Does Acceptance of the Relational Reframe Predict Adolescent Depression and Suicidal Ideation Outcomes?

Attachment-based family therapy (ABFT) is an evidence-based systemic treatment for adolescent depression and suicidality. The first session focuses on shifting therapy goals from behavior management to relational repair. This study explores whether making this shift is associated with adolescent treatment outcomes. First-session recordings from an ABFT randomized controlled trial were coded for shifts in content (from an individual to a relational understanding of symptoms) and affect (from defensive to vulnerable emotions or hopefulness). Greater maternal responsivity to the relational refram

Improving Outcomes for Work-Related Concussions: A Mental Health Screening and Brief Therapy Model.

OBJECTIVE: This study assessed the efficacy of a neurocognitive screening evaluation and brief therapy model to improve RTW outcomes for workers who experienced mild head injuries. METHODS: Patients referred were evaluated using a neurocognitive and psychological screening battery. Work-focused cognitive behavioral therapy was provided when appropriate, addressing the role of negative emotional adjustment and functional sleep disturbance in prolonging recovery. RESULTS: Average time to RTW was 7 weeks post-evaluation, despite workers being off an average of 10 months between injury and refer

Sildenafil for Pulmonary Arterial Hypertension.

Pulmonary hypertension is a life-threatening illness with debilitating physical and emotional consequences. The progression of this devastating disease is characterized by a continuous increase in pulmonary vascular resistance, which results in elevated pulmonary artery pressure and leads to right heart failure. Treatment is focused on targeting the underlying complex etiology via the endothelin, prostacyclin, and nitric oxide (NO) pathways. Emergence of new treatments over the past 2 decades has led to improvement in the functional status and time to clinical worsening. Even with recent advan

Psychological interventions and mental health outcomes among caregivers of children with intellectual disability: a systematic review and meta-analysis.

INTRODUCTION: Mothers and caregivers of children with intellectual and developmental disabilities often experience depression, anxiety, parenting stress, emotional exhaustion, and reduced psychological wellbeing because of prolonged caregiving demands, behavioral difficulties, financial strain, and social stigma. Psychological and behavioral interventions have increasingly been introduced to improve caregiver mental health outcomes. This study examined the effectiveness of these interventions on the mental health of mothers and caregivers of children with intellectual and developmental disabil

DS-CNN: Deep Convolutional Neural Networks for Facial Emotion Detection in Children with Down Syndrome during Dolphin-Assisted Therapy.

In Mexico, according to data from the General Directorate of Health Information (2018), there is an annual incidence of 689 newborns with Trisomy 21, well-known as Down Syndrome. Worldwide, this incidence is estimated between 1 in every 1000 newborns, approximately. That is why this work focuses on the detection and analysis of facial emotions in children with Down Syndrome in order to predict their emotions throughout a dolphin-assisted therapy. In this work, two databases are used: Exploratory Data Analysis, with a total of 20,214 images, and the Down's Syndrome Dataset database, with 1445 i

AN OPEN TRIAL OF EMOTION REGULATION THERAPY FOR GENERALIZED ANXIETY DISORDER AND COOCCURRING DEPRESSION.

BACKGROUND: Although CBT is efficacious for a wide variety of psychiatric conditions, relatively fewer GAD patients achieve high endstate functioning as compared to patients receiving CBTs for other disorders. Moreover, GAD trials that utilized patient samples without prominent depression have tended to report that effect sizes for depressive outcomes were small or diminished to pretreatment levels in the follow-up period. Emotion regulation therapy (ERT) integrates principles from traditional and contemporary cognitive behavioral treatments with basic and translational findings from affect sc

EmotionaL and Language Recovery in Aphasia (ELLA): Codesigning and Proof-of-Concept Testing a Novel Intervention.

PURPOSE: People with aphasia are at risk of becoming depressed, anxious, and isolated. They want therapy to focus on improving communication, confidence, and finding ways to live well; family members want to be involved in the therapy process. We aimed to codesign and test a novel intervention (EmotionaL and Language recovery in Aphasia [ELLA]) that would address these priorities through integrating two evidence-based therapy approaches: elaborated semantic feature analysis (SFA), a naming therapy, and solution-focused brief therapy (SFBT), a psychological therapy. METHOD: A series of worksho

Exposed and embedded corrections in aphasia therapy: issues of voice and identity.

BACKGROUND: Because communication after the onset of aphasia can be fraught with errors, therapist corrections are pervasive in therapy for aphasia. Although corrections are designed to improve the accuracy of communication, some corrections can have social and emotional consequences during interactions. That is, exposure of errors can potentially silence the 'voice' of a speaker by orienting to an utterance as unacceptable. Although corrections can marginalize speakers with aphasia, the practice has not been widely investigated. AIMS: A qualitative study of corrections during aphasia therapy

Changes in perceived psychosocial working conditions after psychotherapeutic consultation at work - exploratory post-hoc analyses from a randomized controlled trial.

BACKGROUND: Psychotherapeutic consultation at work (PT-A) is a low-threshold intervention providing early work-focused treatment to employees with symptoms of common mental disorders (CMD). This post-hoc analysis explores whether PT-A, compared with routine care, is associated with perceived reductions in job demands (i.e., quantitative and emotional demands, dissolution between work and private life) and improvements in job resources (i.e., decision authority, development opportunities, social support). METHODS: The exploratory post-hoc analysis used data from a multicenter randomized contro

The Motor Basis for Misophonia.

