Recurrent peripheral vestibulopathy: Is MRI useful for the diagnosis of endolymphatic hydrops in clinical practice?

OBJECTIVES: Recurrent peripheral vestibulopathy (RPV) is a public health problem, yet the aetiology remains unclear. Recent developments in MRI of endolymphatic hydrops (EH) allow for a better understanding of inner ear disorders. We intended to study the prevalence of EH in patients with RPV, in comparison to those with Meniere's disease (MD). METHODS: MRI examinations were performed 4 hours after intravenous injection of gadoteric acid in 132 patients with RPV (n = 64) and MD (n = 68). Two radiologists retrospectively studied the prevalence and localization of EH in RPV and MD groups. Patie…

Characterization of neuronal cell death in the spiral ganglia of a mouse model of endolymphatic hydrops.

HYPOTHESIS: Spiral ganglion neurons (SGN) in the Phex male mouse, a murine model of postnatal endolymphatic hydrops (ELH) undergo progressive deterioration reminiscent of human and other animal models of ELH with features suggesting apoptosis as an important mechanism. BACKGROUND: Histologic analysis of the mutant's cochlea demonstrates ELH by postnatal Day (P) 21 and SGN loss by P90. The SGN loss seems to occur in a consistent topographic pattern beginning at the cochlear apex. METHODS: SGN were counted at P60, P90, and P120. Semiquantitative reverse transcriptase-polymerase chain reaction …

Use of electrocochleography for assessing endolymphatic hydrops in patients with Lyme disease and Ménière's disease.

From an otological standpoint, Lyme disease can manifest itself as Ménière's disease both clinically and electrophysiologically. The aim of this study was to describe the findings of routine clinical, auditory and vestibular tests in patients with Ménière's and Lyme disease and to use electrocochleography (ECoG) to assess the presence of endolymphatic hydrops (EH) in both diseases. Transtympanic ECoG was performed in 91 patients with Ménière's disease and in 11 patients with confirmed Lyme disease. In both diseases the majority of patients had more than one complaint. There was one case with i…

Intraoperative electrocochleographic monitoring of inner ear surgery for endolymphatic hydrops. A review of cases.

The purpose of this review of cases was to determine whether or not the SP/AP amplitude ratio changes during unidirectional inner ear valved shunt surgery for the decompression of the hydropic labyrinth in Meniere's disease. A series of 62 patients underwent shunt surgery over a 20 month period. In 43 cases (43 ears), ECoG responses were successfully recorded throughout the duration of surgery. A pair of two-tailed paired t-tests were computed for SP/AP amplitude ratios acquired during intraoperative ECoG monitoring at the baseline and closing stage of surgery. The first t-test compared the me

Localization, frequency, and severity of endolymphatic hydrops and the pathology of the labyrinthine membrane in Menière's disease.

The localization, frequency, and severity of endolymphatic hydrops in 22 temporal bones of 16 individuals with Meniere's disease were studied histopathologically. Endolymphatic hydrops was more often observed in the pars inferior (22/22) than in the pars superior (13/22) of the temporal bone, and severe hydrops was observed most frequently in the saccule, followed by the cochlea, the utricle, and the three semicircular canals. In the cochlea, the most severe hydrops was observed in the apical turn, followed by the hook portion, the middle turn, and the basal turn. Clinically interesting observ…

Changes of the action potential, the summating potential and cochlear microphonics in experimental endolymphatic hydrops.

The changes of the action potential (AP), the summating potential (SP) and cochlear microphonics (CM) were investigated in reference to the grade of endolymphatic hydrops produced by endolymphatic sac obliteration in guinea pigs. In a few cases with minimum endolymphatic hydrops, super-normal AP was obtained. With an increase of endolymphatic hydrops, CM responses were reduced although AP responses at intensive stimulation were maintained relatively well. It is thought that the polarity of positive SP, as observed in control, was influenced to deflect towards negative SP by an interaction betw…

Endolymphatic hydrops and its association with Magnetic Resonance Imaging and serum vitamin D levels.

