Biomarkers of Atrial Cardiopathy and Atrial Fibrillation Detection on Mobile Outpatient Continuous Telemetry After Embolic Stroke of Undetermined Source.

BACKGROUND: Biomarkers of atrial dysfunction or "cardiopathy" are associated with embolic stroke risk. However, it is unclear if this risk is mediated by undiagnosed paroxysmal atrial fibrillation or flutter (AF). We aim to determine whether atrial cardiopathy biomarkers predict AF on continuous heart-rhythm monitoring after embolic stroke of undetermined source (ESUS). METHODS: This was a single-center retrospective study including all patients with ESUS undergoing 30 days of ambulatory heart-rhythm monitoring to look for AF between January 1, 2013 and December 31, 2015. We reviewed medical …

Utility of Carotid Ultrasonography in Management of an Atypical High Vascular-Risk Patient with Recurrent Calcified Cerebral Embolic Stroke.

OBJECTIVE: To describe a case of recurrent calcified cerebral emboli (CCE)-related acute ischemic stroke (AIS) and the diagnostic utility of plaque morphology characterization on carotid ultrasound. BACKGROUND: CCE are a rare cause of AIS. CCE-related AIS has been previously reported only in high vascular-risk patients such as those with severe carotid stenosis, widespread atheromatous disease, or cardiac valvular disease. CCE-related AIS from a carotid origin has not been reported in patients without carotid stenosis. CASE: A 69-year-old man with no known medical history presented with hemi

Left atrial appendage morphology assessment for risk stratification of embolic stroke in patients with atrial fibrillation: A meta-analysis.

BACKGROUND: Thromboembolic event (TE) risk stratification is performed by using CHA2DS2VASc score. It has been suggested that left atrial appendage (LAA) morphology independently influences TE risk in patients with nonvalvular atrial fibrillation. LAA morphology has been classified into 4 types: chicken wing, cauliflower, windsock, and cactus. OBJECTIVE: The purpose of this study was to determine TE risk for each LAA morphology in patients with atrial fibrillation with low to intermediate TE risk. METHODS: A systematic review of MEDLINE, Cochrane Library, and Embase for studies that used com

Embolic stroke due to a left atrial thrombus two years after placement of an atrial septal defect closure device.

A 29-year-old woman who had undergone closure of a secundum-type atrial septal defect using the Amplatzer device 2 years previously developed acute and progressive altered mentation. Initial clinical and imaging data confirmed the diagnosis of multiple cerebral, cerebellar and brain stem emboli, and infarcts. She was treated urgently with an intra-arterial thrombolytic agent with only minimal improvement. Transesophageal echocardiography revealed a large thrombus attached to the left atrial disc of the atrial septal defect occluder, which was the source of the emboli. In conclusion, this is th

Membranous Interventricular Septal Aneurysm (MVSA) as a rare potential cause of embolic stroke: a case report and literature review.

INTRODUCTION: Membranous interventricular septum aneurysm is an infrequent source of cardioembolic stroke with only a few cases reported to date. METHODS: We present a case of recurrent ischaemic stroke in a patient with Membranous Interventricular Septal Aneurysm (MVSA) in whom no alternative embolic sources have been identified despite an exhaustive diagnostic work-up. We consequently conducted a comprehensive literature review of similar stroke cases to characterize their clinical and radiological features. RESULTS: We report the case of a 42-year-old woman with recurrent cortical ischaem…

Carotid web in patients with embolic stroke of undetermined source undergoing endovascular therapy for anterior circulation large vessel occlusion.

OBJECTIVE: Carotid web (CaW) is an emerging cause of embolic stroke of undetermined source (ESUS), yet its prevalence and clinical implications among ESUS patients presenting with anterior circulation intracranial large vessel occlusion (LVO) treated with endovascular therapy (EVT) remain unclear. METHODS: We conducted a multicenter retrospective cohort study across three tertiary stroke centers, consecutively enrolling adults with acute ischemic stroke due to anterior circulation intracranial LVO (M1/M2) who underwent EVT between January 2017 and November 2025. ESUS was diagnosed after stand…

Predicting the Propensity of Atrial Cardiopathy and Paroxysmal Atrial Fibrillation in Patients with Embolic Stroke of Undetermined Source (ESUS).

BACKGROUND: There is still a significant population of patients with embolic stroke of Undetermined Source (ESUS) whose specific attributable cause of the stroke remains unknown. OBJECTIVES: Our research aimed to assess clinical, electrocardiogram, laboratory, and echocardiographic parameters that may predict the propensity of paroxysmal atrial fibrillation (PAF). METHODS: We enrolled seventy-five ESUS patients who were in sinus rhythm at the time of stroke diagnosis to undergo in-hospital 7-day Holter monitoring, testing for Pro-BNP, and a standard echocardiographic examination. For statist…

Embolic Stroke as a Complication of Streptococcus gordonii Prosthetic Valve Endocarditis in a Pacemaker Carrier: First Case From Lebanon.

