Management Protocol for Ballistic and Other High-Energy Avulsive Facial Injuries-An Update for the 21st Century.

High-energy ballistic and avulsive injuries to the face represent some of the most complex challenges in modern reconstructive surgery. Since Robertson and Manson's 1999 management protocol, extensive military experience and technological advancements have transformed the treatment principles while preserving the core tenets of staged care. This updated review synthesizes evidence from 36 studies published since 2000, encompassing over two decades of global experience in both military and civilian trauma. Advances in damage-control resuscitation, wound decontamination, and early skeletal stabi

Pediatric burn-related scalp alopecia treated with tissue expansion and the incidence of associated facial burn injuries.

In the pediatric population, cutis aplasia, scalp avulsion, and burn injuries are the leading causes of scalp alopecia that is evaluated for reconstruction by plastic surgeons. Scalp alopecia is seen in 25% of children who suffer burn injuries of the head and neck. These injuries are rarely isolated to the scalp, and the adjacent structures are often affected. This may complicate reconstruction surgery and necessitate multiple reconstructive procedures. A retrospective chart was performed of all cases of staged scalp tissue-expansion cases performed for the reconstruction of burn-related scalp

Iatrogenic Facial Nerve Injury in Head and Neck Surgery in the Presence of Intraoperative Facial Nerve Monitoring With Electromyography: A Systematic Review.

The facial nerve is the seventh of 12 cranial nerves found in the head and neck region that facilitates several nerve fibers and pathways to perform various functions. Iatrogenic facial nerve injury during surgeries of the head and neck is common, ranging from 4-6%, particularly in procedures that involve mobilization or resection of associated anatomical structures. Any injury to the facial nerve or its branches impacts the quality of life and patient satisfaction as the degree of iatrogenic injury may result in partial or complete facial nerve paralysis. Of the various implementable techniqu

Selective Stimulation of Facial Muscles Following Chronic Intraneural Electrode Array Implantation and Facial Nerve Injury in the Feline Model.

BACKGROUND: Our group has previously shown that activation of specific facial nerve (FN) fiber populations and selective activation of facial musculature can be achieved through acute intraneural multichannel microelectrode array (MEA) implantation in the feline model. HYPOTHESIS: Selective stimulation of facial muscles will be maintained in the setting of 1) chronic and 2) acute MEA implantation after FN injury and subsequent recovery. METHODS: This study included seven cats. In three cats with normal facial function, 4-channel penetrating MEAs were implanted chronically in the FN and teste

Recovery from facial paralysis following crush injury of the facial nerve in hamsters: differential effects of gender and androgen exposure.

In recent studies, we have shown that administration of androgens to male hamsters enhances functional recovery from facial paralysis induced by facial nerve crush at the level of the stylomastoid foramen. Furthermore, we have established that the mechanism behind this enhancement of regeneration involves an acceleration of the rate of regeneration, without a shortening in the delay of sprout formation. From those studies, several unexpected findings of inherent sex differences emerged. First, intact (nongonadectomized) females have a faster facial nerve regeneration rate than males and, secon
