Current risk of iatrogenic Creutzfeld-Jakob disease in the UK: efficacy of available cleaning chemistries and reusability of neurosurgical instruments.

The initial cleaning of reusable surgical devices is critical to ensure the efficacy of the subsequent sterilisation process. Transmissible spongiform encephalopathies (TSEs) are incurable and fatal neurodegenerative diseases apparently transmitted simply by the absorption or ingestion of self-aggregating protease-resistant prions (PrP(Sc)), which are very resilient to most standard cleaning chemistries and heat-based decontamination techniques. Therefore there is a risk of iatrogenic transmission from reusable surgical devices if these are allowed to retain potentially infectious material aft

Transmission of hepatitis B virus among persons undergoing blood glucose monitoring in long-term-care facilities--Mississippi, North Carolina, and Los Angeles County, California, 2003-2004.

Regular monitoring of blood glucose levels is an important component of routine diabetes care. Capillary blood is typically sampled with the use of a fingerstick device and tested with a portable glucometer. Because of outbreaks of hepatitis B virus (HBV) infections associated with glucose monitoring, CDC and the Food and Drug Administration (FDA) have recommended since 1990 that fingerstick devices be restricted to individual use. This report describes three recent outbreaks of HBV infection among residents in long-term-care (LTC) facilities that were attributed to shared devices and other br

Impacting health care: study of a reusable urinary drainage system.

This study of a reusable urinary leg bag system was undertaken to ascertain possible increased incidence of urinary tract infection attributable to reusing urinary leg bags. The fourteen patients involved had diagnoses of "neurogenic bladder," and required long-term drainage. Ten were on Foley catheter drainage and four were on external drainage. Before the reusable bags were placed into use, urine cultures were collected to establish baseline information about prevalent urinary organisms for each subject. During the study period, random urine specimens for culture/sensitivity were collected.

Switching from intermittent catheterization with single-use catheter to a reusable catheter has a negative impact on quality of life.

PURPOSE: It has been proposed that reusable catheters are more cost effective and environmentally sustainable than single-use catheters intended for intermittent catheterization (IC). However, the aspect of individuals' well-being and preference for catheter type is not considered. In this study, we investigated the impact on individuals' health-related quality of life (HR-QoL) when testing a reusable catheter. MATERIALS AND METHODS: The study was an open-labeled, single-arm, multicenter investigation with a treatment period of 28 days. Forty subjects using single-use hydrophilic catheters we

N95® filtering facepiece respirator contamination with SARS-CoV-2 following reuse and extended use.

OBJECTIVE: During the COVID-19 pandemic, the United States Centers for Disease Control and Prevention provided strategies, such as extended use and reuse, to preserve N95 filtering facepiece respirators (FFR). We aimed to assess the prevalence of N95 FFR contamination with SARS-CoV-2 among healthcare personnel (HCP) in the Emergency Department (ED). DESIGN: Real-world, prospective, multicenter cohort study. N95 FFR contamination (primary outcome) was measured by real-time quantitative polymerase chain reaction. Multiple logistic regression was used to assess factors associated with contaminat

Decontamination of filtering facepiece respirators using a low-temperature-steam-2%-formaldehyde sterilization process during a pandemic: a safe alternative for re-use.

BACKGROUND: The coronavirus disease 2019 pandemic has caused problems with respirator supplies. Re-use may minimize the impact of the shortage, but requires the availability of an efficient and safe decontamination method. AIM: To determine whether low-temperature-steam-2%-formaldehyde (LTSF) sterilization is effective, preserves the properties of filtering facepiece (FFP) respirators and allows safe re-use. METHODS: Fourteen unused FFP2, FFP3 and N95 respirator models were subjected to two cycles of decontamination cycles. After the second cycle, each model was inspected visually and accumu

[Cost of tetanus toxoid injection using a jet-injector (Imule) in collective immunization in Senegal: comparison with injection using a syringe and resterilizable needle].

Needle-less jet injectors were developed by the US army after World War II. Their principal use, however, has been in the administration of lyophilized vaccines from multidose vials to at-risk populations in developing countries. In 1983, a hepatitis B epidemic occurred among customers of a beauty clinic in California (USA) following the use of jet-injectors, demonstrating a clear risk of cross-contamination associated with this technique. As a result, the WHO and Unicef stopped recommending jet-injectors for collective immunizations in developing countries. To eliminate the risk of contaminat

Acute hepatitis C virus infections attributed to unsafe injection practices at an endoscopy clinic--Nevada, 2007.

