An elbow dislocation in a child with missed medial epicondyle fracture and late ulnar nerve palsy.

Elbow dislocations associated with a medial epicondyle fracture and ulnar nerve palsy are uncommon injuries. We present the case of an 11-year-old girl with an elbow dislocation treated by closed manual reduction. The medial epicondyle fracture was missed initially until she developed an ulnar nerve palsy 2 months later. Intraoperatively we discovered the bony epicondyle piercing the joint capsule and compressing the ulnar nerve. Removal of the bony fragment relieved her symptoms and she returned to normal activities at 1 year follow-up. We would like to highlight this rare occurrence and pres

Management of supracondylar fracture of humerus in children by close reduction and immobilization of the elbow in extension and supination.

BACKGROUND: A Supracondylar fracture of the humerus is the most common fracture in children around the elbow and is also called first decade injury. If not treated properly it may lead to disability due to elbow stiffness, and sequel of neuro-vascular injuries like Volkmann ischemic contracture etc. This study was undertaken to determine the outcome of close reduction and immobilization of the elbow in extension and supination in displaced supracondylar fractures of the humerus in children. METHOD: This Quasi-experimental study was conducted in the Department of Orthopaedics, Postgraduate Med

Complex fractures of the distal humerus in the elderly. The role of total elbow replacement as primary treatment.

Between 1995 and 2000, 19 consecutive patients with fractures of the distal humerus were treated by primary total elbow replacement using the Coonrad-Morrey prosthesis. No patient had inflammatory or degenerative arthritis of the elbow. The mean age at the time of injury was 73 years (61 to 95). According to the AO classification, 11 patients had suffered a C3 injury, two a B3 and two an A3. One fracture was unclassified. Two patients died from unrelated causes and one was unable to be assessed because of concurrent illness. The mean time to follow-up was three years (1 to 5.5). At follow-up 1

Evaluation of the late neurologic deficits accompanied by hypertrophic scars and keloids in children with elbow fractures.

In this study, the relation between hypertrophic scar and keloid (HSc) lesions around incisions and late neurologic deficits was investigated in operated elbow fractures in children. Six elbow fracture patients with HSc lesions were evaluated for neurologic deficits with late onset. The fractures were all closed and treated with open reduction and internal fixation. No neurologic deficit was detected before surgery and in the early postoperative periods. Late neurologic deficits observed in these patients were evaluated according to British Medical Research Committee scoring scale on admission

A retrospective study on elbow function after internal fixation of intercondylar fracture of adult humerus.

Between 1989 and 1993, 20 intercondylar fractures of the distal humerus were treated by open reduction, internal fixation and early postoperative mobilisation. One patient died on the third postoperative day as a result of multiple injuries, leaving 19 patients for evaluation. The mean follow-up period was 4.1 years (range 2.0 to 6.7 years). According to the Muller system, there were 7 type C1 and 12 type C2 fractures. Using the Jupiter criteria, 6 elbows were rated as excellent, 9 good and 4 fair. Complications included late ulnar neuritis in one patient and wound infection in another patient

Handedness as a predictor of increased risk of knee, elbow, or shoulder injury, fractures and broken bones.

Several recent studies suggest that left-handers have higher susceptibility to accidental injury. Two studies investigated the suggested link between sinistrality and accidental injury by assessing the prevalence of joint problems, bone breaks, and fractures, which may be the sequelae of such mishaps. Study 1 used retrospective responses on medical history checklists completed by 1064 males on active duty with the United States Air Force. It found that left-handers were at significantly higher risk for knee problems (RR = 2.74), elbow and shoulder problems (RR = 3.03), and also swollen and pai

The 'floating elbow' in children. Simultaneous supracondylar fractures of the humerus and of the forearm in the same upper limb.

During a six-year period we prospectively studied eight children who presented with supracondylar fractures of the humerus and of the forearm on the same side. They were treated by prompt closed reduction, percutaneous fixation with Kirschner wires, and appropriate management of neurovascular and soft-tissue injuries. The results were assessed clinically and radiographically at a minimum of 12 months after injury. According to a clinical scoring system they were acceptable in seven children and poor in one.

