Vaginal delivery after hemipelvectomy and pelvic radiotherapy for chondrosarcoma.

Chondosarcoma of the proximal femur is a rare malignant disorder in women of (pre-) childbearing age, for which a radical resection through a hemipelvectomy could be indicated. We describe a case of a 36-year-old primigravida with a hemipelvectomy (2004) who had a history of radiotherapy of the pelvic and uterine regions after an atypical cartilaginous tumour. After an uncomplicated pregnancy, she had a spontaneous rupture of the membranes at 41+2 weeks and an uncomplicated vaginal delivery with physiological development of the infant. On the basis of the literature review, vaginal delivery af

[Hip subluxation and coxa valga secondary to an osteoïd osteoma].

We report on the case of a 2-year-old child with extra-articular osteoïd osteoma of the proximal femur leading to subluxation and coxa valga deformity of the hip joint at 10 year follow-up. The osteoïd osteoma reccured twice after surgical excision and percutaneous drilling. Patient's clinical history also reported a prolonged use of medications as treatment for these two recurrences. Second recurrence was combined with migration of the femoral head and retraction of the adductors: surgical treatment consisted in varus osteotomy to facilitate excision of the lesion, combined with a tenotomy of

Periosteal glomus tumor of the femur: a case report.

Osseous abnormalities produced by glomus tumors located in soft tissues of the periungual region have been described. More rare is the location of a glomus tumor within bone, which usually is located in the phalanx of the fingers. However, to the authors' knowledge, there is no previous description of a glomus tumor located in a periosteal location of a long bone. A 50-year-old man with a glomus tumor in a periosteal location of the lower metaphysis of the femur without neoplastic erosion of the cortical surface is reported. Magnetic resonance imaging and intraoperative ultrasonography were ne

Osteosarcoma arising in heterotopic ossification of dermatomyositis: case report and review of the literature.

A patient with dermatomyositis developed malignant transformation of the benign interfascial heterotopic bone. This patient had classic childhood dermatomyositis at the age of 3 years, and the disease was arrested after a one-year course of corticosteroid therapy. Extensive subcutaneous calcinosis cutis and deep interfascial calcinosis were the residua of the disease. Twenty-eight years later, the patient developed a high-grade osteosarcoma within the benign intermuscular heterotopic calcification and ossification that had been previously documented at the age of 8 years by a roentgenogram and

Osteoid Osteoma of the Femoral Neck in Athletes: Two Case Reports Differentiating From Femoral Neck Stress Injuries.

The diagnosis of an intra-articular osteoid osteoma can be a challenging and lengthy process, with reports of delayed diagnosis of greater than 2 years. In the young, athletic patient with an atraumatic onset of groin pain, an overuse injury or muscle strain is the most likely etiology. However, an overuse injury of femoral neck stress fracture must be identified because of the potentially disastrous outcome of fracture completion. The similar clinical presentation of a femoral neck stress fracture and intra-articular osteoid osteoma of the femoral neck can further delay the diagnosis of the o

[Extensive multinodular schwannoma of femoral nerve].

HISTORY AND PHYSICAL EXAMINATION: A 34-year-old woman contacted her general practitioner due to increasing circumference of her right thigh combined with occasional pain. The physical examination showed multiple nodular masses both in the groin and along the thigh that were painful to touch. The remainder of the physical examination was unremarkable. DIAGNOSIS, THERAPY, AND CLINICAL COURSE: Both on ultrasonography and magnetic resonance imaging (MRI) a nodular mass was found on the right thigh extending from the groin to the inner side of the knee. A tissue biopsy with histological reviewing

[Morphological imaging for response assessment in metastatic bone disease].

Response assessment with morphological imaging in metastatic bone disease is difficult. X-Ray is useful only for long bones. CT is the best imaging modality for assessment of osteolysis and osteosclerosis. MRI is the gold standard for bone marrow morphological imaging. Morphological MR imaging using basic sequences, especially T1, provides confident evaluation of the behaviour of lesions under therapy. Radiotherapy and interventional radiology may induce cortical and bone marrow changes.

Abdominal metastasis of a parosteal osteosarcoma of the femur: an unusual cause of large-bowel obstruction.

BACKGROUND: Parosteal osteosarcoma is a rare, well-differentiated, predominantly fibro-osseous variant of osteosarcoma. It is regarded as a distinct form of osteosarcoma with better prognosis than conventional osteosarcoma. AIM: We report an unusual case of abdominal mass recurrence of parosteal osteosarcoma of the left distal femur treated eight years previously with wide resection, hip disarticulation and chemotherapy, which presented as an acute abdomen: we discuss the clinical outcomes of this rare entity. CASE PRESENTATION: We present a 54-year-old patient with low-grade parosteal osteo

Clinical benefits of femoroplasty: a nonsurgical alternative for the management of femoral metastases.

