Pre-post changes in main outcomes of medical rehabilitation in Germany: protocol of a systematic review and meta-analysis of individual participant and aggregated data.

INTRODUCTION: Multidisciplinary, complex rehabilitation interventions are an important part of the treatment of chronic diseases. However, little is known about the effectiveness of routine rehabilitation interventions within the German healthcare system. Due to the nature of the social insurance system in Germany, randomised controlled trials examining the effects of rehabilitation interventions are challenging to implement and scarcely accessible. Consequently, alternative pre-post designs can be employed to assess pre-post effects of medical rehabilitation programmes. We present a protocol

Thiamine dose response in human milk with supplementation among lactating women in Cambodia: study protocol for a double-blind, four-parallel arm randomised controlled trial.

INTRODUCTION: Thiamine (vitamin B1) deficiency remains a concern in Cambodia where women with low thiamine intake produce thiamine-poor milk, putting their breastfed infants at risk of impaired cognitive development and potentially fatal infantile beriberi. Thiamine fortification of salt is a potentially low-cost, passive means of combating thiamine deficiency; however, both the dose of thiamine required to optimise milk thiamine concentrations as well as usual salt intake of lactating women are unknown. METHODS AND ANALYSIS: In this community-based randomised controlled trial, 320 lactating

Diagnostic biomarkers for adult haemophagocytic lymphohistiocytosis in critically ill patients (HEMICU): a prospective observational study protocol.

INTRODUCTION: Haemophagocytic lymphohistiocytosis (HLH) in adults is characterised by toxic immune activation and a sepsis-like syndrome, leading to high numbers of undiagnosed cases and mortality rates of up to 68%. Early diagnosis and specific immune suppressive treatment are mandatory to avoid fatal outcome, but the diagnostic criteria (HLH-2004) are adopted from paediatric HLH and have not been validated in adults. Experimental studies suggest biomarkers to sufficiently diagnose HLH. However, biomarkers for the diagnosis of adult HLH have not yet been investigated. METHODS AND ANALYSIS: T

Treatment decisions for newborns at the threshold of viability: an ethical dilemma.

The major ethical issues involved in decision-making in the care of extremely low birth weight newborns are analyzed here. We propose a schema for assessment and management of these infants that is consistent with ethical principles broadly accepted by the pediatric community, and which takes into account mortality risk at any given institution rather than arbitrary weight limits, with a major decision-making role for the infant's parents. When possible, the decision of whether or not to resuscitate should be made before delivery; when not possible, delivery room resuscitation is recommended,

Ethical cognition, AI anxiety, and attitudes toward artificial intelligence: validation of the GAAIS, AT-EAI, and AIAS in an Albanian higher-education context.

BACKGROUND: Cross-cultural psychometric evidence on instruments measuring attitudes toward artificial intelligence (AI) remains scarce in transitional higher-education settings. This study provides the first Albanian-language validation of three widely used instruments - the General Attitudes toward Artificial Intelligence Scale (GAAIS), the Attitudes toward Ethical Artificial Intelligence Scale (AT-EAI), and the AI Anxiety Scale (AIAS) - and, as a secondary exploratory aim, examines how ethical cognition and AI-related anxiety are associated with positive and negative orientations toward AI.

Learning with AI, not from AI: Ethical and pedagogical implications of ChatGPT in undergraduate education.

AIM: To explore how first-year nursing students engaged with ChatGPT during an assessment task, and to understand the experiences, challenges, and perceptions they reported regarding its use in academic work. BACKGROUND: As artificial intelligence (AI) tools like ChatGPT reshape higher education, new opportunities and ethical tensions emerge in how students learn. Rather than simply adopting AI as a shortcut, there is an urgent need to guide students toward learning with AI critically, ethically, and creatively within assessment practices. Despite the growing use of AI, limited research has e

Synthesis using prospective meta-analysis to reduce youths' e-cigarette use (SPARKE): a protocol for an individual participant data prospective meta-analysis (IPD PMA) examining interventions for the prevention of youth e-cigarette use.

INTRODUCTION: Youth electronic cigarette (e-cigarette) use is a global health challenge, with multiple jurisdictions wrestling with appropriate responses, in the face of limited evidence available on effective interventions. Identifying and synthesising evidence on the effects of interventions to prevent youth e-cigarette use is required to inform prevention-focussed health policy and practice. METHODS AND ANALYSIS: We plan to undertake an individual participant data (IPD) prospective meta-analysis (PMA). We will conduct systematic searches to identify eligible planned or ongoing randomised c

Psychosocial safety climate (PSC) and working conditions, predictors of mental health and antidepressant and opioid use in Australia: a study protocol for longitudinal data linkage.

INTRODUCTION: Work-related stress is a social determinant of global health that represents a huge cost to workers' health and reduces work performance. In Australia, mental well-being is a pressing national issue-with one in five Australians experiencing mental disorders. Antidepressants are a first-line medication commonly used to treat mental disorders. Recently, Australia has seen a dramatic increase in the use of prescribed antidepressant medications to treat mental health related illnesses. Australia has also seen a dramatic increase in the use of prescribed opioid analgesics for non-canc

Ethical analysis of the public's perception towards human fetal and embryological collections.