OBJECTIVE(S): Endolymphatic hydrops is associated with factors such as autoimmune responses and vascular issues. Vitamin D plays a vital role in immune regulation and is associated with various inflammatory conditions. However, evidence regarding the correlation between endolymphatic hydrops and vitamin D is lacking. We clarified the correlation between vitamin D and endolymphatic hydrops to explain clinical symptoms and intervene in endolymphatic hydrops formation and progression. METHODS: Intravenous gadolinium-enhanced delayed magnetic resonance imaging three-dimensional-fluid-attenuated i…

Enhancement of subarachnoid space during magnetic resonance imaging of endolymphatic hydrops: a case report.

Enhancement of the subarachnoid space after intravenous administration of gadolinium contrast agent is not common. Enhancement usually occurs in pathological conditions that increase the permeability of the blood-cerebrospinal fluid barrier, most notably in meningitis. We herein describe possible subarachnoid enhancement in patients with no apparent effect on the meninges. These patients had clinical signs of Meniere's disease and underwent specific magnetic resonance imaging of the inner ear to possibly visualize endolymphatic hydrops. The endolymphatic space can be noninvasively imaged by in…

Longitudinal up-regulation of endolymphatic hydrops in patients with Meniere's disease during medical treatment.

OBJECTIVE/HYPOTHESIS: Meniere's disease (MD) is a common inner ear disease characterized by repeated episodic vertigo, fluctuating sensorineural hearing loss, and tinnitus. Its pathology is defined as endolymphatic hydrops (EH) in the inner ear and EH has been hypothesized to correlate with the clinical symptoms of MD. We presented the dynamics of in vivo EH in MD patients during medical treatments. STUDY DESIGN: Prospective, single-arm repeated measures. METHODS: Eleven MD patients were enrolled. All subjects prospectively underwent gadolinium-enhanced inner ear magnetic resonance (MR) imag…

[Changes in vestibular function and furosemide test after experimental endolymphatic hydrops in guinea pigs].

The present study reports a successful anial model of endolymphatic hydrops (EH) in guinea pigs (n = 48) through a surgical obliteration of the endolymphatic sac and duct. Electronystagmography was calibrated by optokinetic nystagmus. During the formation of EH the vestibular functions (n = 16) were dynamically observed by ratation test. After eight weeks (n = 25) the furosemide tests with different doses were carried out by sinusoidal angular acceleration test. The results showed that the excitability of vestibular function in the operated ears was enhanced for the first week after operation,…

Time-related changes in cochlear potentials in guinea pigs with experimentally induced endolymphatic hydrops.

In order to examine the changes in cochlear function occurring in hydropic ears over time, endolymphatic hydrops was provoked by obliterating the endolymphatic sac in 62 albino guinea pigs. Cochlear potentials (EP, CM, SP and AP) were recorded in the 1st, 2nd, 4th and 12th postoperative weeks, respectively. A significant reduction in the level of EP was already observed in the 2nd week and the potential was further suppressed by the 4th week. There was no significant difference between the levels of the 4th and 12th weeks. The amplitudes of CM and AP decreased progressively as time elapsed aft…

Characterizing Differences in Endolymphatic Hydrops Signatures Among Meniere's Disease Patients with and Without Migraine.

Background/Objectives: Migraine is frequently comorbid with Meniere's disease, which may complicate interpretation of inner ear imaging and clinical diagnosis. While endolymphatic hydrops has been studied in Meniere's disease and vestibular migraine separately, comparative imaging data for Meniere's disease patients with and without migraine remain limited. Methods: We retrospectively analyzed 78 patients with definite Meniere's disease who underwent endolymphatic contrast-enhanced MRI (HYbriD of Reversed image of Positive endolymph signal and native image of positive perilymph signal; or "HYD…

[A case report of middle ear cholesteatoma complicated with labyrinthine fistulaand delayed endolymphatic hydrops].

Delayed endolymphatic hydrops (DEH) is a rare disease that causes vertigo and is often misdiagnosed as other vertigo diseases. This article reports on a patient with vertigo who was easily misdiagnosed. The patient was a middle ear cholesteatoma complicated by labyrinthine fistula (LF); however, his vertigo was episodic vertigo, which could not be explained solely by LF causing labyrinthitis. The possibility of endolymphatic hydrops was suspected, which was confirmed by inner ear magnetic resonance gadolinium imaging. This is the first reported case of middle ear cholesteatoma complicated by L…
