History of electromyography and nerve conduction studies: A tribute to the founding fathers.

The early development of nerve conduction studies (NCS) and electromyography (EMG) was linked to the discovery of electricity. This relationship had been concluded by observing the effect of applying electricity to the body of an animal and discovering that nerves and muscles themselves could produce electricity. We attempt to review the historical evolution of NCS and EMG over the last three centuries by reviewing the landmark publications of Galvani, Adrian, Denny-Brown, Larrabee, and Lambert. In 1771, Galvani showed that electrical stimulation of animal muscle tissue produced contraction an…

Electromyography and muscle biopsy in chronic isolated Myalgia: A prospective study.

INTRODUCTION: Chronic muscle pain affects approximately 10% of the general population. It is unknown whether electromyographic (EMG) examination or muscle biopsy is most useful in this situation. METHODS: In a 7-year, single-center prospective study we investigated the EMG and muscle pathology characteristics of patients with: (1) diffuse muscle pain lasting >1 year; (2) normal clinical examination; (3) normal extensive blood tests; and (4) no evidence of a coexisting disorder. RESULTS: One hundred eighty consecutive patients were enrolled. In 178 patients (99%), EMG studies and muscle biops

Surface Electromyography of the Forearm Musculature During the Windmill Softball Pitch.

BACKGROUND: Previous studies investigating the windmill softball pitch have focused primarily on shoulder musculature and function, collecting limited data on elbow and forearm musculature. Little information is available in the literature regarding the forearm. This study documents forearm muscle electromyographic (EMG) activity that has not been previously published. PURPOSE: Elbow and upper extremity overuse injuries are on the rise in fast-pitch softball pitchers. This study attempts to describe forearm muscle activity in softball pitchers during the windmill softball pitch. Overuse injur

Frequency study of facial electromyography signals with respect to emotion recognition.

Emotional intelligence is one of the key research areas in human-computer interaction. This paper reports the development of an emotion recognition system using facial electromyogram (EMG) signals focusing the ambiguity on the frequency ranges used by different research works. The six emotional states (happiness, sadness, fear, surprise, disgust, and neutral) were elicited in 60 subjects using audio visual stimuli. Statistical features were extracted from the signals at high, medium, low, and very low frequency levels. They were then classified using four classifiers - naïve Bayes, regression

Preoperative donor nerve electromyography as a predictor of nerve transfer outcomes.

PURPOSE: We hypothesized that health of the donor nerve and corresponding muscle, as assessed by electromyography (EMG), could predict the outcome of nerve transfer surgery. METHODS: A retrospective review was performed to investigate outcomes of nerve transfers for elbow flexion and shoulder abduction. Motor strength was graded preoperatively and after a minimum 1-year follow-up. Preoperative EMG results were classified as functionally normal or affected based on motor unit recruitment pattern and correlated with follow-up motor strength and range of motion. RESULTS: Forty nerve transfers w…

Facial, lingual, and pharyngeal electromyography in infants with Pierre Robin sequence.

INTRODUCTION: We evaluated the role of electromyography (EMG) in assessing orofacial neurological dysfunction in 81 infants with Pierre Robin sequence (PRS). METHODS: Needle EMG of muscles of the face, tongue, and soft palate, and blink responses were recorded. A two-channel EMG recorded sucking and swallowing during bottle feeding. RESULTS: Neurogenic EMG signs were detected in facial or oral muscles in 17 of 24 associated PRS and 1 of 57 isolated PRS cases (P < 0.0001). Soft palate muscles showed low-amplitude traces in 41.4% of patients who required two surgical steps for cleft palate rep…

The role of laryngeal electromyography in the diagnosis of vocal fold immobility in children.

INTRODUCTION: Pathology may affect the muscles that control vocal function directly by affecting peripheral function or indirectly by affecting the central nervous system. Clinically, muscle function can be assessed by observing the movements of the structures themselves or by recording the electrical activity of the muscles (electromyography, EMG). Since EMG is an invasive technique, it has enjoyed limited use in the diagnosis and management of voice disorders, especially in children. Laryngeal EMG may be helpful in those patients with voice problems of suspected neurological or neuromuscular…

Electromyography and nerve conduction studies of the lower extremity: uses and limitations.