In this article, we report the case of a 57-year-old hypertensive man with a prosthetic valve and a pacemaker who presented after many hours of onset of left-sided hemiplegia, left facial drop and dysarthria. Computed tomography of the brain revealed an ischemic stroke, and after an extensive workup the diagnosis of an embolic stroke induced by a Streptococcus gordonii endocarditis was established. This organism is considered to be a rare cause of endocarditis as only a dozen cases were previously reported in the literature. We describe the clinical interventions used, expose the diagnostic ap…

Isolated Pulmonary Arteriovenous Fistula-Related Embolic Stroke With Large Vessel Occlusions Mimicking Artery-to-Artery Embolism: A Case Report.

Isolated pulmonary arteriovenous fistula (PAVF) leading to paradoxical embolism and stroke is rare, particularly in cases involving large vessel occlusions. Here, we present the case of a 69-year-old female with occlusion of the M2 segment of the middle cerebral artery (MCA) and stenosis of the common carotid artery (CCA) caused by PAVF, which mimicked artery-to-artery embolism. CT angiography revealed occlusion of the left M2 segment of the MCA and stenosis of the CCA. After administration of recombinant tissue plasminogen activator, both the left MCA occlusion and CCA stenosis were completel

Impact of Cerebral Embolic Protection Devices on Disabling Stroke After TAVR: Updated Results From the STS/ACC TVT Registry.

BACKGROUND: Cerebral embolic protection devices (EPDs) were developed to mitigate the risk of stroke during transcatheter aortic valve replacement (TAVR), but their benefit remains unproven. In the PROTECTED-TAVR trial (Stroke Protection With Sentinel During Transcatheter), EPD use did not reduce periprocedural stroke (primary study outcome) but led to a 62% reduction in the secondary end point of disabling stroke. Given these results, the impact of EPDs during TAVR remains unclear. METHODS: We used STS/ACC TVT registry data to examine the association between EPD use and a proxy for disabling

The Role of Galectin-3 Levels for Predicting Paroxysmal Atrial Fibrillation in Patients with Embolic Stroke of Undetermined Source.

Background/Objectives: Paroxysmal atrial fibrillation (PAF) is an important cause that is thought main potential factor in Embolic stroke of undetermined source (ESUS). Extended Holter ECG is an expensive and time-consuming examination. It needs another tools for predicting PAF in ESUS patients. In this study, serum galectin-3 levels, ECG parameters (PR interval, P wave time and P wave peak time) LA volume index, LA global peak strain and atrial electromechanical conduction time values were investigated for predicting PAF. Methods: 150 patients with ESUS and 30 volunteers for the control group…

Experience and insights from a prolonged electrocardiographic monitorization with a wearable system after an embolic stroke of unknown source.

BACKGROUND: Embolic cerebrovascular events that remain of unknown etiology after a thorough diagnostic evaluation, are known as Embolic Strokes of Undetermined Source (ESUS). Subclinical atrial fibrillation (AF) represents a significant underlying cause of ESUS. Our aims were to examine the overall diagnostic yield of a prolonged cardiac monitoring wearable system (PCMw) after an ESUS to detect AF and factors associated with it, including the time frame from the ESUS event to PCMw initiation. Additionally, to evaluate the frequency of unexpected arrhythmic events (UAE) and their prognostic imp…

The macrosphere model-an embolic stroke model for studying the pathophysiology of focal cerebral ischemia in a translational approach.

The main challenge of stroke research is to translate promising experimental findings from the bench to the bedside. Many suggestions have been made how to achieve this goal, identifying the need for appropriate experimental animal models as one key issue. We here discuss the macrosphere model of focal cerebral ischemia in the rat, which closely resembles the pathophysiology of human stroke both in its acute and chronic phase. Key pathophysiological processes such as brain edema, cortical spreading depolarizations (CSD), neuroinflammation, and stem cell-mediated regeneration are observed in th

Hand Knob Stroke Due to an Embolism of a Metastatic Tumor in the Left Atrium.

A stroke confined to the precentral hand knob is uncommon and often mistaken for peripheral neuropathy. Embolic mechanisms are common, most frequently arising from large-artery atherosclerosis, but unusual sources may occur. We describe the case of a 68-year-old man with weakness restricted to the ulnar-sided fingers due to a small infarct in the medial precentral hand knob. Cervical vascular imaging was unremarkable, but echocardiography revealed a highly mobile left atrial mass arising from the ostium of the left upper pulmonary vein. The patient had a history of sublingual gland adenoid cys

Cholesterol Crystal Embolism Following Mechanical Thrombectomy for Acute Ischemic Stroke.

Cholesterol crystal embolism (CCE) is a rare but serious complication of atherosclerotic plaque rupture, often occurring after endovascular interventions. We report the case of a 73-year-old man who developed CCE following mechanical thrombectomy (MT) for an acute ischemic stroke (AIS) due to left internal carotid artery occlusion. The patient, with a history of hypertension and hyperlipidemia, underwent successful MT with complete recanalization. However, four weeks after the procedure, he presented with blue toe syndrome and acute kidney injury. Histopathological analysis of a skin biopsy co