BACKGROUND: Bone metastases occur frequently in advanced cancer. The spine, pelvis, ribs, skull and femur are the most affected sites. It is reported that up to 83% of the patients develop pain at some point of the disease. The patient can also develop fractures and disability, particularly in the femur.. OBJECTIVES: To evaluate the effectiveness of percutaneous femoroplasty in patients with metastatic osseous disease located in the proximal femur (trochanter, neck, and femoral head). STUDY DESIGN: A retrospective clinical review, comparing pain status "before vs after" intervention. SETTIN

Erdheim-Chester disease as complex clinical presentation and diagnosis: A case report and concise review of literature.

RATIONALE: Erdheim-Chester disease (ECD) is a rare multisystemic disease characterized by the infiltration of multiple organs by foamy CD68 + CD1a-histiocytes. The genetic background consists of gain-of-function somatic mutations in the mitogen-activated protein kinase pathway. The purpose of the present paper is to make a contribution to the scientific literature on ECD by reporting our experience with a complex clinical case report, along with a concise review of the literature. We discussed the unusual clinical presentation, the complex diagnostic process and the comparison with other publi

Extramedullary Lengthening After a Distal Femoral Replacement to Improve Bone Stock and Restore Leg Length: A Case Report.

CASE: A 10-year-old boy with a left femur osteosarcoma treated with a growing distal femoral prosthesis developed a 6.5 cm limb-length discrepancy with erosion of the remaining femur. He underwent revision of the replacement with femoral transport and lengthening using an extramedullary lengthening nail. After treatment, his bone stock increased by 14 cm and his discrepancy decreased to 3.6 cm. He ambulates without assistive device using a shoe insert. CONCLUSIONS: Extramedullary lengthening can increase bone stock and improve leg lengths. Multiple procedures may be required, and deformities

[Multiple pulmonary emboli in a 15-year-old boy with a femoral venous aneurysm].

Pulmonary embolism (PE) in childhood is rare and correlated with high morbidity and mortality, and diagnosis is often delayed. This is a case report of a 15-year-old boy presenting with chest pain, dyspnoea and pain in the right inguinal region, who was found to have multiple pulmonary emboli secondary to a 14 cm long femoral venous aneurysm. Two weeks before he had seen his GP due to dyspnoea, where asthma was suspected. He was treated with low molecular weight heparin but developed recurrent PE and underwent vascular surgery. Clinical suspicion to PE is the key to a rapid diagnosis, treatmen

"Slipped capital femoral epiphysis in a 25-year-old hypogonadic man with a large cranial chondroma: causality or coincidence? ".

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is a hip disorder frequently occurring in adolescence. In adults it is rare and so far very few cases have been documented. CASE PRESENTATION: This report presents a 25-year-old patient diagnosed with an anterior fossa giant chondroma, hypogonadotropic hypogonadism, and SCFE. The patient underwent surgical and hormonal therapy. His symptoms revealed, and he became a father. CONCLUSIONS: Every patient diagnosed with SCFE in adulthood should undergo endocrinological assessment based on physical examination and laboratory tests.

[Non-ossifying fibroma (metaphyseal fibrous defect)].

Non-ossifying fibroma (NOF) or metaphyseal fibrous defect (MFD) is benign fibroblast proliferation with the presence of osteoclast-like multinucleated giant cells. The most cases of NOF/MFD occur in the metaphysis of the long tubular bones of the lower extremities, more commonly in the metaphysis of the femur and in the proximal metaphysis of the tibia. This lesion has a characteristic X-ray pattern and requires no surgical intervention, except for cases of a pathologic fracture or a risk for the latter. The paper analyzes 35 NOF/MFD cases in children and adolescents. It has been found that on

[Solitary distal femoral osteosarcoma in 89-year-old woman. Case report].

The authors describe the case of an 89-year-old patient with a one-month history of pain in the right knee and a pathological fracture of the distal femur. Excisional biopsy showed a classical osteogenic sarcoma. The patient died after six months of palliative surgical and oncological therapy; her primary disease generalised and progressed with numerous metastases. The authors discuss the possibilities of histological diagnostics of osteogenic sarcoma and its differential diagnosis. They draw attention to a possible occurrence of this type of sarcoma also in elderly patients, and not only in a

Femoral mesenchymal chondrosarcoma with secondary aneurysmal bone cysts mimicking a small-cell osteosarcoma.

Mesenchymal chondrosarcoma is a rare but aggressive, high-grade malignancy of primitive cartilage-forming mesenchyme that arises most commonly from skeletal sites. Although there are radiological findings suggestive of the diagnosis, imaging features often overlap with those of other skeletal sarcomas. The definitive diagnosis relies on the histological finding of a typical bimorphic appearance, consisting of nests of small, round, poorly differentiated cells and more mature cartilaginous tissue. To highlight this, we present the case of a 21-year-old man who was referred to our institution wi
