Saralasin (1-sar-8-ala-angiotensin II), a competitive inhibitor of angiotensin II, was administered to 32 patients with renovascular or essential hypertension before (Day 1) and after (Day 2) they were mildly sodium depleted by furosemide (1 mg/kg body weight). A blood pressure lowering effect of saralasin was observed in 16 of 17 patients with renovascular hypertension on Day 2, but in only 10 of the 17 on Day 1. Of the 15 patients with essential hypertension, only the four with high renin levels exhibited a vasodepressor response on Day 2; three responded similarly on Day 1. The Average net …

The data presented establish that in early or mild essential hypertension there is a state of inappropriate hypermineralocorticoid activity represented by the sum of aldosterone and 18-hydroxy-11-deoxycorticosterone concentrations in the plasma. This disturbance, associated with a "normal" or excessive salt intake, would produce the arteriolar cationic changes in sodium, potassium, calcium, or magnesium leading to hypersensitivity or hyperresponsiveness of the arteriolar actomyosin to normal levels of circulating norepinephrine or angiotensin. The nature of the cationic changes in the arteriol…

Seventeen normal subjects and 21 patients with essential hypertension were examined by [131I]hippuran renography. Both groups were studied twice on the same day, first in a state of moderate dehydration and then after rehydration. From these data, four sets of reference intervals were established for several renographic parameters. Differences between the two groups were defined. The most impressive observation was a greater functional disparity between the two kidneys of one individual in the group of hypertensive patients, particularly when in a state of moderate volume depletion. This was r…

Patients with low-renin essential hypertension have certain features consistent with excessive mineralocorticoid activity. Because known mineralocorticoids are normal in the majority of low-renin essential hypertension patients, an unknown mineralocorticoid was sought in the urine of such patients. Urine extracts from patients with low-renin essential hypertension were assayed for mineralocorticoid activity in adrenalectomized rats and found to contain more such activity than could be accounted for by the known mineralocorticoids in the extracts. The factor responsible for the unexplained mine…

BACKGROUND: In experimental renovascular hypertension, aldosterone has been implicated in myocardial remodeling and fibrosis, but it is uncertain whether excess aldosterone effects left ventricular structure and function in hypertensive patients.
METHODS: Hypertensive patients from the Cardiovascular Center of the New York Presbyterian Hospital-Weill Cornell Medical Center in New York and the Russian Cardiovascular Research Institute, Moscow, Russia, were studied. The sample included 35 patients with primary aldosteronism and 35 controls with essential hypertension matched for age, gender, an…

To investigate the Ca-sensitivity of the erythrocyte membrane in hypertension, the changes of the osmotic fragility of erythrocytes by Ca-loading and the effects of Ca-channel blockers or calmodulin-antagonist were observed in patients with essential hypertension. Erythrocytes were obtained from untreated patients with essential hypertension and age-matched normotensive subjects. Treatment of erythrocytes with Ca-ionophore A23187 and Ca in bathing medium caused the reduction of the osmotic fragility of erythrocytes dose-dependently on Ca-concentration. The degree in alteration of the osmotic f…

MicroRNA-155 (miR-155) has emerged as a pivotal regulator in the development of hypertension. However, its role in the mechanism underlying hypertension remains incompletely defined, especially in vascular inflammation and dysfunction. This review synthesizes and evaluates the published literature on the role of miR-155 in the regulation of inflammation, endothelial dysfunction (ED), and vascular smooth muscle cells (VSMCs) function in essential hypertension (EH). We evaluated experimental and clinical studies investigating the expression patterns, molecular targets, and pathways involving miR…

Essential hypertension, a common multifactorial condition, is a significant public health concern. In India, the prevalence of hypertension is high, with estimates indicating a rising trend. Shirodhara, an Ayurvedic therapy, has exhibited the potential for stress reduction and relaxation. Thus, the review investigates the effect of Shirodhara on systolic and diastolic blood pressure and its impact on clinical symptoms like palpitation, insomnia, giddiness, fatigue, and headache in essential hypertensive patients. We searched PubMed, Embase, Scopus, Google Scholar, and Shodhganga, Web of Scienc…

INTRODUCTION: Hypertension is a major and modifiable risk factor for cardiovascular diseases. Essential, primary, or idiopathic hypertension accounts for 90-95% of all cases. Identifying novel biomarkers specific to essential hypertension may help in understanding pathophysiological pathways and developing personalized treatments. We tested whether the integration of circulating microRNAs (miRNAs) and clinical risk factors via machine learning modeling may provide useful information and novel tools for essential hypertension diagnosis and management.
MATERIALS AND METHODS: In total, 174 parti…

Many patients still have high blood pressure (BP) after treatment with angiotensin II type 1 (AT1) receptor blockers (ARBs). We compared the efficacy and safety of azilsartan to those of olmesartan in a prospective randomized clinical trial. Sixty-four hypertensive patients who were treated with ARBs other than azilsartan and olmesartan were enrolled in this study. We randomly assigned patients to changeover from their prior ARBs to either azilsartan or olmesartan, and followed the patients for 3 months. Systolic BP (SBP) in the azilsartan group was significantly decreased at 3 months, and dia

Relationships between plasma catecholamine responses, and levels and types of aggression in hyper- and normotensives were investigated by analyses of data obtained in a large psychophysiological experiment on 97 hypertensives (EH) and 98 normotensives (CO) each. Subjects were divided according to levels (high vs low) and types (repressed vs manifest) of aggressiveness according to self rating questionnaire scores. Their plasma catecholamine responses to defined stressors indicating sympathetic arousability were compared by four factor analyses of covariance adjusting for age. Repressed aggress