Can total elbow arthroplasty effectively treat post-traumatic sequelae after proximal ulna or radius fracture and fracture dislocations?

BACKGROUND: In the United States, more than 50% of total elbow arthroplasties (TEAs) are performed for acute trauma or post-traumatic sequelae. Outcomes of TEA performed for distal humerus fracture or nonunion have been well-documented; however, less is known regarding outcomes of TEA for sequelae of complex proximal ulna or radius fractures. This study aimed to (1) identify operative considerations and outcomes and (2) determine TEA survivorship when performed for post-traumatic sequalae after proximal ulna or radius fracture or fracture-dislocation. MATERIALS AND METHODS: Patients who under

Elbow hemiarthroplasty and total elbow arthroplasty provided a similar functional outcome for unreconstructable distal humeral fractures in patients aged 60 years or older: a multicenter randomized controlled trial.

BACKGROUND: Semiconstrained total elbow arthroplasty (TEA) is an established treatment for elderly patients with distal humeral fractures not amenable to stable internal fixation (unreconstructable). In recent years, there has been increasing interest in elbow hemiarthroplasty (EHA), a treatment option which does not entail restrictions on weight-bearing as opposed to TEA. These 2 treatments have not been compared in a randomized controlled trial (RCT). The aim of this study was to compare the functional outcome of EHA and TEA for the treatment of unreconstructable distal humeral fractures in

Percutaneous pinning through the elbow joint versus cross-pinning for paediatric supracondylar humeral fractures: a retrospective analysis.

PURPOSE: Supracondylar humeral fractures are common in children and require early surgical intervention to prevent complications. Although percutaneous pinning is a standard treatment, the optimal pinning technique remains controversial. Therefore, this study aimed to compare the outcomes of lateral and through-the-elbow joint pinning (group J) and cross-pinning (group C) for paediatric supracondylar humeral fractures. METHODS: This retrospective analysis included paediatric patients with supracondylar humeral fractures treated at two trauma centres. Group J underwent lateral and through-the-

Does an elbow arthrogram change management after closed reduction of mildly displaced lateral condyle fractures in children?

The purpose of this study was to investigate whether an elbow arthrogram after closed reduction and percutaneous fixation of lateral condyle fractures results in a change in surgical management. This is a retrospective review of lateral condyle fractures managed with closed reduction and percutaneous fixation followed by an elbow arthrogram at our institution between 2008 and 2019. Chart and radiographic review was performed and operative notes were reviewed to determine rates of subsequent intervention after arthrogram. Descriptive statistics were used to summarize the data. This study includ

Identifying Risk Factors for Elbow Injury in Children Presenting to the Emergency Department With Distal Forearm Fractures.

BACKGROUND: Pediatric distal forearm fractures are one of the most common injuries seen in the emergency department. Orthopedic teaching instructs providers to radiograph the joint above and the joint below the fracture site for concurrent injury. Despite this teaching, there is little evidence to suggest that elbow injuries are common in children with distal forearm fractures. Similarly, there are few data that help the clinician predict which children are at higher risk for co-injury. METHODS: We conducted a cross-sectional analysis of the 2011 National Emergency Department Sample data set.

The outcome of peri-operative humeral condylar fractures after total elbow replacement in patients with rheumatoid arthritis.

We compared the outcome of peri-operative humeral condylar fractures in patients undergoing a Coonrad-Morrey semiconstrained total elbow replacement with that of patients with rheumatoid arthritis undergoing the same procedure without fractures. In a consecutive series of 40 elbows in 33 patients, 13 elbows had a fracture in either condyle peri-operatively, and 27 elbows were intact. The fractured condyle was either fixed internally or excised. We found no statistical difference in the patients' background, such as age, length of follow-up, immobilisation period, Larsen's radiological grade, o
