Reconstruction for Ebstein's Anomaly.

Several modifications of the cone procedure as first described by da Silva have been reported. We have used a complete 360-degree leaflet detachment coupled with a 60-degree counterclockwise rotation in performing the cone procedure in children and adults. We believe that complete leaflet detachment coupled with counterclockwise rotation provides both for better alignment of the leaflets and subvalvular apparatus and for a more symmetric distribution of tension after leaflet reattachment, thus potentially resulting in better valve function and longevity of repair.

Electro-vectorcardiographic and electrophysiological aspects of Ebstein's anomaly.

Ebstein's anomaly is a congenital heart disease where the most important anatomic feature is the inferior displacement of the tricuspid valve leaflets. Vectorcardiographic features are mainly forgotten and electrocardiographic features may be unrecognized by cardiologists handling adult patients.

Cardiac Resynchronization Therapy in Patients with Ebstein's Anomaly.

Ebstein's anomaly is a rare congenital heart disease characterized by apical displacement of the septal and posterior tricuspid valve leaflets. It is often complicated with left ventricular (LV) dysfunction as well as right-sided abnormalities. On the other hand, in the presence of LV dysfunction, right ventricular pacing is likely to aggravate the diseased LV function, which is termed pacemaker-induced cardiomyopathy. Thus, deteriorating effects of RV pacing on cardiac function might be enhanced and result in pacemaker-induced cardiomyopathy in patients with Ebstein's anomaly, even if they ha

Outcome of neonates with Ebstein's anomaly in the current era.

Ebstein's anomaly is frequently detected before birth, with prenatal detection accounting for the majority of cases in the current population. This study aimed to identify the outcome variables among these infants. The medical records of 59 patients with neonatal Ebstein's anomaly managed at the Asan Medical Center between January, 2001 and June, 2012 were investigated retrospectively. In 46 cases, the diagnosis was made prenatally. Surgical/interventional procedures were performed for 27 of the analyzed patients. Biventricular repair was successful for 12 patients but not for 9 patients with

Ebstein's anomaly of the tricuspid valve: the cone repair.

Ebstein's anomaly is a cardiac malformation affecting the tricuspid valve and right ventricle with a wide range of anatomic and pathophysiologic presentations. The cone repair technique is described in detail, as applied to several types of this complex congenital heart disease. The importance of extensive mobilization of displaced and tethered tricuspid leaflets to permit a good leaflet-to-leaflet coaptation after the cone construction is highlighted. This technique was performed in 100 patients with a hospital mortality rate of 3.0%, good clinical outcome, and no need for tricuspid valve rep

Fetal Ebstein's anomaly: early and late appearance.

OBJECTIVE: To present sonographic findings in early and late appearance of fetal Ebstein's anomaly. METHODS: Fetal sonography was performed in 53,447 consecutive pregnant women at 14 to 16 weeks' gestation. RESULTS: Ebstein's anomaly was detected in eight fetuses, in four of them additional anomalies were observed. All eight pregnancies were terminated. In an additional fetus, a normal four-chamber view without valvular regurgitation was visualized at 15 weeks' gestation. At 24 weeks, a normal four-chamber view was depicted. No Doppler examination was performed at that time. The newborn was

[Simultaneous operation for Ebstein's anomaly and lung cancer].

The number of patients with coexisting cardiac disease and lung cancer have been increasing. The issue of performing simultaneous pulmonary resection and cardiac surgery remains controversial. We report a patient who underwent simultaneous surgery for Ebstein's anomaly and lung cancer with a good outcome. The tricuspid valve replacement with bioprothesis and the closure of a foramen ovale for Ebstein's anomaly was 1st performed under cardiopulmonary bypass. The resection of the right upper pulmonary lobe and mediastinal lymph nodes followed. The postoperative course was uneventful.

Three-dimensional transesophageal echocardiography in Ebstein's anomaly.

Three-dimensional (3D) transthoracic echocardiography has advantages over traditional two-dimensional (2D) echocardiography in visualizing tricuspid valve morphology in Ebstein's anomaly. We describe the application of intra-operative 3D transesophageal echocardiography during a tricuspid valve repair procedure in a patient with Ebstein's anomaly. Intra-operatively three-dimensional transesophageal echocardiographic (3D TEE) data sets revealed morphology and function of the tricuspid valve, right ventricle outflow tract (RVOT) and pulmonary valve before and after repair. Tricuspid valve leafle

Ebstein's anomaly in pregnancy: maternal and neonatal outcomes.

AIM: Ebstein's anomaly is a rare congenital cardiac abnormality, associated with cyanosis and arrhythmia. Patients often reach childbearing age and pregnant women pose a challenge to the treating physician. We describe the course and outcome of pregnancy in women with Ebstein's anomaly and discuss the related management issues. METHODS: Analysis of pregnancies in four women with Ebstein's anomaly was carried out in a referral institute in northern India. Data of two women were analyzed retrospectively and the other two women were prospectively followed in their pregnancies during the study pe

How I manage neonatal Ebstein's anomaly.

Ebstein's anomaly of the tricuspid valve is characterized by a wide spectrum of severity. The condition involves more than a malformation of the tricuspid valve alone; a substantial portion of the right ventricle may be involved as well. Those patients presenting with symptoms in the first month of life represent a challenging group with a high mortality and uncertainty as to the best treatment options. In this review, the practical decision making process and the outcomes for neonates at one center are discussed.

[Ebstein's anomaly and interventricular communication, a rare association].

The Ebstein's anomaly is a malformation of the tricuspid valve, in which the septal and posterior leaflets are attached to the wall of the right ventricle. The usual association is with an atrial septal defect, followed by pulmonary stenosis and pulmonary atresia; the ventricular septal defect is unusual. We present three cases with diagnosis of Ebstein's anomaly and ventricular septal defect. The initial presentation of the first two was an acyanotic heart defect with congestive heart failure and increased pulmonary flow, whereas the third patient was cyanotic and functionally impaired. The t

Ebstein's anomaly: a complex congenital heart defect.

Ebstein's anomaly is a complex, congenital heart defect characterized by a malformation of the tricuspid valve and right side of the heart. A variety of cardiac abnormalities are associated with Ebstein's anomaly, including atrial septal defect, conduction system abnormalities, patent foramen ovale, pulmonary stenosis or atresia, and ventricular septal defect. The clinical course of a patient with Ebstein's anomaly depends on the severity of the abnormalities present. Surgical repair of Ebstein's anomaly involves repair or replacement of the tricuspid valve and repair of any associated cardiac