Electrodiagnostic testing (EDX), the combination of electromyography and nerve conduction studies, is a useful extension of the physical exam. In the proper clinical context, it can help distinguish between the various nerve processes that may contribute to foot pain. This article discusses those processes, their clinical presentation, and the relative usefulness of EDX for each.

The logical choice of muscles for the needle-electromyography evaluation of cervical radiculopathy.

OBJECTIVE: To find the theoretically smallest subsets of muscles for needle-electromyography screening of cervical radiculopathies that meet or exceed the American Association of Electrodiagnostic Medicine (AAEM) guideline: ascertaining that each root and its adjacent roots are represented by at least 2 muscles each, innervated by those roots via different peripheral nerves. DESIGN: Twenty-two and 36 muscle sets and their myotomal innervation were derived from the literature, and rearranged into 15 and 23 unique muscle groups by root and peripheral nerve innervation. All 2 circumflex 15 and 2…

Diagnostic value of electromyography and muscle biopsy in arthrogryposis multiplex congenita.

Arthrogryposis multiplex congenita (AMC), a clinical syndrome characterized by multiple congenital joint contractures, frequently is caused by lesions in the peripheral nervous system. Two standard tests for the evaluation of the motor unit are nerve conduction studies/electromyography (NCS/EMG) and muscle biopsy. We reviewed the diagnostic value of these two studies in the evaluation of AMC over a 23-year period, analyzing 38 patients with AMC who had NCS/EMG, muscle biopsy, or both. Final diagnoses were classified as neurogenic (8 patients), myopathic (10 patients), "other" (12 patients), or…

[The importance of larynx electromyography for the differential diagnosis of vocal cord fixation].

Fourteen patients have been studied, with precedents of endotracheal intubation, suffering dysphonia and fixation of one vocal cord (13 cases) or both (1 case). The lack of neutrogen signs was confirmed through conventional electromyography, and objectivity because the clinical findings pointing to a direct traumatism of the vocal cord, the cricoarytenoid joint or an inflammatory origin of the damage. We support the limitation of electromyographic procedures of the larynx facing the conductive techniques and concede the possibility of the use of magnetic stimulation with this purpose.

A multifocal neurinoma of the hypoglossal nerve with motor paralysis confirmed by electromyography.

A rare case of neurinoma in a 72 year-old Japanese woman derived from the hypoglossal nerve is reported. The tumour was composed of three interconnected nodules occurring simultaneously in the left submandibular and sublingual regions. The lesion, which presented as a neck mass, caused a slight left-sided hemiparesis of the tongue with tongue deviation to the affected side not noticed by the patient. An electromyographic (EMG) study revealed decreased muscle activity on the left side of the tongue muscle, indicating dysfunction of the hypoglossal nerve. EMG was useful for diagnosis.

An electromyography study of wrist extension orthoses and upper-extremity function.

OBJECTIVE: This study examined the effect of commonly used long and short styles of commercially produced wrist extension orthoses on the activity of the proximal muscles of the shoulder and elbow and on wrist flexor and extensor muscle activity. METHOD: While 17 women between 22 and 40 years of age (M = 26.6) performed a specified movement wearing each of the two styles of orthosis and without an orthosis, their motor unit recruitment of five proximal joint muscle groups, wrist extensors, and wrist flexors was measured by surface electromyography. RESULTS: Motor unit recruitment was signifi…

Recording of single fiber electromyography in patients with peripheral facial palsy.

We determined muscle fiber density with single fiber electromyography (SFEMG) to discuss the condition of re-innervation after the recovery of facial movement in cases of peripheral facial palsy. Muscle fiber density was then compared with electroneurography (ENoG) which is thought to correlate with the severity of facial neuropathy in the early stage after the onset. The degree of neuropathy and subsequent recovery of innervation was also discussed. The subjects were 36 patients with peripheral facial palsy (30 patients with Bell's palsy and 6 with Hunt's syndrome), treated at our hospital, i…

Circumvaginal surface electromyography in women with urinary incontinence and in healthy volunteers.

In this study we developed a probe for surface EMG investigation of the circumvaginal muscle function in women with various forms of urinary incontinence. 69 women were investigated, 20 healthy volunteers, 25 patients with stress incontinence, 11 with urge incontinence and 13 with a mixed type of incontinence. We found that the circumvaginal surface EMG activity at short lasting (2 sec) maximum contraction was significant lower in patients with incontinence compared to healthy volunteers. We also found that the ability to hold a maximum contraction during one minute was significant reduced in
