Non-operative treatment with immobilization or isometric traction has been abandoned as treatment for fractures of the distal femur at the end of the 1960ies. The technique of open reduction and internal fixation with a condylar plate as suggested by the AO has been the golden standard since the 1970ies. However, anatomic reconstruction of the condylar region with interfragmentary screw fixation and axial realignment of the femur shaft with a plate are challenging procedures especially in the presence of severely compromised soft tissues and put periosteal blood supply at risk. Background/Objectives: For femoral neck fractures (FNFs) treated with hip arthroplasty, cemented femoral fixation is frequently recommended due to its association with reduced early perioperative fracture and reoperation rates. However, newer-generation collared, cementless triple-tapered (CCTT) stems may present benefits compared with conventional press-fit designs. This study sought to assess 30-day survivorship of a CCTT stem in patients undergoing hip arthroplasty for FNF via the direct anterior approach (DAA). Methods: We conducted a retrospective review of all patients who underwent hemi

PURPOSE: Intertrochanteric femoral fractures (IFF) are one of the most common traumatic conditions, but there are no established treatment methods for this condition due to implant failure and re-operation rates. The proximal femoral bionic nail (PFBN), which is a new design of the cephalomedullary nail, was developed by our team. The objective of this study was to assess the clinical and radiographic outcomes of PFBN in patients with unstable IFF.
METHODS: From October 2020 to August 2021, 12 patients diagnosed with unstable IFF (31-A2, 3) were treated with PFBN at the Third Hospital of Hebe…

BACKGROUND: The fractures of femoral neck account for 50% among hip fractures with around 3%-10% occurring in younger population of below 65 years. The newly introduced FNS as management approach appears to be a potential alternative to the traditional CCS. The aim of this study was to compare the clinical efficacy and outcome of the femoral neck system (FNS) and the cannulated cancellous screw (CCS) in the treatment of femoral neck fractures in adult below 65 years of age.
METHODS: Data of 114 patients between 18-65 years, admitted in our department for femoral neck fracture from January 201

Congenital femoral deficiency in children can be treated with femoral lengthening. A common complication is fracture soon after removal of the external fixator, often despite prophylactic hip spica cast application. These fractures present special challenges because the patients have tight soft tissues and sclerotic intramedullary canals. We treated nine such fractures in eight children (average age, 5.4 years). Most were 'spontaneous' events resulting in transverse fracture through regenerate bone or pin sites. All were stabilized with intramedullary Rush pins using special insertion techniqu

Seven patients presenting between 1992 and 1999 with osteoarthritic knee symptoms in consequence of old malunited femoral shaft fractures, were treated by open femoral shaft osteotomy. The osteotomies were sited at the apex of the deformity and were fixed by locked intramedullary nailing. The patients were followed up until osteotomy union and reviewed clinically and radiologically with particular emphasis on symptoms and function. The mean age at presentation was 48 years and the mean time from fracture was 28 years. The mean time to osteotomy union was 28 months. Five of the seven patients r

This study presents a case report of a 54-year-old female patient with osteopetrosis who experienced re-fracture after undergoing osteosynthesis for a femoral shaft fracture. The patient visited our emergency department for a right distal femur fracture. The patient had undergone surgery for a right femoral shaft fracture six years prior to this visit, at which time osteopetrosis was diagnosed at another medical facility. Following three years of postoperative care, successful osteosynthesis was achieved, and the surgical implant was removed. The re-fracture occurred at the site of the previou

Osteoporosis and fragility fractures (FFs) are considered critical health problems by the World Health Organization (WHO) because of high mo … ularly for bisphosphonates (BPs). However, long-term and high adherence to BPs may lead to atypical femoral fractures (AFFs). These latter are tensile side stress fractures of the femur, with high rates of complications, including delayed and non-healing. An effective FLS should be able to prevent both FF and AFF. A comprehensive and interdisciplinary approach, thr …

An 82-year-old woman sustained a trochanteric fracture of the left femur after a fall. Fracture fixation was performed using proximal femoral nail antirotation (PFNA) II, and she was able to walk with a T-cane after 3 months. Eleven months following the operation, the patient presented with left hip pain after a fall. Radiographs showed a subchondral collapse of the femoral head located above the blade tip. The authors removed the PFNA-II and subsequently performed cemented bipolar hemiarthroplasty. Histological evaluation of the femoral head showed osteoporosis with no evidence of osteonecros

PURPOSE: To evaluate fixation failure following femoral neck system fixation (FNS) for intracapsular femoral neck fractures (FNF) and to determine whether fracture characteristics, particularly fracture orientation, were associated with fixation failure.
METHODS: A retrospective cohort study was performed at a Level I trauma center. 82 patients with intracapsular femoral neck fractures treated with FNS fixation were included. Fixation failure was defined as implant cut-out, loss of fixation, nonunion, or revision surgery. Associations between fracture characteristics and fixation failure were

A 9-year-old boy initially presented with a closed metaphyseal transverse fracture of the distal right femur following a fall from a trampoline. Due to the adequate trauma and lack of initial radiological indications for an underlying pathological bone lesion, the fracture was primarily treated with antegrade elastic stable intramedullary nailing (ESIN). Postoperatively, the patient developed persistent pain, progressive soft tissue swelling and highly atypical callus formation accompanied by osteolytic lesions. Following referral to our pediatric trauma center, the radiological diagnostics of

Intramedullary nailing is the gold standard of treatment for atypical femoral fractures, with a few reports of secondary atypical subtrochanteric femoral fractures following intramedullary nailing for atypical diaphyseal femoral fractures. However, there are no reports of secondary atypical diaphyseal femoral fractures following intramedullary nailing for atypical subtrochanteric femoral fractures. A 71-year-old woman with adult-onset Still's disease sustained a right atypical subtrochanteric femoral fracture and was treated with a mid-length intramedullary nail. One year after the surgery, th…

We report our experience in two cases of subchondral insufficiency fracture of the femoral head with a history of internal fixation of the femoral neck fracture. Both cases were young females (26 and 43 years old, respectively). Their hip pain occurred more than 2 years and 1 year after the primary surgical treatment, respectively. MRI revealed bone marrow edema pattern, as well as a low-intensity band on the T1-weighted images, which are commonly observed in osteonecrosis. However, the low-intensity bands were parallel to the subchondral bone end-plate, which are characteristic findings of su

OBJECTIVE: Femoral neck fractures that are associated with femoral shaft fractures have historically been associated with high rates of missed diagnosis. Despite the potentially serious consequences of a missed femoral neck fracture, little work has been conducted to rigorously evaluate the ability of commonly used imaging studies to detect such fractures. Our hypothesis was that axial-view computed tomography is superior to plain radiography at detecting femoral neck fractures.
DESIGN: Blinded assessment of a randomized image set that included axial-view computed tomographic scans of the pel