Misophonia is a common disorder characterized by the experience of strong negative emotions of anger and anxiety in response to certain everyday sounds, such as those generated by other people eating, drinking, and breathing. The commonplace nature of these "trigger" sounds makes misophonia a devastating disorder for sufferers and their families. How such innocuous sounds trigger this response is unknown. Since most trigger sounds are generated by orofacial movements (e.g., chewing) in others, we hypothesized that the mirror neuron system related to orofacial movements could underlie misophoni

The Role of Affect Management for HIV Risk Reduction for Youth in Alternative Schools.

OBJECTIVE: Adolescents in alternative schools for behavioral and emotional problems have an earlier sexual onset and higher rates of sexual risk than their peers. They also often have difficulty managing strong emotions, which can impair sexual decision making. Human immunodeficiency virus (HIV) prevention programs for these adolescents may be most effective if skills for coping with strong emotions during sexual situations are included. METHOD: This article reports the 6-month outcomes of a three-arm randomized controlled trial comparing an HIV prevention intervention with affect management

Time to wake up to the potential benefits of targeting sleep in osteoarthritis management.

Sleep disturbance is highly prevalent in people with osteoarthritis (OA) yet it is rarely considered in routine care. Emerging evidence suggests that sleep disturbance contributes to heightened musculoskeletal pain and poorer physical and emotional outcomes in those with OA. This narrative review synthesises current knowledge on the epidemiology and impact of sleep disturbances in OA, including their associations with and impact on fatigue, depressive symptoms, pain catastrophising, and negative mood. It examines the effects of core OA treatments such as exercise, diet and weight loss on sleep

Can kisspeptin be a new treatment for sexual dysfunction?

The neuropeptide kisspeptin activates the hypothalamic-pituitary-gonadal (HPG) axis and influences neural circuits controlling sexual behavior. Animal studies have determined its sex-specific roles in reproductive behaviors, whereas human research has linked kisspeptin to increased brain activity in regions associated with sexual and emotional processing, making it a potential treatment for disorders of sexual desire. Here I discuss the current evidence on the promise of kisspeptin as a therapy for sexual dysfunction, highlight the challenges currently hindering its application, and advocate f

Collaboration in cognitive-behavioral therapy.

Collaborative empiricism, which involves a systemic process of therapist and patient working together to establish common goals in treatment, has been found to be one of the primary change agents in cognitive-behavioral therapy (CBT). This article focuses on the development of a therapeutic relationship and implementation of collaborative empiricism along with the elements that lead to success in treatment. This method is used to uncover patients' automatic thoughts and underlying beliefs in treating an array of emotional and behavioral disorders. The role of the therapist is discussed in deve

Delta-Beta Coupling in Adolescents with Depression: A Preliminary Examination of Associations with Age, Symptoms, and Treatment Outcomes.

Cognitive behavioral therapy (CBT) is effective in treating adolescent depressive symptoms but with variable treatment response. Heterogeneity in treatment outcome may be due in part to individual differences in cognitive and emotional processes in depressed adolescents, and there is a need to identify biomarkers associated with symptoms and treatment outcomes. There has been growing interest in leveraging electroencephalography (EEG) data to examine correspondence between multiple frequency bands, and delta-beta coupling in particular is thought to underlie emotion regulation and offers a pro

Multiple Chemical Sensitivity: A Clinical Perspective.

OBJECTIVE: The etiology of multiple chemical sensitivity (MCS) is still debated, which is an obstacle to assessing treatment options. An analysis of the scientific literature combined with the clinical experience can suggest some avenues. METHODS: The etiology of MCS and its underlying mechanisms were reviewed from the scientific literature to identify the main factors contributing to its development. The results of the studies involving biomarkers and cerebral imaging techniques on MCS subjects were compared with those performed on subjects having the comorbidities of MCS. From the scientifi

Systems Training for Emotional Predictability and Problem Solving for borderline personality disorder: A systematic review.

Systems Training for Emotional Predictability and Problem Solving (STEPPS) is a group treatment program for patients with borderline personality disorder (BPD). The program was intended to be highly accessible, both for patients and therapists. During STEPPS, patients are taught emotion regulation and behavior management skills. This systematic review synthesizes the current empirical status of STEPPS, focusing on research designs, quality of studies, target groups, protocols, and outcome. We selected 20 studies, with three randomized controlled trials. Patients with BPD, subthreshold BPD, and

When the Beat Breaks the Heart: A Case of Arrhythmia-Induced Mid-ventricular Takotsubo Cardiomyopathy.

Takotsubo cardiomyopathy (TTC) is a transient and reversible form of left ventricular systolic dysfunction that often mimics acute coronary syndrome and is usually precipitated by emotional or physical stress. We describe an unusual presentation of mid-ventricular TTC in which ventricular arrhythmia was the dominant clinical feature and a probable precipitating factor. A 71-year-old woman presented with left shoulder pain and palpitations in the absence of chest pain or an identifiable emotional stressor. Initial electrocardiography demonstrated frequent runs of rapid non-sustained ventricular
