PURPOSE: Lesch-Nyhan disease is an inherited metabolic disorder characterized by overproduction of uric acid and neurobehavioral abnormalities. The purpose of this study was to describe macrocytic erythrocytes as another common aspect of the phenotype.
METHODS: The results of 257 complete blood counts from 65 patients over a 23-year period were collected from 2 reference centers where many patients are seen regularly.

Twenty-nine juvenile, captive-reared African black-footed penguins (Spheniscus demersus) were hematologically monitored every 2 wk over the period of 24 wk during their first outdoor exposure. Blood samples taken from the penguins were screened for 12 blood evaluation parameters. Parasitemic penguins were medically treated. Eighteen birds (62.1%) experienced naturally acquired malaria and 11 birds (37.9%) remained nonparasitemic. A total of 32 avian malaria episodes were noted; 25 (78.1%) were identified as Plasmodium elongatum, 5 (15.6%) as Plasmodium relictum, and 2 (6.3%) as Plasmodium spp.

The influence of the spleen on the blood has been assessed in a series of 187 consecutive patients with chronic liver disease. Patients were described as having 'hypersplenism' if the white blood count and/or platelet count were below 4.0 X 10(9)/1 and 100 X 10(9)/1 respectively at the time of biopsy diagnosis and on at least one subsequent occasion. End-stage renal disease (ESRD) involves chronic microinflammation, oxidative stress, and increased red blood cell (RBC) destruction. This study examined their effects on erythrocyte integrity and oxidative stress in ESRD patients. Seventy ESRD patients were enrolled and received selenium (200 µg) and vitamin E (400 units) supplementation for 60 days. Phosphatidylserine exposure on RBCs was assessed to evaluate eryptosis. Serum levels of malondialdehyde (MDA), glutathione peroxidase (GPx) activity, ferritin, interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α), and iron (Fe) were measured.

To explore the functional significance of cGMP-dependent protein kinase type I (cGKI) in the regulation of erythrocyte survival, gene-targeted mice lacking cGKI were compared with their control littermates. A premature male infant with hyaline membrane disease received a 34 ml transfusion of packed erythrocytes from a CPDA1 blood unit on day 2 with no adverse consequences. On day 34 he was given a transfusion, and the platelet count decreased. Intravenous immune globulin therapy was initiated and the platelet count steadily increased. Human leukocyte (HLA) antigen and platelet antibody testing showed that the thrombocytopenia was due to passive transfer of platelet-specific antibody from the blood donor.

Terminal renal failure secondary to the hemolytic uremic syndrome (HUS) developed 18 months after initial transplantation and 4 1/2 months after the child received his second cadaveric renal allograft. Recurrence of the syndrome was evidenced by gross hematuria and hypertension after a 'flu-like' illness, sudden decrease in platelet count and hemoglobin, and erythrocyte fragmentation. Renal biopsy findings were compatible with HUS. Evidence is presented that the HUS was a recurrence of the original disease.