On January 2, 2008, the Nevada State Health Division (NSHD) contacted CDC concerning surveillance reports received by the Southern Nevada Health District (SNHD) regarding two persons recently diagnosed with acute hepatitis C. A third person with acute hepatitis C was reported the following day. This raised concerns about an outbreak because SNHD typically confirms four or fewer cases of acute hepatitis C per year. Initial inquiries found that all three persons with acute hepatitis C underwent procedures at the same endoscopy clinic (clinic A) within 35-90 days of illness onset. A joint investi

Pain is associated with reduced adherence to safer injection practices among people who inject opioids.

BACKGROUND: Injection opioid use is associated with fatal and non-fatal harms including accidental overdose and infectious diseases. Syringe service programs offer sterile injection equipment and safer injection training to reduce the incidence of injection-related harms. Nevertheless, factors that might reduce adherence to safer injection practices are understudied. This report explores pain as one such factor. METHODS: People who inject opioids (n = 130) were recruited from a syringe service program for a cross-sectional survey assessing the relationship between fluctuations in pain severit

Biomechanical and surface physico-chemical analyses of used osteosynthesis plates and screws--potential for reuse in developing countries?

Reprocessing of single-use devices is an upcoming issue in the Western world, but has been for many years in developing countries. In developing countries, the number of bone fractures due to traffic or industrial accidents is high. Patients often need an osteosynthesis with plates and screws, but most patients, however, cannot afford this because of the costs involved and have to rely on the application of used plates and screws. This study aims to determine whether used plates can be safely reused in another patient from a biomechanical, surface physico-chemical, and biological point-of-view

Microbiological evaluation of reused catheter guides in a Brazilian hospital.

Within Brazilian hospitals, as well as in hospitals in other countries, the safety of reusing single-use medical devices is being questioned. At the same time, there has been a constant, growing increase in the number of such reprocessed products. In an attempt to evaluate such procedures, we analyzed 30 catheter guide units that were reused four times in patients at a public hospital of the state of São Paulo, Brazil. After each use, the catheter guides had been sterilized with a mixture of ethylene oxide/chlorofluorocarbons (12:88). Each unit analyzed was provided by the hospital individuall

Physical effects of reuse and repeated ethylene oxide sterilization on transscleral cyclophotocoagulation laser G-probes.

PURPOSE: The authors documented the physical effects of reuse and repeated ethylene oxide sterilization on transscleral cyclophotocoagulation laser G-probes. METHODS: Transscleral cyclophotocoagulation was performed using G-probes on fresh porcine eyes. Each of two G-probes was used for four transscleral cyclophotocoagulation procedures, with three cycles of ethylene oxide sterilization in between. The power output from the G-probes was measured by a laser output meter before and after each transscleral cyclophotocoagulation procedure. The G-probes were also examined under a slit lamp for sig

Infections acquired in barbershops - A review.

Barbershops are important venues for men to get their hair and beards done and hence, play an important role in men's social life. But barbershops can also be a source of infections. The barber's profession brings the barber into direct contact with customers who may carry pathogens, and contaminated instruments or skin-to-skin contacts might transmit infectious agents. Since barbers work with non-sterile and reusable sharp objects, a simple nick might facilitate blood-borne infections. In our review article we summarize current knowledge regarding barbershop-acquired infections including tran

High-efficiency carbon-coated steel wool filter for controlling cooking-induced oil smoke.

Cooking oil smoke (COS) contains many harmful substances, such as particulate matter, formaldehyde, and phenyl esters. Currently, commercial COS treatment equipment is expensive and requires a large space. Furthermore, a large amount of agricultural waste is generated and is mainly burned onsite, producing large amounts of greenhouse gases and air pollutants. This waste could be reused as a precursor for biochar and activated carbon. Therefore, this research used saccharification and catalytic hydrothermal carbonization to process rice straw and produce compact carbon-based filters (steel wool

Glove Reuse Limits in Hair Dyeing: Permeation of p-Phenylenediamine and Aminophenols Through Natural Rubber Latex and Neoprene After Repeated Washing.

BACKGROUND: Hairdressers often reuse disposable gloves, yet the effect of repeated washing on protection against oxidative hair-dye sensitizers is unclear. OBJECTIVES: To determine how reuse and wash type influence the permeation of p-phenylenediamine (PPD) and aminophenols through natural-rubber-latex (NRL) and neoprene gloves. METHODS: Closed-loop ASTM F739 tests (27°C) were run for five consecutive 8-h exposures, each followed by either a water-only rinse or a detergent wash. Breakthrough time (BT), steady-state permeation rate (PR) and cumulative permeated mass (CPM) were quantified; lin