BACKGROUND: Lack of informed consent is the main ethical challenge in human fetal and embryological collections as it violates autonomy, disregards cultural sensitivities, and reflects historical injustices. It is essential that where future frameworks are established, they are codesigned with relevant communities. METHODS: A survey was used to assess the perceptions of the public toward human fetal and embryological collections, with a focus on informed consent. The study analyzed the results through the lens of macro, meso, and micro ethics, with the consideration of chrono and exo ethics.

"Don't Tell Them Anything": Should Surrogate Decision-Makers Be Allowed to Withhold Information from Other Family Members or Prevent Them from Visiting with a Patient?

While patients have the right to control who has access to their health information and designate visitors, it is not always clear whether-when a patient lacks capacity-their surrogate also exercises such rights. States and federal laws are often vague about the limits of surrogate authority. Even where legal or institutional guidance on this issue is clear, requests by surrogates to withhold information or restrict visitation with a patient can be a source of ethical uncertainty and distress on the part of the clinical team. This paper explores the ethical issues raised by such requests. To d

My Diagnostic Odyssey-A Call to Expand Access to Genomic Testing for the Next Generation.

I attended the NSIGHT Ethics and Policy Advisory Board's meeting on sequencing newborns as a research associate in a joint apprenticeship between the University of California, San Francisco, Institute for Human Genetics and the university's Program in Bioethics. But I also came to the meeting with a deeply personal perspective: I had spent nearly my entire childhood in search of a diagnosis and therefore was eager to hear the board's discussion on how to ethically include genomic sequencing early in life. Genomic sequencing in the newborn period could have helped me avoid my diagnostic odyssey

Educators' Perspectives on Generative AI Use in Nursing Education: Friend or Foe?

AIM: This study explores nursing educators' perspectives on the challenges and benefits of integrating generative artificial intelligence (AI) into nursing education. There is little empirical evidence on how educators perceive these technologies and how such perceptions influence their integration into curriculum design, teaching practices, assessment, and student research and learning. DESIGN: Exploratory-descriptive qualitative study underpinned by the Actor-Network Theory. METHODS: Four focus group sessions were conducted with ten nursing educators across Australia, New Zealand, Austria

The other side of the room.

Objective: When college students return to campus housing following psychiatric hospitalization, institutional policies appropriately focus on the returning student, but one stakeholder is systematically overlooked: the roommate. This review examines the evidence gap surrounding roommate impact, analyzes the ethical tensions in current approaches, and proposes policy recommendations. Methods: A scoping review conducted in accordance with PRISMA-ScR. Searches of PubMed, PsycINFO, ERIC, Google Scholar, and ProQuest Dissertations & Theses Global (2000 to 2025) were supplemented by gray literature

[The stigma of vulnerability and the autonomy fallacy in care. Ethical keys from a relational anthropology].

Vulnerability has become a bridging concept capable of traversing clinical bioethics, research ethics, and contemporary discussions on social justice, without a stable consensus on what the term exactly designates, what it normatively justifies, and how it is operationalized in specific contexts. This article offers an argumentative synthesis based on a systematic literature review (January 2015-August 2025) conducted in PubMed, Scopus, and Web of Science. This study, inspired by PRISMA, identifies four thematic areas encompassing the most significant ethical challenges and emerging approaches

Spatial Ethics of an Imposed Euthanasia Clinic Within a Catholic Hospital Space.

Catholic healthcare institutions worldwide are under increasing pressure to participate in euthanasia-often through forced implementation rather than chosen partnership, creating ethical challenges in balancing institutional conscience and theological integrity with their obligations to serve the broader public. This paper develops an interdisciplinary spatial ethics framework of healthcare space grounded in epistemology, environmental psychology, and Catholic moral theology; it examines three spatial dimensions-operational, missional, and moral-and Catholic social teaching principles of subsi

The ethics of child maltreatment surveys in relation to participant distress: Implications of social science evidence, ethical guidelines, and law.

Epidemiological surveys measuring the prevalence of child maltreatment generate essential knowledge that is required to enhance human rights, promote gender equality, and reduce child abuse and neglect and its effects. Yet, evidence suggests Institutional Review Boards (IRBs) may assess the risk of these studies using higher than normal thresholds, based on a perception they may cause high distress to participants. It is essential for IRBs and researchers to have an accurate understanding of the nature and extent of participant distress associated with these studies, and of the duties of resea

Tube-feeding decisions in the elderly.

Tube-feeding decisions in the elderly can be most challenging. We begin our decision making with the premise that artificial nutrition is a medical therapy and therefore that its prescription is based on a calculation of net benefits over burdens for the patient to whom it is given. When the burdens of this therapy outweigh the benefits of prolonged life, tube feeding may be ethically withheld or withdrawn. For the cognitively impaired and in the absence of known patient preferences, the ratio of benefits to burdens may best be figured after a time trial of therapy. If restraints are needed to

Integrating the Common Elements Treatment Approach and motivational interviewing to improve HIV pre-exposure prophylaxis engagement among women who use drugs in Tanzania: protocol for a pilot randomised controlled trial.

INTRODUCTION: Women who use drugs in Tanzania face a disproportionately high burden of HIV and mental health disorders. Despite the availability of pre-exposure prophylaxis (PrEP), uptake remains low, highlighting the need for integrated, scalable interventions that address co-occurring substance use and mental health challenges. Motivational interviewing (MI) and cognitive-behavioural approaches, such as the Common Elements Treatment Approach (CETA), show promise for enhancing HIV prevention outcomes in this population. This study presents the protocol for a pilot feasibility trial assessing
