Ocular manifestations in classic homocystinuria.

BACKGROUND: Classic homocystinuria (HCU), or cystathionine beta-synthase (CBS) deficiency, is a rare inborn error of methionine metabolism. … opography. RESULTS: Ten patients with HCU (20 eyes) were included. The most frequent findings were ectopia lentis(n = 20) and myopia (n = 9). Biometry, ultrasound, OCT and topography findings were available for four patients. One patient had keratoconus; one had abnormal retinal pigmentation; and two had lens surgery scars with irregular astigmatism. CONCLUSIO …

Homocysteinemia due to MTHFR deficiency in a young adult presenting with bilateral lens subluxations.

The most common cause of isolated inherited homocysteinemia is a deficiency of the enzyme cystathionine β-synthase (CBS). Clinical manifestations of CBS deficiency can include ectopia lentis, thromboembolism, marfanoid habits, and intellectual disability. CBS deficiency, which affects the transsulfuration pathway, is marked biochemically by elevated serum homocysteine and plasma methionine. We report a patient with homocysteinemia, low plasma methionine, and no significant neurological abnormalities who presented with bilateral subluxated crystalline lenses due to a 5,10-methylenetetrahydrofol…

Novel Marfan Syndrome-Associated Mutation in the FBN1 Gene Caused by Parental Mosaicism and Leading to Abnormal Limb Patterning.

Marfan syndrome is an autosomal dominant disorder of the connective tissue caused by mutations in the fibrillin-1 (FBN1) gene. Mutations affecting cysteine residues within the epidermal growith factor-like calcium-binding domains (EGF_CA) of FBN1 are associated with Marfan syndrome features and, especially, with ectopia lentis. We report a novel substitution, affecting the first cysteine of an EGF_CA-binding module encoded by exon 63 of FBN1 (C2571Y), in a patient presenting with typical Marfan syndrome features but without ectopia lentis. The involvement of this particular carboxi-terminal do…

Selected eye defects of special importance in pediatrics.

This article reviews six defects of special importance in the care of children: aniridia, ectopia lentis, cataracts, glaucoma, colobomata, and optic nerve hypoplasia. In addition to causing potentially serious impairment of vision, these ocular disorders may be associated with significant systemic disease and genetic abnormalities.

A novel mutation in the fibrillin gene (FBN1) in familial arachnodactyly.

Mutations of the fibrillin gene (FBN1) are known to cause classical Marfan's syndrome, ectopia lentis and neonatal Marfan's syndrome. We have identified a novel missense mutation in exon 28 of the FBN1 gene (R1170H) which is responsible for an atypical marfanoid phenotype characterised by dolichostenomelia and arachnodactyly.

Genetic and functional linkage between ADAMTS superfamily proteins and fibrillin-1: a novel mechanism influencing microfibril assembly and function.

Tissue microfibrils contain fibrillin-1 as a major constituent. Microfibrils regulate bioavailability of TGFβ superfamily growth factors and are structurally crucial in the ocular zonule. FBN1 mutations typically cause the Marfan syndrome, an autosomal dominant disorder manifesting with skeletal overgrowth, aortic aneurysm, and lens dislocation (ectopia lentis). Infrequently, FBN1 mutations cause dominantly inherited Weill-Marchesani syndrome (WMS), isolated ectopia lentis (IEL), or the fibrotic condition, geleophysic dysplasia (GD). Intriguingly, mutations in ADAMTS [a disintegrin-like and me…

Weill-Marchesani syndrome: report of an unusual case.

We report a single case of Weill-Marchesani syndrome, typically characterized by progressive joint stiffness, brachiymorphy, brachydactyly, and ectopia lentis. The clinical case appears particularly interesting as the patient also had primary osteoporosis, which until now has not been considered as a possible manifestation of Weill-Marchesani syndrome.

Marfan's syndrome: a refractive challenge for optometrists.

We present an interesting case of a child with Marfan's syndrome with ectopia lentis, who was followed for five years. The changes in refractive and visual findings of this child during this period are discussed. The importance of careful oculo-visual examination and possible management options in such children are highlighted.

Arachnodactyly--a key to diagnosing heritable disorders of connective tissue.

Here, we focus on the differential diagnosis of diseases with features of the marfanoid habitus. Ectopia lentis (lens dislocation) and aortic root dilation or dissection are cardinal features of MFS. Distinguishing MFS from other HCDTs has important implications for treatment, as cardiovascular and ocular complications commonly seen in patients with MFS are n …

Myopia in the Diagnosis of Marfan Syndrome: An Important Early Sign of a Systemic Condition.

Marfan syndrome is a genetic connective tissue disorder that is a frequent cause of aortic dissection in younger patients. We report a case of a patient with a history of ectopia lentis in his third decade and abdominal striae since adolescence, who presented with Stanford type A dissection at age 48.

Exudative retinal detachment secondary to dislocated lenses.

2 cases of retinal detachment with ectopia lentis are described. In each, the retinal elevations had surface features consistent with exudative detachments. Following removal of the posteriorly displaced lenses spontaneous retinal attachment was noted. The clinical and surgical course of these patients suggest a diagnosis of lens-induced exudative retinal detachment.

[Posterior chamber implantation of iris-claw intraocular lenses in a child with bilateral dislocated spherophakia].

A 6-year-old boy with blurred vision for 18 months was diagnosed as bilateral spherophakia and ectopia lentis. He received lensectomy and implantation of an iris-claw intraocular lens in the posterior chamber in both eyes. Good visual results were achieved, and there were no complications during the two-year follow-up.

Acute aortic dissection (Stanford type B) during pregnancy.

We report a case of acute aortic dissection (Stanford type B) that occurred in pregnant woman at 34-week gestation. She had no systemic characteristics of Marfan syndrome, however she exhibited a mutation of FBN1, Arg 545 Cys, which has been found to correlate with ectopia lentis but not with aortic dissection.

[Hereditary diseases with lens dislocation: clinical aspects].

Ectopia lentis is a common denominator of some connective tissue diseases such as Weill-Marchesani, Ehlers-Danlos, and Marfan's syndromes, osteogenesis imperfecta and homocystinuria. Early differential diagnosis (e.g., Marfan's syndrome vs. homocystinuria) is mandatory for prognostic (aortic aneurysm in Marfan's syndrome), therapeutic (dietetic and vitamin therapy in homocystinuria), and genetic reasons (genetic counseling).

Mutations in ASPH cause facial dysmorphism, lens dislocation, anterior-segment abnormalities, and spontaneous filtering blebs, or Traboulsi syndrome.

We have previously described a syndrome characterized by facial dysmorphism, lens dislocation, anterior-segment abnormalities, and spontaneo … to severely impair the enzymatic function of ASPH, which suggests a possible link to other forms of ectopia lentis given that many of the genes implicated in this phenotype encode proteins that harbor EGF domains. Developmental analysis of Asph revealed an expression pattern consistent with the proposed link to the human syndrome. Indeed, Asph-knockout mice had …
